        과체중 여성에서 L-carnitine과 Isoflavone 보충 섭취가 체중 및 내장지방 감소에 미치는 영향

        This study was performed to examine the combined effects of L-carnitine and isoflavone supplementation on weight reduction and body fat distribution in overweight women. Overweight/obese women (body mass index > 23 ㎏/㎡) who were not diagnosed any type of diseases were included in this study and sixty subjects (41.1 ± 1.5 years, 25.9 ± 0.3 ㎏/㎡) were randomly assigned to a placebo (n = 30) or a supplement group (n = 30, L-carnitine 300 ㎎+isoflavone 40 ㎎/day). We measured anthropometric parameters, abdominal fat distribution by computerizd tomography and blood components before and after the 12 week intervention period. After the 12 weeks of supplementation, subjects in L-car-nitine and isoflavone supplement group showed a significant reduction of body weight (p < 0.001), body fat % (p < 0.05), and waist to hip ratio (p < 0.01) whereas placebo group did not show any changes. In a CT-scanned results, total fat area at L4 level was significantly reduced by 8.1 % (p < 0.01) with the reduction of visceral fat area (-11.1 %, p < 0.001) and subcutaneous fat area (-7.0%, p < 0.05) in the supplement group. The supplementation of L-carnitine and isoflavone showed the significant improvement of HDL-C (p < 0.01) and apoB (p < 0.05) concentrations, however, change values in those markers were not significant compared with those of the placebo group. In addition, a significant increase of adiponectin level (p < 0.001) was observed in the supplement group after the intervention. The result of present study demonstrated that supplementation of 300 ㎎ L-carnitine and 40 mg isoflavone per day for 12 weeks can give beneficial effects on weight reduction and visceral fat accumulation. These potential antiobesity supplement can produce more favorable effects when combined with lifestyle modification.

        <P>The structure and function of the Antarctic marine diatom <I>Chaetoceros neogracile</I> antifreeze protein (Cn-AFP), as well as its expression levels and characteristics of the ice-binding site, were analyzed in the present study. <I>In silico</I> analysis revealed that the <I>Cn-AFP</I> promoter contains both light- and temperature-responsive elements. Northern and Western blot analyses demonstrated that both Cn-AFP transcript and protein expression were strongly and rapidly stimulated by freezing, as well as temperature and high light stress. Immunogold labeling revealed that Cn-AFP is preferentially localized to the intracellular space near the chloroplast membrane. Recombinant Cn-AFP had clear antifreeze activity. Protein-folding simulation was used to predict the putative ice-binding sites in Cn-AFP, and site-directed mutagenesis of the Cn-AFP b-face confirmed their identification.—Gwak, Y., Jung, W., Lee, Y., Kim, J.         만성 족관절 불안정성을 가진 환자군에서 변형 브로스트롬 술식과 봉합 테이프를 추가한 술식 간의 결과 비교

        Purpose: The modified Broström repair (BR) technique has yielded good outcomes in patients with chronic ankle instability. This study compared clinical and radiological outcomes between two groups of patients who underwent modified BR or lateral ligament augmenta- tion using suture tapes (ST). Materials and Methods: Seventy-seven patients (ST group [n=47], BR group [n=30]; body mass index <26.61 kg/m2; mean age, 30.7±11.0 years [range, 17~39 years]; mean follow-up, 34.0±12.0 months [range, 24~59 months]) were retrospectively reviewed between January 2014 and July 2017. The Foot and Ankle Outcome Score (FAOS), American Orthopedic Foot and Ankle Score (AOFAS), Foot and Ankle Ability Measure (FAAM), visual analogue scale (VAS) score, and Sefton grading system were used for clinical assessment. The talar tilt angle and anterior talar translation were measured using the Telos stress device (Telos GmbH, Marburg, Germany) at 150 N for radiological evaluation. Results: FAOS, AOFAS, FAAM, and VAS scores improved in both groups at final follow-up (ST, 91.1±5.2, 93±2, 88.1±4.5, 1.5±0.7 vs. BR, 91.3±5.4, 93±3, 83.3±4.8, 1.2±0.7, respectively; p=0.854, 0.971, <0.001, 0.04, respectively). According to the FAOS, mean sports ac- tivity scores for the ST and BR groups at the final follow-up were 90.3±3.2 and 76.6±4.2, respectively, reflecting superior outcomes in the ST group (p<0.001). Sefton grading revealed satisfactory functional outcomes (ST, 91.5% vs. BR, 90.0%) . There was significant improve- ment in the talar tilt angle and anterior talar translation in both the ST and BR groups (7.6°±1.2°, 10.5±1.8 mm vs. 4.9°±1.1°, 7.9±1.5 mm, respectively; p<0.001). Conclusion: The ST group demonstrated comparable clinical but better improvement in mechanical stability and FAOS sports scores than the BR group.

      • D2AS Score Predicts Development of Hepatocellular Carcinoma in Chronic Hepatitis B Who Are Outside the Current Treatment Recommendations

        Aims: As antiviral therapy for chronic hepatitis B (CHB) reduces therisk of hepatocellular carcinoma (HCC), ideally, no HCC should developin those who are not recommended for therapy. Yet, HCC developmentwho were outside of current treatment recommendation hasbeen reported.Methods: A simple HCC risk score was developed from 971 patientswith CHB with elevated HBV DNA levels who were outside the currenttreatment criteria due to having normal or mildly elevated alanineaminotransferase (ALT) levels (of whom 26 patients developed HCCduring follow-up). Variables included in the risk score were serumHBV DNA level (used twice), age, and sex (D 2AS score). The scorewas validated from an independent cohort of 507 patients (of whom15 patients developed HCC).Results: A 4-point risk scale was developed, with HCC risk rangingfrom 0-29.1% at 5 years for the lowest and highest D 2AS score.The D 2AS score had the highest area under receiver operating curves(AUROCs) for predicting development of HCC at 3/5 years(0.895/0.884), compared with that of risk estimation for hepatocellularcarcinoma in chronic hepatitis B (REACH-B) (0.814/0.812)and Fibrosis-4 (FIB-4) (0.759/0.702) scores, age (0.739/0.718), andALT (0.666/0.766) and HBV DNA (0.559/0.556) levels. The calculatedAUROCs to predict development of HCC at 3/5 years were 0.889(95% confidence interval [CI〕 0.796-0.983)/0.876 (95% CI,0.789-0.963) in the validation cohort, with 5-year HCC incidencerates of 0%, 0.6%, 6.3%, and 19.2% for very low, low, high, andvery high D 2AS scores, respectively.Conclusions: HCC developed in patients with elevated HBV DNAlevels and normal or mildly increased ALT levels. The D2AS riskscore can play a valuable role in risk stratification, and may beuseful to guide clinical decisions for enhanced surveillance or treatmentto reduce HCC risk in this population.

        유산균 생존율 향상을 위한 식품첨가물 등급의 동결보호제 탐색

        김치제조가 미생물첨가에 따른 발효조절로 패러다임이 변화함에 따라 김치에서 분리한 유용 미생물의 분말형 제제 기술을 위한 식품첨가등급의 우수한 보호제를 탐색하고자 하였다. 모든 실험균주에서 보호제를 사용할 경우 무처리구보다 높은 생존율을 나타내었고, 특히 10% 콩가루를 동결보호제로 사용하였을 때 90% 수준의 높은 보호효과를 나타내었다. 콩가루의 농도를 증가시킨 경우 동결건조 후 실험균주의 생존율은 농도 의존적으로 증가하지 않았다. 두 가지 보호제를 1:1의 비율로 조합하여 적용할 경우 생존율 향상에 관한 상승효과는 관찰되지 않았다. Food-grade protective agents, namely, skim milk, yeast extract, soy powder, and trehalose, were studied for their ability to improve the viability of freeze-dried lactic acid bacteria (LAB), including Weissella cibaria SW1-1, Lactobacillus plantarum A-1, Lactobacillus sakei 2-12 24, and Leuconostoc citreum 3526. The best results were obtained with 10% soy powder; approximately 90% cell viability was observed during the freeze-drying process. Increase in the concentration of soy powder did not cause a proportional increase in the survival rate of LAB. Further, no significant difference was observed when two agents were combined in a 1:1 ratio (p<0.05).

        신체활동량을 기준으로 한 폐경 후 여성에게 있어 근력운동 실시유무에 따른 골밀도 관련 요인, 골밀도 및 골다공증 비교 연구

        The purpose of this study is to compare the problems of bone in postmenopausal women who have different physical activity and muscle strength exercise using Koera National Health and Nutrition Examination Survey (KNHNES) data. Complex sampling design data was obtained from the fourth (2008-2009) and fifth (2010-2011) KNHNES. The subjects, total 3,307 postmenopausal women excluding those who were in the menopause over 20 years, were classified into six groups based on physical activity and muscle strength exercise. Data were analyzed by complex sample linear model, chi-squared analysis and logistic regression. All statistical significance levels were set to .05. The results of this study are as follows. 1) Height, AUDIT, protein consumption and calcium consumption of people who did muscle strength exercise in moderate physical activity (MM) group were significantly higher than those with no muscle strength exercise in moderate physica l activity (MN) group. Also age and menopause period of MM group was significantly lower than MN group. 2) The difference of body meneral density (BMD) in MM group was significantly higher than MN group. 3) The prevalence of osteopenia and osteoporosis in all of muscle strength exercise group was lower than non-muscle strength exercise group. 4) The prevalence of osteoporosis in MN group was significantly higher than MM group. In summary, the MM group has positive effects on the influencing factors of bone mineral density (BMD), BMD and osteoporosis compared with other groups.

        폐경 후 여성에서 이소플라본과 감마 리놀렌산의 보충 섭취가 혈중 지질 농도 및 갱년기 증상에 미치는 영향

        본 연구 결과를 요약하면 다음과 같다. 1) 산화 LDL 농도는 이소플라본 및 감마 리놀렌산 보충 군에서 유의적으로 감소 (p = 0.006), 플라시보군에서 유의적인 변화 없었음. 두 군간 비교에서 유의적인 차이가 있었음 (p = 0.011). 2) 혈청에서의 중성지방, 총 콜레스테롤, LDL 콜레스테롤, HDL 콜레스테롤, 아포지단백 A1, B 등의 농도는 두 군에서 모두 유의적인 차이가 없었음. 3) LDL particle size와 Paraoxonase 활성도는 두 군에서 모두 유의적인 차이가 없었음. 지질과산화지표인 혈장 MDA 농도는 두 군간 변화량 비교에서만 유의적인 차이가 있었음 (p = 0.010). 4) 갱년기 증상 지표인 Modified kupperman index (KI)는 이소플라본 및 감마 리놀렌산 보충군과 플라시보군에서 모두 유의적으로 감소하였음 (p < 0.001). KI 수치의 군간 비교에서는 유의적인 차이가 없었음. 이소플라본 및 감마 리놀렌산 복합제재를 12주간 섭취하였을 때 이소플라본 및 감마 리놀렌산 보충군에서 산화 LDL 농도가 유의적인 감소하였고, 두 군간의 비교시 MDA 농도 변화량의 유의적인 차이를 확인하였다. 이소플라본 및 감마 리놀렌산 복합제재 섭취는 산화적 스트레스로부터 체내 LDL-콜레스테롤의 산화를 예방하는데 도움을 줄 수 있을 것으로 사료된다. 우리나라에서 폐경 후 여성을 대상으로 이소플라 본 및 감마리놀렌산 복합 제재 섭취와 관련 된 연구는 매우 미흡한 실정임으로 복합제재의 용량 설정을 세분화 하고 복용기간을 연장한 후속 연구가 필요할 것으로 사료된다. This study was performed to examine the combined effects of gamma linolenic acid and isoflavone supplementation on menopausal symptoms and serum lipids in 73 postmenopausal women. A total subjects were randomly assigned to isoflavone (30 ㎎) + gamma-linolenic acid (110 ㎎) group or placebo group. We measured menopausal symptoms by modified Kupperman Index (KI) and oxidized LDL, lipid peroxides, blood components and anthropometric parameters before and after the 12 week intervention period. After the 12 weeks of supplementation, supplement group and placebo group showed a significant reduction of modified kupperman index (p < 0.001). Isoflavone (30 ㎎) + gamma-linolenic acid (110 ㎎) supplement group showed a significant reduction of oxidized LDL cholesterol concentration (p = 0.006) whereas placebo group did not show significant change. Isoflavone and gamma-linolenic acid consumption did not significantly affect plasma concentrations of total, LDL, HDL cholesterol, triglyceride, apo A1, B and blood components. The result of present study demonstrated the supplementation of 30 ㎎ isoflavone and 110 ㎎ gamma-linolenic acid per day for 12 weeks may protect LDL cholesterol from oxidative stress.
