소아에서 발생한 슬관절의 결핵성 관절염

Tuberculosis still ranks as one of the three most important infectious diseases in the world in terms of morbidity and mortality. In Korea, an increased incidence of tuberculosis has been observed in recent years. With the resurge of tuberculosis, extrapulmonary tuberculosis has increased, too. From this point of view, tuberculous arthritis affecting knee is rare in all forms of tuberculosis, but we can consider tuberculous arthritis in patients with osteomyelitis. We report the case of a 14-month-old male child who presented with fever and swelling on the left knee joint. Histologic examination of knee joint fluid showed consistent with tuberculosis. Ziehl-Neelsen stain of joint fluid was positive for acid-fast bacilli(AFB). The outcome was favorable after treatment with anti-tuberculosis medication. Because clinical signs and symptoms of musculoskeletal tuberculosis in children is more indolent, we can misdiagnose or delay diagnosis. The diagnosis of tuberculous arthritis can be elusive, necessitating a high index of suspicion. 국내에서는 아직도 소아연령에서 결핵 감염이 적지 않게 존재하기 때문에 폐외 결핵 환자가 발생될 위험도 지속적으로 존재한다. 이런 측면에서 폐외 결핵의 하나인 골관절 결핵은 매우 드물기는 하나, 관절염을 주소로 내원한 환자의 원인으로 고려해 볼만하다. 나이 어린 소아에서는 증상과 징후가 비특이적이기 때문에 오진이나 진단 지연이 우려되므로, 항생제 치료에 잘 반응하지 않는 관절염 환자에서는 결핵성 골관절염에 대한 진단과 치료를 시도하는 것이 중요하다.

GaAs/AlGaAs 3-Quantum Well 양자폭포레이저 (Quantum Cascade Lasers)에서 허용되는 에피정밀도를 위한 활성영역 모의실험

양자폭포레이저에서 활성영역의 모의실험을 위하여 Runge-Kutta 방법과 shooting 방법을 이용하여 슈뢰딩거 방정식의 해를 구하였다. 활성영역의 두께 변화에 대하여 발진파장, 포논공명 에너지, 분극행렬요소 (dipole matrix element) 등의 특성변화를 관찰하였고, 이로부터 양자폭포레이저를 위한 에피성장에서 허용될 수 있는 최소한의 두께 정밀도를 제안하였다. For the simulation of active region in the quantum cascade lasers (QCL), we solved Schrodinger equation utilizing Runge-Kutta method and Shotting method. Wavelength, phonon resonant energy, and dipole matrix element were simulated with the variation of active region thickness. As a result of such simulation, it was suggested the tolerance range of epi-layer thickness error when 3-quantum well QCL structures are grown.

사회적 혼합아파트에 대한 거주자 인식 연구

In Korea, apartment complexes are built according to the social strata for which they are intended to house, thus the buildings tend to separate society both physically and even visually. Because of the sense of social exclusion this has caused the government has conceived a plan to develop a "social-mix" apartment complex master plan. Perhaps the foremost example of this type of plan is the Seoul Eunpyeong New Town. This study examines how resident's perception of the social mix plan has evolved, and also attempts to better understand the effectiveness of the government's attempt at social integration. The result shows that the perception of social-mix housing has improved after residents moved into the Eunpyeong New Town and that people responded positively to the concept of actually mixing residents socially. From the result, we can see that the visually unexposed environmental elements of rental housing and socials exclusion which was felt in the past has been reduced to a certain extent. However, residents living in solid-lot apartments have a negative reaction to social mixing especially when they are in the same building. Therefore, to achieve better social integration in a socially-mixed apartment complex, we need now to change the perception toward socially mixed housing among the residents living in solid-lot apartment buildings.

김남천의 『사랑의 水族館』에 나타난 '현대'의 성격과 '현대청년'

Aquarium of Love written by Nam-cheon Kim is a novel that was published serially from 1939 to 1940 in Chosun Ilbo. It features the characteristics of modern times that isolates the main character. The author also delineated them in his previous work. While Gwang-joon Kim, brother of Gwang-ho Kim, who ended up in dying of acute tuberculosis after abandoning his old-style wife and cohabiting together with a barmaid was .1 prototype of previous generation that implied the background of the times when the ideology of the past socialists collapsed, Gwang-ho Kim, the main character of this work, represents "the characteristics of modern times". Nam-cheon Kim already showed his interest in the way of the world and public morals through the theories of complaint literature and view literature. Accordingly, his delineation of the characters including Gwang-ho Kim and Gyeong'-hee Lee is just a complaint about and view of the then social reality, and the features of their living were the epitome of the social reality of the times. In this work, Nam-cheon Kim explores the love, belief, the way of the world, and public morals of young people who represent "the characteristics of the times" and are represented by Gwang-ho Kim and Gyeong-hee Lee. Outwardly, however, the story progresses with a focus on the plot of conflicts and reconciliation that arise in the course of love and marriage of Gwan-ho Kim and Gyeong-hee Lee. Furthermore, since the real time from 1939 to 1940 and the internal time of the story coincide, the work fully reproduces the sense of reality of the times. The reason why Nam-cheon Kim positively delineates the new senses of "modernized young people" is that vogue is closely associated with productive relationship and it is a means to sensitively view the public morals of 'modern times'.

측두하악장애 환자의 보존적 치료결과의 예측에 관한 연구

This study was performed to predict the conservative treatment outcome of TMD patients by investigating the prognostic factors ; symptom duration, history of previous treatment, history of previous medication, history of trauma, disability of daily activity, severity of pain, noise, limitation of mouth opening(LOM) and maximum comfortable opening(MCO). Two hundreds and fifty-four subjects were selected for this study among the TMD patients who had visited the Dept. of Oral Medicine BNUH and been treated conservatively with medication, physical therapy, behavioral treatment, and splint therapy from 1991 to 2000. The subjects were divided into two groups improved or unimproved according to the treatment response following six months of conservative treatment. Those who showed less than 1 on NAS for pain, TMJ noise, and opening limitation belonged to the improved group and those who showed more than 2 on NAS belonged to the unimproved group. The two groups were compared with respect to symptom severity, number of diagnosis, history of trauma, previous treatment, previous medication, and disability of daily activity. A prognostic equation with the factors revealed to be significantly related to the prognosis of conservative treatment was obtained. The obtained results were as follows ; 1. In improved group, mean duration of history was 12 months, mean treatment duration of a patient was 4 months an mean number of treatment was about 10 times. In other words, in unimproved group, mean duration of history was 27.4 months, mean treatment duration of patient was 10.5 months and mean number of treatment was 19 times. 2. In unimproved group, multiple diagnosis, chronicity, disability of daily activity were significantly greater than that of the improved group. 3. Patients in unimproved group revealed severe noise at first visit and smaller maximum comfortable opening comparatively. 4. Prognostic factors such as duration of treatment, number of treatment, multiplicity, and chronicity and disability of daily activity showed a significant relation in prediction of improvement. 5. Prognostic equation with significant variables is as follows ; Y = 1.984 - 0.251Noise + 0.068MCO - 0.673Multiplicity. - 0.958Chronicity - 0.065Disability. Classification accuracy of 70.3 %, sensitivity of 71.4% and specificity of 66.7% were shown. 6. Prognostic equation with all factors is as follows : Y = 1.599 - 0.038Pain - 0.256Noise - 0.006Limitation + 0.068MCO - 0.580Multiplicity - 1.025Chronicity - 0.720Disability - 0.329Medication - 0.087Treatment + 0.740Trauma. Classification accuracy of 70.3 %, sensitivity of 73% and specificity of 64.3% were shown. 7. Prognostic value of the improved group with significant factors was $1.0446{\pm}1.0726$ and prognostic value of the unimproved group with significant factors was $-0.013{\pm}1.0146$. Prognostic value of the improved group with all factors was $1.0465{\pm}1.0849$ and prognostic value of the unimproved group with all factors was $-0.057{\pm}1.0611$.

양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례

Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

교뇌경색 이후 배뇨장애가 악화된 소양인 환자의 한의 치료 1례 보고

■Objective The purpose of this case report is to report the effect of Korean medicine treatment on a patient with pontine infarction complaining of discomfort due to aggravated voiding dysfunction after the onset of the stroke. ■Methods The patient was hospitalized for 44 days and treated with herbal medicine, acupuncture, electroacupuncture, moxibustion, and interferential current therapy(ICT). We checked the amount of self-voided volume and postvoid residual urine volume(PVR), and asked the patient to subjectively evaluate the degree of discomfort due to voiding dysfunction. ■Results After 44 days of hospitalization, the patient responded that the subjective discomfort due to residual urine remained 30% and delayed urine remained 50% compared to the day of admission. The maximum amount of PVR was 234 ml on the 3^rd day, and the minimum amount of PVR was checked on 25^th and 35^th day as 0 ml. ■Conclusions This study suggests that Korean medicine treatment may be effective in the treatment of voiding dysfunction after stroke.

시호 추출물의 oxLDL 유도 Foam Cell 형성 억제 작용

The oxidative modification of low density lipoprotein(LDL) has been implicated in the development of atherosclerosis. Oxidized LDL(oxLDL) is captured into macrophage and stimulates to form macrophage foam cell. And it can induce an inflammation and smooth muscle proliferation in atherosclerotic plaque. Objective : In this study, we aimed to investigate the effect of Bupleuri radix(SH) on the foam cell formation, a critical initiation stage of atherosclerosis. Methods : To achieve the goal, we examined the effect of SH on LDL oxidation, nitric oxide production in RAW264.7, and the effect of SH on cupuric sulfate-induced cytotoxicity, LDH release, and macrophage activity. Results : SH inhibited the formation of oxidized LDL from native LDL in RAW264.7 cell culture, and decreased the release of LDH from cupric sulfate-stimulated RAW264.7 cell. In other experiments, SH activated RAW264.7 cell, and prolonged the survival time, and inhibited foam cell formation induced by oxLDL in Raw 264.7 cells. Conclusion : These results showed that SH might prevent atherosclerosis by controlling the early stages of foam cell formation.
