Background: Recently dermoscopic characteristics mainly vascular structures of molluscum contagiosum (MC) have been described and there has been no study on dermoscopic features of central located white structures. Objectives: To evaluate the dermoscopic findings of MC based on white structures and vascular structures. Methods: After confirming presence of Henderson Paterson bodies under a microscope dermoscopic images were taken and evaluated. Results: We evaluated 178 lesions. Vascular structures found in 167 lesions (93.8%) and were classified into single vascular patterns and mixed patterns. Crown or crown+radial arrangement accounts for 77.6%. White structures found in 175 lesions (98.3%) were white circle (33.7%) fourleaf clover like (28.7%) polylobular (22.5%) and unspecific structure (13.5%). Fourleaf clover like structure shows tendency to be a feature of smaller lesions while polylobular structure shows tendency to be a feature of larger lesions. Also some polylobular structures were consist of fourleaf clover like and white circle like lobules. It is possible to explain that initial smaller lesion which has fourleaf clover like structure may proliferate into larger lesion which has polylobular structure. Conclusion: In addition to typical vascular patterns recognition of morphology of white structures could aid diagnosis of MC.
자기회귀시차분포 모델을 적용한 병원 건물의 시간별 난방부하 예측

This paper is intended to develop the prediction model for the hourly heating load of a building, applying an autoregressive distributed lag (ARDL) model, which reflecting the dynamic property of time series data. The information on the outside weather obtained from the meteorological office, and occupancy is only used for the modeling. The prediction accuracy of the ARDL model was 28.3 % CVRMSE (Coefficient of Variation of Root Mean Squared Error) without overall bias. This is an improvement of 21.2 % in comparison with the multiple linear regression (MLR) model, not having the time lagged terms of variables. It is confirmed that there is little nonlinear relationship between response and explanatory variables, when judging from the result of an artificial neural networks (ANN) model using the same input variables to the ARDL model. The effects of influencing factors such as outside temperature, solar radiation and occupancy on the hourly heating loads were analyzed through the coefficients of the ARDL model. The prediction model in this paper can be applied as the baseline for evaluating the future changes of the building heating loads.

<P><B>Abstract</B></P> <P>A ground source heat pump system (GSHP) with 450 RT capacity composed of ten heat pump units provides the heating and cooling energy to an entire hospital building. The seasonal heating performance of 3.21 and system operation properties of the system were analyzed using <I>in situ</I> monitoring data from Nov. 2016 to Mar. 2017. On this basis, hourly GSHP system performance prediction models applying a multiple linear regression (MLR) and an artificial neural network (ANN) were developed. The quantitative effects of influencing variables on the system performance, including the entering source and load water temperatures (EST, ELT) were analyzed by elaborated MLR model with statistical significance. The prediction accuracy was 3.56% by the MLR, and 1.75% by the ANN, based on the coefficient of variation of root mean squared error (CVRMSE) without overall bias. <P><B>Abstract</B></P> <P>Adherent-invasive <I>E. coli</I> colonization and Toll-like receptor (TLR) expression are increased in the gut of inflammatory bowel disease (IBD) patients. However, the underlying mechanism of such changes has not been determined. In the current study, it was examined whether gut serotonin (5-hydroxytryptamine, 5-HT) can induce adherent-invasive <I>E. coli</I> colonization and increase TLR expression. In a co-culture system, commensal <I>E. coli</I> strain (BW25113, BW) adhered minimally to colon epithelial cells, but this was significantly enhanced by 5-HT to the level of a pathogenic strain (EDL933). Without inducing bacterial virulence, such as, biofilm formation, 5-HT enhanced BW-induced signaling in colon epithelial cells, that is, NADPH oxidase (NOX)-dependent superoxide production, the up-regulations of IL-8, TLR2, TLR4, and ICAM-1, and the down-regulations of E-cadherin and claudin-2. In a manner commensurate with these gene modulations, BW induced an increase in NF-κB and a decrease in GATA reporter signals in colon epithelial cells. However, 5-HT-enhanced BW adhesion and colon epithelial responses were blocked by knock-down of NOX2, TLR2, or TLR4. In normal mice, 5-HT induced the invasion of BW into gut submucosa, and the observed molecular changes were similar to those observed <I>in vitro</I>, except for significant increases in TNFα and IL-1β, and resulted in death. In dextran sulfate sodium-induced colitis mice (an IBD disease model), in which colonic 5-HT levels were markedly elevated, BW administration induced death in along with large amount of BW invasion into colon submucosa, and time to death was negatively related to the amount of BW injected. Taken together, our results demonstrate that 5-HT induces the invasion of commensal <I>E. coli</I> into gut submucosa by amplifying commensal bacteria-induced epithelial signaling (superoxide production and the inductions of NOX2 and TLR2/TLR4). Hereditary spherocytosis is the most common form of congenital hemolytic anemia, which is characterized by spheroidal red blood cells, increased osmotic fragility and splenomegaly. The development of protein research revealed various primary defect resides in the erythrocyte membrane proteins of the spherocyte. Recently, we studied a 36-year-old male hereditary spherocytosis patient with anemia and splenomegaly, Analysis of the erythrocyte membrane proteins by SDS-PAGE showed a deficiency of protein 4.2.
