Complications Related to Gastric Endoscopic Submucosal Dissection and Their Managements

Endoscopic submucosal dissection (ESD) for early gastric cancer is a well-established procedure with the advantage of resection in an en bloc fashion, regardless of the size, shape, coexisting ulcer, and location of the lesion. However, gastric ESD is a more difficult and meticulous technique, and also requires a longer procedure time, than conventional endoscopic mucosal resection. These factors naturally increase the risk of various complications. The two most common complications accompanying gastric ESD are bleeding and perforation. These complications are known to occur both intraoperatively and postoperatively. However, there are other rare but serious complications related to gastric ESD, including aspiration pneumonia, stenosis, venous thromboembolism, and air embolism. Endoscopists should have sufficient knowledge about such complications and be prepared to deal with them appropriately, as successful management of complications is necessary for the successful completion of the entire ESD procedure.

Background/Aims: Eosinophilic esophagitis (EoE) is often erroneously diagnosed as gastroesophageal reflux disease (GERD). The aim of this study is to investigate the prevalence of EoE and the expression of tight junction (TJ) proteins in patients with GERD symptoms. Methods: One hundred patients with GERD symptoms and 10 healthy controls were prospectively studied. Sixty-two patients had symptoms refractory to proton pump inhibitors (PPI). All patients underwent esophageal biopsy. Patients were diagnosed with EoE if the number of eosinophil granulocytes per high-power field was ≥15. Immunohistochemical analysis of TJ proteins (claudin-1, claudin-4, occludin, and zonula occludin-1 [ZO-1]) was performed. Results: EoE was diagnosed in six of 100 patients (6%) with GERD symptoms and in six patients (9.7%) of 62 patients with PPI-refractory GERD. Only one had typical EoE endoscopic findings. The proportion of ZO-1-positive cells was significantly lower in the lower than in the middle esophagus (56.0%±14.0% vs 66.0%±11.5%, p<0.05). There were no significant correlations between TJ protein expression and GERD symptoms. Conclusions: The prevalence of EoE among patients with PPI-refractory GERD is approximately 10%. Regardless of endoscopic findings, esophageal biopsy is crucial in diagnosing EoE. The disruption of ZO-1 expression in the lower esophagus is significantly associated with GERD symptoms. Electrical properties of fuel cell based on DNA film

We have investigated the electrical properties of a fuel cell based on DNA lm. It is found that open-circuit voltage appears in theDNA lm, optimum condition exists. From the analyses of the frequency dependence of electrical conductivity, it is also found that elec-trical conductivity consists of the component of electrical conductivity caused by the dielectric dispersion, and the component of DCelectrical conductivity. Furthermore, we have found that the ratio,d, of DC conductivity to the component caused by dielectric disper-sion becomes large in the middle-humidity region between 17% and 60% and reaches a maximum at approximately 57% where open-loss are achieved. From these results, it is deduced that optimum conduction of the fuel cell based on DNA lm is determined bynot only the proton conductivity but also by the dielectric loss accompanied by the dielectric dispersion.

Estimating above-ground biomass is important in establishing an applicable methodology of Measurement, Reporting and Verification (MRV) System for Reducing Emissions from Deforestation and Forest Degradation-Plus (REDD+). We developed an estimation model of diameter at breast height (DBH) from IKONOS-2 image that led to above-ground biomass estimation (AGB). The IKONOS image was preprocessed with dark object subtraction and topographic effect correction prior to watershed segmentation for tree crown delineation. Compared to the field observation, the overall segmentation accuracy was 64%. Crown detection percent had a strong negative correlation to tree density. In addition, satellite-based crown area had the highest correlation with the field measured DBH. We then developed the DBH allometric model that explained 74% of the data variance. In average, the estimated DBH was very similar to the measured DBH as well as for AGB. Overall, this method can potentially be applied to estimate AGB over a relatively large and remote tropical forest in Northern Borneo.

Colorectal endoscopic submucosal dissection (ESD) is now a well-established endoscopic treatment for early-stage colorectal neoplasms, especially in Asian countries, including Japan. Despite the spread of colorectal ESD, there are still situations in which achieving successful submucosal dissection is difficult. Various novel techniques and devices have been developed to overcome these difficulties, and past reports have shown that some of these strategies can be applied to colorectal ESD. We review several recent developments in the field. The techniques reviewed include the pocket creation method and traction methods and the devices reviewed include the overtube with balloon and electrosurgical knives with water-jet function. These improved techniques and devices can facilitate safer, more reliable ESDs and expand its applicability and acceptability all over the world.
