        GaAsAl 레이저 자극이 흰쥐의 압통역치에 미치는 영향

        This study was designed to evaluate the analgesic effect of low power GaAsAl laser on the pain threshold of mechanical stimulation using different treatment points, acupuncture point (zusanli) and non-acupuncture points(back). Furthermore, we investigated the analgesic effect of low power GaAsAl laser using the different duration and intensity of laser in mechanical stimulation induced pain behavior. The results were summarized as follows: 1. The threshold of mechanical stimulation was significantly increased by GaAsAl laser stimulation into zusanli point after 15 and 30 min after laser stimulation(P<05). However, the laser stimulation into non-acupoint did not affect the pain threshold of mechanical stimulation. with dose dependent manner. 2. In order to investigate the analgesic effects of BV depending upon different intensities of laser stimulation, the experimental animals were divided into three groups: 3 mW treated group, 6 mW treated group and 10 mW treated group. The low power GaAsAl laser stimulation was applied into zusanli acupoint for 30 min with different intensity of laser stimulation. Six and ten mW of laser stimulation significantly increased the pain threshold of mechanical stimulation at 15 min after laser stimulation as compared to that of control group(P<.05). Moreover, the analgesic effect of 10 mW laser stimulation was maintained for 30 min after laser stimulation (P<.05). 3. Finally, we tested the analgesic effect of 10 mW laser stimulation using different duration such as 10 min, 30 min or 1 hr after application of mechanical stimulation. In 30 min treatment group, the pain threshold of mechanical stimulation was increased at 15min and 30min after laser stimulation(P<.05). However, laser stimulation for 60 min dramatically increased the pain threshold of mechanical stimulation at 0 min after laser stimulation and the analgesic effect of laser stimulation was observed until 1 hr after laser stimulation. In conclusion, these data apparently demonstrate that low power GaAsAl laser has analgesic effect on mechanical induced pain model in rats. In addition, the treated point, intensity and duration of laser stimulation should be concerned before clinical application for pain management purpose.

        GaAsAl 레이저가 물리적 통증반응과 관련된 척수내 신경세포의 활성에 미치는 영향

        The present study was designed to investigate the effect of low power GaAsAl laser on Fos expression in the spinal cord induced by noxious mechanical stimulation. Noxious mechanical stimulation was applied to the right hind paw following 30min of low power laser treatment using different intensity and treatment point and the resulting Fos expression in the spinal cord dorsal horn was compared to that obtained in rats exposed only to the noxious mechanical stimulation. The results were summarized as follows: 1. In intact control rats, only a few Fos like immunoreactive(Fos-IR) neurons were evident in the lumbar spinal cord dorsal horn. Similarly, following prolonged inhalation anesthesia, Fos-IR neurons were absent in the dorsal horn of the lumbar spinal cord. In animals treated with noxious mechanical stimulation, neurons with nuclei exhibiting Fos immunostaining were distributied mainly in the medial half of ipsilateral laminae I-V at lumbar segments L3-5. These findings directly indicated that prolonged anesthesia used in this study did not affect the Fos expression in the spinal cord dorsal horn of intact animals and noxious mechanical stimulation treated animals. 2. In acupoint treated animals, 10mW of laser stimulation, not 3mW intensity, significantly reduced the number of Fos immunoreactive neurons in the spinal dorsal horn induced by noxious mechanical stimulation(P<.01). However, the supressive effect of low power laser stimulatin was not observed in 3m Wand 10m W of laser stimulation into non-acupoint. These data indicate that 10mW of low power laser stimulation into acupoint is capable of inhibiting the expression of Fos in the dorsal horn induced by noxious mechanical stimulation. In conclusion, these findings raise the possibility that low power laser stimulation into acupoint may be a promising alternative medicine therapy for the mechanical stimulation induced pain in the clinical field.

        Purpose : The aim of this study was to analyze the duties of physical therapists by developing a curriculum( DACUM) method, and to present the frequency, importance and difficulty of the main duties and tasks currently conducted in Korea. Methods : The definition of physical therapists and job description was developed based on DACUM. In order to achieve this analysis, a DACUM workshop, and a survey was conducted on 524 Korean physical therapists working in the whole country 84 areas in order to analyze the order of priority for all tasks. Results : This study defined the task of physical therapists and five kinds of duties were identified : diagnosis and evaluation, intervention, education and counseling, management and community service. Twenty-six tasks and Sixty-three task elements were classified. Conclusions : The results of this study will be of help for the development in clinical education and university curriculum.

        기능적 전기 자극에 대한 고찰

        Functional Electrical Stimulation (FES) is used for muscle reeducation, reduction of spasticity, delay of atrophy, and muscle strengthening. FES stronger stimulation than other forms of electrical stimulation. The efficacy of FES in improving function has been substantiated in the literature. Treatment programs employing FES - activation of muscular tissue through the intact peripheral nervous system - can be broken into five major categories, according to the goal of treatment. These broad areas would include the use of FES to: (1) a direct excitation onto the alpha motoneuron, through peripheral stimulation of the Ia myotatic sensory system and ascending afferent information, which will be integrated at conscious and subconscious level of the CNS. (2) The quality of a stimulated muscle contraction is determined by combination of many parameters, including stimulus amplitude, pulse duration, stimulus frequency, and duty cycle. (3) A unit that has a pulse duration between 200 and $400{\mu}sec$ will be more than adequate for FES applications. (4) The neuromuscular plasticity is critically important to return of function using muscle re-education and facilitation applications. (5) Prior to using FES as an electrical orthosis, the patient should build up endurance in the muscles to be stimu1ated during the gait cycle.

        머리척추각(CVA)이 스마트폰 중독 대학생의 근골격계 통증에 미치는 영향

        The purpose of this study is to examine the effect on musculoskeletal pain, specifically CVA (Craniovertebral angle) among the college students who are addicted to use smartphone. The subjects of study was intended for 30 students who attend D college in Suwon, Gyeonggi Province. Each student was X-rayed provision 900 and measured CVA using QXvue program. Also, each student was asked to respond the smartphone addiction scale questionnaire. Prior to the x-ray measurement, each student was asked to stare at his/her eyes in a mirror to keep the nature head posture and head in a same position. Then the X-ray the side the head and neck were used to be taken pictures. The analysis of data was performed using the spss 18.0, craniovertebral angle. Then examined the relationship of pain and craniovertebral angle by two sample t-test. Also performed variance analysis to investigate the relationship between craniovertebral angle and pain. The significance level for all statistical tests was estimated to be within 5%. The results are as follow; 1. Statistically, CVA angle in relation to the accompanying pain was significant. In other words, CVA angle was higher in the group with the pain. 2. The significance of relations between CVA angle and pain area was confirmed. Post-hoc test results hands/wrists and waist group statistically significant while the remaning two groups were not significant between.

        Lymphocytic interstitial pneumonia (LIP) is a benign lymphoproliferative disorder characterized histologically by diffuse infiltration with predominantly mature lymphocytes, plasma cells and reticuloendothelial cells in the alveolar septae and along the lymphatic vessels. LIP is an uncommon disease frequently associated with autoimmune diseases such as Sjogren`s syndrome, systemic lupus erythematosus, chronic active hepatitis, thyroiditis, myasthenia gravis, autoimmune hemolytic anemia and Human Immunodeficiency Virus (HIV) or Epstein-Barr virus (EBV) infection. LIP can progress to pulmonary or systemic lymphoma. LIP in children has variable disease courses such as spontaneous resolution, episodic worsening or progression to frank respiratory failure. Intravenous immunoglobulin and corticosteroids yield variable results, and some children may respond to them for several years. We report herein a case of EBV-associated, HIV-negative LIP diagnosed by thoracoscopic lung biopsy and a clonality study in a 5 year-old boy who was admitted with recurrent coughing and dyspnea. The patient was treated and fully recovered with methylprednisolone and high dose acyclovir. At the 3-year follow-up, he showed no clinical symptom despite increased EBV copy in bronchoalveolar lavage and progressive bronchiectasis in the right middle lobe.         Background: Atopic dermatitis (AD) is a chronically relapsing allergic skin inflammatory disease with a high incidence in the early childhood. Staphylococcus aureus colonization (S. aureus) is frequent in the skin of patients with AD and is one of the important environmental factors that develop or exacerbate this disorder. Objective: The aim of this study was to investigate the different clinical features of AD with S. aureus colonization and to seek its relation with the clinical features of AD. Method: We recruited 342 children with AD who visited the pediatric allergy clinic in Asan Medical Center from July 2003 to January 2007. They were divided into 2 groups based on the results of skin cultures: group 1 with no bacteria cultured and group 2 with S. aureus cultured. The clinical and laboratory data, such as SCORAD (SCORing Atopic Dermatitis), total eosinophil count (TEC), eosinophil fraction (%), serum total IgE, serum eosinophil cationic protein (ECP) and staphylococcal enterotoxin A and B -specific IgE levels, were evaluated and were compared between individual groups. Result: There was a statistical difference between groups 1 and 2 in TEC, eosinophil fraction (%), total IgE, serum ECP and SCORAD index. There was a significant difference in total IgE between the enterotoxin-negative and enterotoxin-positive groups. S. aureus colony counts were significantly correlated with total eosinophil count (r=0.259, P= 0.003), eosinophil (%) (r=0.196, P=0.009), and serum ECP (r=0.212, P=0.035). Total IgE levels were significantly correlated with specific IgE levels to enterotoxins A and B (r=0.335, P=0.012; r=0.393, P=0.002). S. aureus colonization and specific IgE levels to staphyloccal enterotoxins A were correlated with SCORAD index and total IgE levels. Conclusion: Skin infections with S. aureus may be associated with pathogenesis of AD. (Korean J Asthma Allergy Clin Immunol 2008;28:105-112)
