For the development of new cephalosporin antibiotics with aminothiazolcarboxymethylethoxyimino moiety on the C-7 position and tetrazolymethyl moiety on the C-3 position of cephem ring, 70-[(Z)-2-(2-aminothiazol-4-yl)-2-(1-carboxy-1-methylethoxyimino)acetamido]-3-[5-(substituted)tetrazol-2-yl]methyl-3-cephem-4-carboxylic acids(28-35) were synthesized. These compounds were tested for antimicrobial activity in vitro against Gram(+) and Gram(-) bacteria. They showed remarkable antibacterial activity against Escherichia coli AB 1157, Escherichia coli AB 0111, Escherichia coli BE 1186, Micrococcus luteus ATCC 9341, Salmonella typhimurium TV 119, Salmonella typhimurium SL 1102, Staphylococcus aureus IFO 12732, Staphylococcus aureus R-209, but these compounds were not active against Pseudomonas aeruginosa N-10.

당뇨병성 케톤산증은 내분비적 응급질환으로 유발 요인 제거 및 조기 치료를 필요로 하며, 갑상선중독증에 의해 유발된 당뇨병성 케톤산증은 심각한 합병증이 발생할 수 있으므로, 특히 제1형 당뇨병에서 발병된 당뇨병성 케톤산증에서는 갑상선중독증에 의해 유발될 수 있음을 반드시 고려해야 할 것이다. It is rare, but thyrotoxicosis may precipitate diabetic ketoacidosis. Cases of the simultaneous presentation of thyrotoxicosis and diabetic ketoacidosis have been reported and a delayed diagnosis result in serious complications such as cardiac arrest or death. Thyroid hormone has been well-documented in patients with glucose intolerance and thyroid hormone`s lipolytic effects on adipocytes may promote hepatic beta-oxidation. We report here on a case of diabetic ketoacidosis that was precipitated by thyrotoxicosis in a woman with type 1 diabetes mellitus. We propose that thyrotoxicosis is a significant contributor to the development of diabetic ketoacidosis. (Korean Clinical Diabetes J 10:115-117, 2009)

A jejunal ectopic pancreas, where pancreatic tissue is found outside of the usual anatomical location, is a rare submucosal tumor that may cause obscure gastrointestinal (GI) bleeding. After initial negative endoscopic evaluation of the obscure GI bleeding, including colonoscopy and/or upper endoscopy, it is reasonable to proceed with further evaluation of the small bowel. Diagnostic options for the evaluation of the small bowel may include capsule endoscopy, push enteroscopy, or barium contrast small bowel studies. Here, we report a case of obscure GI bleeding caused by a jejunal ectopic pancreas, diagnosed through capsule endoscopy and barium contrast small bowel studies, which was treated successfully with single incision access laparoscopy. Quaternary Ammonium Salt Cephalosporin계 抗生劑 合成

For the development of quaternary ammonium salt cephalosporin antibiotics with improved antimicrobial activities, a series of 7β-[(Z)-2-(2-aminothiazol-4-yl)-2-(methox yimi no) acetamido]-3-[4-alkyl (or aryl)-3- thiosemicarbazid-1- nicotinonium] methyl -3- cephem -4- carboxylate were synthesized. Antimicrobial activities of these new compounds were examined against Gram(+) and Gram(-)bacteria. It was found that the moiety synthesized cephalosporin derivatives were good antibacterial activities against Escherichia coli. Almost these compounds were more antibacterial activities than cefazoline. These new compounds showed lower activity than cefotaxime against Pseudomonas aeruginosa.
