Extended lymphadenectomy in locally advanced rectal cancers: a systematic review

Purpose: The surgical treatment of advanced low rectal cancer remains controversial. Extended lymphadenectomy (EL) is the preferred option in the East, especially in Japan, while neoadjuvant radiotherapy is the treatment of choice in the West. This review was undertaken to review available evidence supporting each of the therapies. Methods: All studies looking at EL were included in this review. A comprehensive search was conducted as per PRISMA guidelines. Primary outcome was defined as 5-year overall survival, with secondary outcomes including 3-year overall survival, 3- and 5-year disease-free survival, length of operation, and number of complications. Results: Thirty-one studies met the inclusion criteria. There was no significant publication bias. There was statistically significant difference in 5-year survival for patient who underwent EL (odds ratio, 1.34; 95 confidence interval, 0.09–0.5; P=0.006). There were no differences noted in secondary outcomes except for length of the operations. Conclusion: There is evidence supporting EL in rectal cancer; however, it is difficult to interpret and not easily transferable to a Western population. Further research is necessary on this important topic.

<P>The electrochemical oxidation of toluene in <I>N</I>-butyl-<I>N</I>-methylpyrrolidinium bis(trifluoromethylsulfonyl)imide ([bmpyr]<SUP>+</SUP>[Ntf<SUB>2</SUB>]<SUP>−</SUP>) was investigated by using cyclic voltammetry and galvanostatic electrolysis in the presence of Co(<SMALL>II</SMALL>) at a Pt disc working electrode. Cyclic voltammetry (CV) investigations revealed that Co(<SMALL>II</SMALL>)–Co(<SMALL>III</SMALL>) oxidation is a diffusion controlled electron transfer process. The diffusion coefficient values of Co(<SMALL>II</SMALL>) were found to increase from 0.38 × 10<SUP>−7</SUP> to 1.9 × 10<SUP>−7</SUP> cm<SUP>2</SUP> s<SUP>−1</SUP> as the temperature was increased from 25 °C to 80 °C. The CV peak current for toluene electro-oxidation increased by nearly 7 fold in the presence of Co(<SMALL>II</SMALL>) demonstrating a good catalytic effect. Prediction of response of reinforced concrete frames exposed to fire

The objective of this work is to study the restraining effect in fire resistance of framed structures and to evaluate the global response of reinforced concrete frames when exposed to fire based on advanced finite element method. To study the response a single portal frame is analyzed. The effect of floor slab on this frame is studied by modeling a beam-column-slab assembly. The evolution of temperature distribution, internal stresses and deformations of the frame subjected to ISO 834 standard fire curve for both the frames are studied. The thermal and structural responses are evaluated and a comparison of results of individual members and entire structure is done. From the study it can be seen that restraining forces has significant influence on both stresses and deflection and overall response of the structure when compared to individual structural member. Among the various structural elements, columns are the critical members in fire and failure of column causes the failure of entire structure. The fire rating of various structural elements of the frame is determined by various failure criteria and is compared with IS456 2000 tabulated fire rating.

A major problem in Switched Reluctance Motor (SRM) is torque ripple, which causes undesirable acoustic noise and vibration. This work focuses on reducing the undesirable torque ripple in SRM by modifying stator and rotor geometry. This paper presents a comparative study on torque ripple minimization in SRM with modified pole shapes such as stator pole taper, stator pole face with non-uniform air gap and pole shoe attached to rotor pole. Further this paper presents a detailed sensitivity analysis of the effect of different geometrical parameters that alter the pole face shapes on the performance of SRM. The analysis is performed using finite-element method considering average torque and torque ripple as performance parameters. Based on the analysis, a design combining stator pole taper with non-uniform air gap is proposed to improve the torque characteristics of SRM. The dynamic characteristics of the proposed design are simulated and the results show satisfactory reduction in torque ripple.

The electrochemical determination of semicarbazide (SCB) executed by using cobalt oxide (Co3O4)nanoparticle modified electrode which was fabricated by using simple precipitation method. The assynthesizednanoparticles have been well characterised by X-ray diffraction (XRD), Furrier transforminfra-red (FTIR), UV-visible (UV-Vis) spectroscopic techniques, energy dispersive analysis of X-ray(EDAX), BET surface area, thermogravimetric (TG) analysis and transmission electron microscopic (TEM)techniques. The XRD shows face centred cubic (FCC) structure, the FTIR demonstrated a major bandsappeared at 574 cm 1 and 669 cm 1 are suggesting the (Co-O) vibrational mode of Co3O4. The TEM ofCo3O4 NPs has been confirmed its ultra-small particle size is of2 nm 0.5 nm. Whereas, EDAX showsthe only cobalt and oxygen are available confirms Co3O4 having high BET surface area. The fabricatedCo3O4 acted as a highly sensitive electrochemical sensor for the determination of SCB by using linearSweep voltammetry (LSV), cyclic voltammetry (CV), electrochemical impedance spectroscopy (EIS) in0.5 M KOH solution. Significantly, anodic onset potential observed at 0.2 V vs. SCE, linear range of scanrate and concentration (1 mM–100 mM) with (LOD 0.13 and LOQ 0.46) and high current and potentialstability with pH dependent behaviour confirms Co3O4 based electrocatalytic system is good for oxidativedetermination of SCB. The selectivity of the sensor also tested by using mixture of other environmentalactive species with SCB by using LSV measurements. This proposed system is applicable in food andpharmaceutical industries for the determination of SCB as an amperometric sensor.
Bilateral Total Hip Arthroplasty in a Rare Case of Multicentric Reticulohistiocytosis

Multicentric reticulohistiocytosis (MRH) is a rare systemic disease, which commonly manifests as muco-cutaneous papulonodules and inflammatory erosive polyarthropathy. In this research, we report the clinical manifestations and management of a rare case of MRH with destructive arthropathy of bilateral hip joints and arthritis mutilans presenting with characteristic deformities. Disabling hip arthropathy that occurs secondary to MRH can be successfully managed with bilateral total hip arthroplasty (THA). Osteopenia and acetabular bone defects must be anticipated during THA. This case is reported due to its rare occurrence and because little literature has been published regarding THA in such patients.

Cyclocondensation of 2-aminothiophenol and aryl/heteryl aldehydes has been carried using biomimetic catalyst, β-Cyclodextrin in water and obtained 2-aryl/heteryl benzothiazoles with better to excellent yields. This biomimetic catalyzed route is simple, economic, and environmentally benign.
