Continuous epidural analgesia has been used widely for treatment of acute abdominal postoperative pain. Neurologic complications related to epidural analgesia occur infrequently but may be caused by various chemical, mechanical, ischemic or idiopathic factors. We report a case of abdominal hysterectomy in which unilateral lower extremity paralysis occurred after continuous epidural analgesia.

Spontaneous intracranial hypotension (SIH) is a rare syndrome which causes postural headache as- sociated with low cerebrospinal fluid (CSF) pressure, without preceding events such as lumbar puncture, back trauma, operative procedure or illness. The headche is usually accompanied by nausea, vomiting, tinnitus, neck stiffness, vertigo, photophobia and in rare cases diplopia, transient visual obscuration. Brain MRI may show subdural effusion, downward displacement of brain and diffuse and continuous meningeal enhancement when enhanced by gadolinium. Low opening pressure was shown through lumbar puncture. The value of protein and cell count of CSF may be slightly elevated. This syndrome usually resolves itself spontaneously or with strict bed rest. When the headache is persistent or incapacitating, more aggressive treatment may be necessary. We treated two cases of SIH in which epidural blood patch produced immediate and complete relief of the symptoms.

결혼이주여성의 개인배경특성, 원가족건강성, 정신건강이 결혼안정성에 미치는 영향

본 연구에서는 결혼이주여성의 개인배경, 원가족건강성, 정신건강 변인과 결혼안정성과의 관계를 살펴봄으로써, 결혼이주여성의 안정적인 결혼생활 한국생활 적응 지원을 위한 기초자료를 얻고자 하였다. 본 연구대상은 전라북도지역의 다문화가족지원센터를 이용하는 결혼이주여성으로 한국어 인식이 가능한 여성들을 대상으로 하여 설문조사 방법으로 실시되었다. 자료분석은 빈도분석, 백분율, 평균과 표준편차, t검증, ANOVA, 위계적회귀분석을 실시하였고, ANOVA는 사후검증을 위해 Duncan Test를 실시하였으며, 신뢰도 검증을 위하여 Cronbach`s α값을 산출하였다. 본 연구에서 얻어진 결론은 다음과 같다. 첫째, 결혼이주여성의 원가족건강성, 정신건강, 결혼안정성의 전반적 경향은 평균이상의 점수 분포로 나타났다. 그러나 정신건강은 하위요인별 차이가 있었는데, 불안·우울감이 사회부적응감보다 상대적으로 낮은 점수를 보이고 있어, 결혼이주여성의 정서적 지원이 좀 더 필요함을 시사하고 있다. 둘째, 개인배경변인에 따른 결혼안정성은 한국문화수용도가 분명한 경우, 월소득 수준이 높은 경우결혼안정성이 높게 나타나 집단간 차이가 있는 것으로 나타났다. 셋째, 개인배경특성, 원가족건강성, 정신건강이 결혼안정성에 미치는 영향에 대한 위계적 회귀분석결과, 종교가 있는 경우, 직업이 없는 경우, 한국문화를 수용할수록, 원가족건강성이 높을수록, 정신건강 수준이 높을수록 결혼안정성이 높은 것으로 나타났다. 이 중 가장 큰 영향을 미치는 변인은 원가족 건강성이었다. 이의 결과는 결혼이주여성의 결혼적응을 높이기 위해서는 문화적 수용도 및 개인적 성장을 위한 교육이나 상담진행시, 결혼이주여성의 원가족 문제를 다룰 필요가 있음을 시사하는 것이라 하겠다. This study aimed to get the basic data for supporting married immigrant women`s stable marriage life and adapting to life in Korea by examining the relationships among the married immigrant women`s personal background, health of family-of-origin, psychological health variables and marital stability. The study subjects were married immigrant women whoseek the services of multicultural family support centers in the Jeollabuk-do region. The results of this study are as follows. First, the health of the family-of-origin, the mental health, the marital stability and general tendencies of these women were revealed to possess a higher than average score. Nevertheless, there was a difference in the sub-elements in that scores in anxiety and depression were relatively lower than those in the sense of an inability to adjust to society. This result suggests that the married immigrant women need additional emotional support. Second, as for marriage stability according to personal background variables, marital stability was high when the monthly income was high and when the degree of accepting Korean culture was distinct. Third, the results of the hierarchical regression analysis on the influences of the personal background variable, family-of-origin health, and psychological health on marital stability showed that marital stability was high when the women followed a religion, were unemployed, and accepted Korean culture, and when their level of psychological health was high as well as the health of their family-of-origin. The variable that had the most significant influence was the health of the woman`s family-of-origin. Such results suggest that in order to increase the marriage adaptation of married immigrant women, it is necessary to address married immigrant women`s family-of-origin problems when conducting education or consulting for the degree of cultural acceptance and personal growth.

보육교사의 가족형태 인식, 수용성 및 가족가치관에 관한 연구

This study is all about child care and to grasp their family cognition. family acceptability, and family value, classify their types depending on family cognition. Two important factors of family cognition we have, 1) Family acceptability - to examine differences of attitudes or ways of your family. 2) Family value - is to identify the value of being importance of each others upon a member of as one family. Family cognition, means to recognize the impotance and value of ther relation as one family. We ha also nuclear family that includes two married parents of opposite gender it has also single parent and remarried families. In Family acceptability nuclear family they were highly acceptive rather than homosexual families. We have 3 types of family circle as adoptive single remarried Families. How ever they showed a significantly low level of acceptability for homosexual families compare to others. As for family value, its sub-factors showed significant differences, that is, view of marriage showed significant differences according to presence of religion and income, premarital cohabitation according to presence of religion, and view of children according to presence of a spouse, presence of children, presence of religion, age, and income.

산모에서 피부로부터 요추부 경막외강까지의 거리

N/A Background: This study was designed to determine the distance from skin to lumbar epidural space in obstetric parturients and whether weight, height, or PI(ponderal index, kg/m) might influence the epidural depth. Methods: 7l obstetric patients undergoing elective cesarean section during epidural anes- thesia in L2-3 level were partitioned into groups according to their prepregnant BMI(body mass index), and in each group weight, height, PI, epidural depth were measured. Results: All patients were classified as underweight(n=l8), normal(n=49), overweight(n= 4) and no one was partitioned into obese group. The distance from skin to lumbar epidural space was found to be 3.7 cm(underweight), 4. l cm(normal), 4.7 cm(over weight) and total mean distance was found to be 4.0cm. The epidural depth had correlations with weight and height in underweight, and weight and PI in normal, but had no correlation with any measurements in overweight group. Conclusion: These results suggest body weight may be a useful parameter for predicting the distance from skin to lumbar epidural space in underweight and normal weight obstet- ric parturients.

N/A Background: Transcutaneous Electrical Nerve Stimulation (TENS) has been widely used for pain relief after surgery instead of opioid analgesics therapy. This study was designed to study the effectiveness of TENS on perianal pain after anal surgery. Methods: Forty-eight patients who underwent anal surgery were evaluated in this suudy. Caudal anesthesia with 1.5% lidocaine 30 ml mixture 1: 200,000 epinephrine was performed. TENS was done at 100 Hz for 25 min. duration, postoperatively at 4 hrs and 24 hrs. Pain was also measured at the 4 hrs and 24 hrs postoperatively by visual analogue scale (VAS). VAS value were compared before and after TENS application. Results: Values of VAS after TENS were significantly lower compared to those before TENS. Conclusions: From these results, we concluded, TENS may be effective for postoperative perianal pain relief after anal surgery.

Spontaneous intracranial hypotension(SIH) is a rare syndrome which causes postural headaches associated with low CSF pressure. Headaches are usually accompanied by on rare occasions diplopia, and transient visual obscuration as well as nausea, vomiting, tinnitus, neck stiffness, vertigo, photophobia, and phonophobia. CSF leakage from a spinal meningeal defect may be the most common cause of this syndrome. Downward sagging of the brain due to lw CSF pressure may produce headaches by traction of intracranial and meningeal pain-sensitive structures. Lumbar puncture reveals low opening pressure. CSF protein and cell count may be slightly elevated. Brain and cervical MRI may show diffuse and continuous pachymeningeal enhancement with gadolinium, subdural effusion, or downward displacement of the brain. We present two patients with SIH, in whom epidural blood patch produced an immediate and complete resolution of the symptoms. We couldn't identify CSF leakage site in either of them.
