S-281 : Malignant fibrous histiocytoma presenting with hemothorax: a case report

Malignant fibrous histiocytoma (MFH), among the most common soft tissue sarcomas in adults, develops most commonly in the extremities (70-75%), followed by the trunk and retroperitoneum. Development of MFH on the chest wall and pleura is very rare. We report a case of a 72-year-old woman in whom MHF was detected incidentally after she had a traffic accident. A 72-year-old woman complained of right-sided chest pain and orthopnea following a traffic accident that occurred 1 month previously. Chest radiography and computed tomography revealed large amounts of right pleural effusion and a pathological seventh rib fracture. At the previous hospital, the patient underwent thoracentesis for the aspiration of hemorrhagic fluid. She was then diagnosed with traumatic hemothorax and a closed thoracostomy was performed. Laboratory examination of the pleural effusion revealed that it was exudative, and had an adenosine deaminase level of 15.7 IU/L, white blood cell count of 980/mm3 (lymphocyte count 80%), and red blood cell count of 6,560/mm3. Except for the level of cancer antigen 125, the levels of tumor markers such as carcinoembryonic antigen and cytokeratin-19 fragment remained within the normal range. After drainage of 1.5 L of the bloody pleural effusion, the dyspnea and chest pain improved. Chest radiography performed 7 days after closed thoracostomy revealed a large amount of the remaining pleural effusion. Thoracentesis was performed for the aspiration of a sticky hemorrhagic fluid. We then performed video-assisted thoracoscopic surgery for the persistent retained hemothorax. However, lung decortication and rib resection had to be performed because of the presence of a very large mass at the incision site. The mass on the chest wall and pleura was confirmed as a pleomorphic MFH. Thus, the large pleomorphic MFH was initially misdiagnosed as traumatic hemothorax. In conclusion, we should consider the possibility of malignancy in patients in whom pleural effusion remains, even in patients who are strongly suspected of having traumatic hemothorax.

Background: This study was conducted in order to elucidate the effects of docetaxel on the growth of peroxiredoxin 1 (Prx1) knockdown A549 xenograft tumors and further tested the role of Prx1 as a predictor for how a patient would respond to docetaxel treatment. Methods: Effects of docetaxel on the growth of scrambled- and shPrx1-infected A549 xenograft tumors in nude mice were measured. Moreover, immunohistochemical expression of Prx1 was evaluated in paraffin-embedded tissues from 24 non-small cell lung cancer patients who had received docetaxel-cisplatin regimens as a first-line treatment. Results: Docetaxel treatment in Prx1 knockdown xenograft tumor resulted in reduced tumors growth compared with other groups. Prx1 knockdown increased the production of cleaved caspases-8 and -9 in the control itself compared to scramble tumors. Moreover, docetaxel treatment in Prx1 knockdown tissue led to an increased protein band. Phosphorylated Akt was found in Prx1 scramble tissues. Phosphorylated FOXO1 was detected in the docetaxel treatment group. On the other hand, Prx1 knockdown completely suppressed the Akt-FOXO1 axis. The median progression-free survival (PFS) of patients with low Prx1 expression was 7 months (95% confidence interval [CI], 6.0-7.7), whereas the median progression-free survival of patients with high Prx1 expression was 4 months (95% CI, 4.0-5.0). However, high Prx1 expression was not associated with decreased PFS (p=0.114). Conclusion: Our findings suggest that elevated Prx1 provides resistance to docetaxel treatment through suppression of FOXO1-induced apoptosis in A549 xenograft tumors, but may not be related with the predictive significance for response to docetaxel treatment.

Previously, papiliocin was isolated from the swallowtail butterfly Papilio xuthus and its antimicrobial activity was suggested. In this study, the antifungal mechanism of papiliocin against Candida albicans was investigated. Confocal laser scanning microscopy (CLSM) and 1, 6-diphenyl-1, 3, 5-hexatriene (DPH) fluorescence analysis indicated that papiliocin disturbed the fungal plasma membrane. Moreover, the assessment of the release of FITC-dextran (FD) from liposomes further demonstrated that the antifungal mechanism of papiliocin could have originated from the pore-forming action and that the radius of the pores was presumed to be anywhere from 2.3 to 3.3 nm.

<P>In this paper, we consider the CSMA/CA multihop networks where the two end-nodes transmit their packets to each other and each intermediate node adopts network coding for delivering bidirectional flows. In addition, the neighbor nodes are randomly uniformly deployed with the Poisson Point Process. By varying the combination of the physical carrier-sensing range of the transmitter node and the target signal-to-interference ratio (SIR) set by the receiver node, we can control the interference level in the network and the degree of spatial reuse of a frequency band. The larger the carrier-sensing range is, the smaller the interference level, while the smaller the opportunity of getting a channel by a node. Similarly, the higher the target SIR value is, the more probable the retransmission (by the exponential random backoff) is, while the better the link quality on successful transmission is getting. Under this tradeoff context, we find the optimal combinations of these two factors that make the end-to-end throughput of the flow maximal for three different retransmission schemes.</P>

Malignant fibrous histiocytoma (MFH), a type of sarcoma, is a malignant neoplasm with uncertain origins that arise from both the soft tissues and the bone. The occurrence of MFH on the chest wall is extremely rare. We hereby report a case of a 72-year-old woman who was incidentally detected with MFH after a traffic accident.

The diagnosis of acute colonic diverticulitis and the assessment of its severity may be difficult. Especially, in the elderly patients with it, immediate diagnosis and initiation of therapy greatly influence the outcome. Pericolonic abscess and fistula formation, perforation, luminal obstruction, and peritonitis are well- known complications of diverticulitis. Rarely, the inflammatory process can extend into the venous drainage of the affected portion of the colon. Ultrasonography is a newly used imaging modality in patients under the suspicion of acute colonic diverticulitis and its complications. We experienced a case of colonic diverticulitis of which delayed diagnosis resulted in the development of portal and mesenteric vein gas and thrombosis. The literature on portal and mesenteric venous gas associated with colonic diverticulitis is reviewed, and the usefulness of ultrasound as a rapid, noninvasive tool to diagnose this rare condition is discussed.
