        변조이음향방사 억제 측정에 미치는 대측 자극 소음 강도의 영향

        As an objective and non-invasive method to evaluate functional state of medial olivo cochlear (MOC) system, there has been growing interest in contralateral suppression of distortion product otoacoustic emission (DPOAE). Also, MOC system can be quantified by measuring the difference DPOAE amplitude recorded in the presence of contralateral acoustic stimulation (CAS). The purpose of this study is to identify the optimal CAS level for measuring the DPOAE contralateral suppression. In order to do that, the effect of gender, direction of ear, and the CAS level of the DPOAE suppression for normal hearing adults were tested. Data were collected from the thirty six ears of the eighteen subjects with normal hearing in the both ears ranged from 21 to 26 years of age. DPOAE was tested by ILO 292 analyzer in DP-gram function. For each measurement, primary tones frequencies were ratio f2/f1 = 1.22(f1 < f2) and intensities were L1 = 65 dB SPL, L2 = 50 dB SPL. 2f1-f2 DPOAE was elicited using two pure tones and frequencies were measured in the range of 1,000 Hz to 6,727 Hz. Broad band noise was used for CAS of DPOAE. CAS was delivered through a TDH 39 headphone that was placed in the participant’s contralateral ear. CAS level was randomly presented at 50, 60, 65, 70, and 80 dB SPL. The results showed that, first, there was no significant effect of the direction of ear for DPOAE suppression (p > .05). The results showed that the DPOAE suppression was not significantly different between male and female (p > .05). However, the means and SDs of DPOAE suppression for 5 different CAS levels, 50, 60, 65, 70, and 80 dB SPL, were .66(±1.20), 1.02(±1.23), 1.47(±1.73), 1.58(±1.70), and 1.21(±1.46) dB. As the CAS levels increased, the majority of DPOAE suppression increased generally until 70 dB SPL. Taken together, the results of this study showed that CAS at different levels induced mainly DPOAE amplitude suppression. The amount of suppression increased as the CAS level was increased with the rolling phenomenon at 80 dB SPL than 70 dB SPL in CAS level. Consequently, the 70 dB SPL or more level appears to generate the acoustic reflex in normal ears, and the 70 dB SPL or less CAS level is expected to be efficient for observing decent DPOAE suppression.

        Effects of the Target Talker Gender and the Number of Competing Talkers on Acceptable Noise Level (ANL) of Korean Normal-Hearing Adults

        본 연구에서는 한국건청인의 수용소음레벨을 측정하고 이에 주요한 영향을 미치는 변수를 확인하고자 하였다. 실험측정변수로는 목표화자성별과 배경화자수를 조절하여 두 변수가 한국건청인의 수용가능한 소음레벨(acceptable noise level, ANL)에 미치는 영향을 알아보았다. 본 연구의 청취대상자는 건청성인 20명(남: 9, 여: 11, 평균연령: 26세)이었다. 한국어 ANL 측정을 위해 한국표준어를 사용하는 성인 6명(남: 3, 여: 3)에 의해 목표어음을 녹음하여 목표화자성별의 영향을 확인하였다. ANL 배경어음은 한국표준어를 사용하는 성인 8명(남: 4, 여: 4)이 신문, 잡지를 약 5분간 읽고 녹음하였다. 녹음된 배경어음의 평균 RMS (root mean squared) 강도를 조절한 후 mixing 작업을 통해 다섯 가지 종류의 배경어음(1-male, 1-female, 2-, 4-, 8-talker maskers)을 생성하였다. 모든 ANL 측정방법은 기존 ANL 측정방법(Nabelek et al., 1991)에 기초하였다. 실험결과 목표화자의 성별에 따라 ANL 결과는 유의미한 차이를 보이지 않아, 목표화자가 남자이든 여자이든 ANL에 큰 영향을 주지 않음을 확인하였다. 그러나 배경화자수가 8명에서 1명으로 수가 적어질수록 ANL이 유의하게 높아졌다. ANL 결과가 높을수록 청자가 더 적은 소음을 허용했음을 의미하므로, 배경화자수가 8명에서 1명으로 감소될 경우 배경어음의 의미가 보다 잘 전달되어 의미적 차폐(informational masking)가 발생하여 적은 소음을 수용할 수 있었던 것이다. 한국, 미국 건청인의 ANL을 비교한 결과 ANL 범위는 비슷하나 대체적으로 한국건청인의 ANL값이 비교적 낮은 편이었다. 향후 다양한 한국 청자군의 ANL을 측정하여 ANL과 청각보조기기 착용 성공여부와의 상관성을 확인하여 그 임상적 활용도를 높이고자 한다.

        이동 궤적을 고려한 링크 상태 예측을 통한 공중 백본 네트워크 라우팅 성능 향상 방법

        The airborne backbone network(ABN) provides communication transport services between airborne nodes, surface nodes and satellite nodes. Such ABN is generally constructed with wide-body and high-capacity planes such as AWACS, which can fly long-term along pre-defined flight paths. In this paper, we propose an efficient method to improve routing performances by reconfiguring routing path before link failure based on the prediction of link state with the information of pre-defined backbone nodes' trajectories. Since the proposed method does not need additional information exchange between airborne nodes in order to acknowledge the link failure, it can be effectively used for airborne backbone network with limited bandwidths.

        주관적 이명의 재활 방법

        Purpose: The purpose of this paper is to provide information about rehabilitation options for subjective tinnitus. While there are several clear treatment options for objective tinnitus, there is no clear treatment option for subjective tinnitus. Although many researchers and clinicians have suggested various rehabilitation options for subjective tinnitus, it is still difficult to determine which rehabilitation options are proper because types of subjective tinnitus vary across patients. Methods: While there are several clear treatment options for objective tinnitus, there is no clear treatment option for subjective tinnitus. Although many researchers and clinicians have suggested various rehabilitation options for subjective tinnitus, it is still difficult to determine which rehabilitation options are proper because types of subjective tinnitus vary across patients. Results: Researchers have suggested several rehabilitation options to reduce effects of tinnitus such as depression, stress, and disturbed sleep. Rehabilitation options for subjective tinnitus are counseling, sound therapy, tinnitus retraining therapy, neuromonics, biofeedback, cognitive behavioral treatment, and mindfulness therapy. Conclusion: This paper will provide useful information about various rehabilitation options for subjective tinnitus which can be applicable in clinics.

        성심농아재활원생들의 청력 실태 조사연구

        Purpose: The purpose of was to assess the hearing impairment status, investigate the situation of aural rehabilitation service, and improve the communicational environment of Sungsim Deaf Rehabilitation Center. Methods: The total members who participated were 58 (mean age: 14.72, age range: 7-36), with 35 males and 23 females. Out of 58 members, only 52 were fitted with hearing devices and 13 had multiple disabilities. Otoscopic examination, pure tone audiometry (PTA), tympanometry, functional gain test, and International Outcome Inventory for Hearing Aids (IOI-HA) were performed. Speech recognition threshold (SRT) and word recognition score (WRS) were measured in the sound field with operating hearing devices. Results: Both of otoscopic examination and tympanometry showed normal at 105 ears (91%). PTA for air conduction thresholds were 109.81 and 110.34 dB HL at right and left ears. Functional gain values were 52.14 dB in a total average of 52 members. According to type of hearing devices, 72.16 dB for cochlear implanted unilaterally (19), 69.03 dB for cochlear implanted bilaterally (3), 61.68 dB for bimodal fitting (6), 32.73 dB for hearing aided unilaterally (6) 32.04 dB for hearing aided bilaterally (18) were revealed in order. The SRT and WRS were 52.41 dB HL and 35%. IOI-HA scores of each item was 3.81 in average showing good satisfaction level of the hearing devices. Conclusion: Conclusively, the fitting status of hearing devices was not sufficient to understand speech sound although their external and middle ears were in good condition. Improving and refitting their hearing devices were recommended and periodical monitoring of hearing and hearing devices were thought to be necessary.

        해외 이명 협회 및 이명 연구 기관 현황

        This paper reviewed the tinnitus associations in the U.S.A., Australia, United Kingdom, European Federation, and one tinnitus research initiative. The American Tinnitus Association (ATA), which was the first tinnitus association in the world established in 1971, has pursued to find a cure for tinnitus and funded vital tinnitus research. The ATA provides informational brochures. The British Tinnitus Association (BTA) aims to support tinnitus patients and to seek effective management of tinnitus. The BTA offers several education programs for clinicians dealing with tinnitus patients in addition to fund tinnitus research. Moreover, the BTA offers direct counsel to tinnitus patients on the phone, and publishes twenty one of informal leaflets. In Australia, two of four tinnitus associations were reviewed; The Australian Tinnitus Association (ATA, Australia TA) and Tinnitus South Association (Tinnitus SA). The Australia TA provides the information about tinnitus, supports tinnitus patients, publishes a tinnitus magazine, and informs people tinnitus treatments through their home page. The Tinnitus SA was established by support of government of South Australia on the purpose of informing people about tinnitus. The Tinnitus SA provides an on-line tinnitus questionnaire, and off-line counselling for patients. The European Federation of Tinnitus Association (EUTI) was recently formed by tinnitus associations from 18 countries within Europe in order to cooperate to help patients. The Tinnitus Research Initiative (TRI) has been established by some research institutes in several countries in order to perform cooperative research and share the results. However, in Korea, there is no tinnitus association except one formed by Korean veteran, which has no information about tinnitus. Considering increasing number of tinnitus patients in Korea, the tinnitus association for helping patients and leading research should be formed in the near future.
