Hypoglossal nerve palsy after general anesthesia is an exceptionally rare complication, which has varied etiology. We present a case of unilateral hypoglossal nerve palsy resulting from repeated airway intervention for general anesthesia. A 57-year-old woman was scheduled to undergo modified radical mastectomy. During endotracheal intubation, the patient had Cormack’s grade III—a severe airway condition. After the first intubation attempt failed, the intubation was attempted a second time using a stylet inside the endotracheal tube with cricoid pressure; this attempt was successful. In the evening of the operation day, the patient complained of dysarthria and dysphagia. Physical examination revealed deviation of the tongue to the right, which may have been caused by traumatic hypoglossal nerve injury. This case reviews the pathophysiology, prevention, and management of hypoglossal nerve palsy.
WTO 수산보조금 협상동향 연구 - 제10차 WTO 나이로비 각료회의 쟁점사안을 중심으로 -

The purpose of this study is to examine the issues and the member’s proposals and positions on these issues of the fisheries subsidies sector under WTO/DDA negotiations including the 10th ministerial conference and the negotiating group on rules WTO fisheries subsidies and propose some feasible solutions for the development of fisheries’ industry of the Republic of Korea. There were major issues in this conference, for example, a definition and a scope of prohibitions of fisheries subsidies, special and differential treatment, transitional arrangements, standstill, transparency, surveillance, monitoring and notification related to the fisheries subsidies. The proposals of FFG and ACP included to improve and clarify the definition of the prohibited fisheries subsidies for implement of UN sustainable development goals until 2020 but the other members disagreed on disciplines of fisheries subsidies, therefore the negotiations were ended to be divided without any outputs for agreement regardless of endeavor and existence of most of members’ convergence on this proposal. However, it will be expected to be progress for regulations of fisheries subsidies in the next ministerial conferences or negotiations and to be converged toward achievements on this sector.

Background: Whether thrombectomy benefits differ according to sex remains debatable. We aimed to investigate whether there was a difference in stroke outcomes between men and women treated with thrombectomy.Methods: We studied 173 patients with anterior circulation strokes. Failed recanalization was defined as thrombolysis in cerebral infarction grade 0-2a. Scores >2 on the modified Rankin Scale at 3 months were regarded as poor outcomes. To prove that failed recanalization mediated the association between sex differences and functional outcome, the four steps of the reasoning process adapted from Baron and Kenny’s causal-steps approach were tested. The adjusted odds ratios (aORs) and 95% confidence intervals (CIs) were calculated. Results: This study included 76 women and 97 men. Women were older and presented with atrial fibrillation more frequently than men. Female sex was independently associated with failed recanalization (aOR, 2.729; 95% CI, 1.334–5.582), which was an independent predictor of poor outcomes (aOR, 4.630; 95% CI, 1.882–11.389). Women were associated with poor outcomes in the analysis adjusted for confounders, except for failed recanalization (aOR, 2.285; 95% CI, 1.064–4.906). However, the association became insignificant in the additional analysis adjusted for failed recanalization (aOR, 1.670; 95% CI, 0.738–3.784). The indirect effect between female sex and poor outcomes via failed recanalization was statistically significant (aOR, 1.038; 95% CI, 1.010–1.127). Conclusion: Our study showed that failed recanalization mediated the association between women and poor outcomes after thrombectomy. Nonetheless, this might be explained by chance given our limited study population.
