      • Efficacy of the multidisciplinary tumor board conference in gynecologic oncology: A prospective study

        목적: Evidence has shown that multidisciplinary tumor board conferences (MTBCs) improve patient management for various cancer types. However, few retrospective studies have investigated MTBC efficacy for patients with gynecologic cancers. Here, we aimed to prospectively evaluate how MTBCs influence patient management in gynecologic oncology. 방법: This prospective study included 85 consecutive cases that were presented at gynecologic oncology weekly MTBCs of our tertiary university hospital between January 2015 and April 2016. The primary endpoint was treatment plan change rate, which included both major and minor changes. Major changes were defined as exchange, addition, or subtraction of treatment modality. Minor changes included all other, such as intramodality changes or treatment time changes. The secondary endpoints were the change rates of diagnosis, diagnostic work-up, and radiological and pathological findings. 결과: The overall change rate in treatment plan was 27.1%, which included 10.6% major changes and 16.5% minor changes. Among the treatment plan changes, change rate in treatment plan alone was 16.5%, and changes in diagnosis and radiological findings occurred in 7.1% and 3.5% of cases, respectively. Diagnosis and radiological findings were also changed in 9.4% and 10.6% of cases, respectively. However, there were no changes in pathological findings. Moreover, there was a change of diagnostic method for further work-up in 23.5% of cases. 결론: Gynecologic oncology MTBCs resulted in considerable changes in treatment plans. Diagnosis, diagnostic work-up, and radiological findings were also influenced by MTBCs. The data emphasizes the importance of a multidisciplinary team approach for gynecologic cancer management.

        자율무인잠수정은 탐사 목적에 따라 다양한 임무를 수행해야 하며, 임무에 따라 자율무인잠수정 행동의 유형과 순서는 달라질 수 있다. 그러나 대부분의 자율무인잠수정은 한정된 임무에 대하여 프로그램 내부에 고정된 행동 유형으로 동작하며, 다른 유형의 임무를 수행해야 할 경우에는 프로그램을 수정해야 하는 문제점이 있다. 따라서 본 연구에서는 자율무인잠수정이 수행할 수 있는 다양한 임무를 명시할 수 있는 임무 언어를 개발하였다. 이 임무 언어는 명령어의 실행 순서를 제어할 수 있는 제어문과 자율무인잠수정의 행동을 지정하거나 자율무인잠수정의 상태를 입출력 할 수 있는 명령어, 그리고 변수 정의를 제공하기 때문에, 사용자가 자율무인잠수정의 임무를 자유롭게 표현하는 것이 가능하다. 임무 언어로 작성된 임무파일은 전용 어셈블러에 의해 이진 형식의 실행이미지로 변환된 후에, 자율무인잠수정 내장 소프트웨어 내부의 가상기계 기억장치에 적재되어 실행된다. 실행이미지를 가상기계에서 해석하고 실행하는데 필요한 시스템의 자원을 최소화하기 위하여 임무 언어는 자율무인잠수정의 임무를 표현하기 위한 필수적인 부분만을 고려하여 설계되었으며, 문법은 ARM v5 어셈블리와 유사한 형태이다. 개발된 임무 언어는 한국해양연구원에서 개발한 이심이100 자율무인잠수정에 적용되었으며, 이후 개발할 6,000m급의 이심이6000 자율무인잠수정에도 사용될 예정이다.

      • Influence of hormone therapy on platinum-based chemotherapy using ovarian cancer cell line

        Objective: In epithelial ovarian, primary peritoneal, or fallopian tube cancer (EOC), no studies have analyzed impacts of hormone therapy combined with platinum-based chemotherapy according to hormonal receptor expression. This study evaluated influence of hormone therapies (according to hormonal receptor expressions) and their molecular mechanisms on efficacy of platinum-based chemotherapy using a high-grade serous ovarian cancer (HGSC) cell line. Methods: Caov3 cells were stimulated with the following therapeutic agents: paclitaxel (P); carboplatin (C); tamoxifen (T); MPA (M); paclitaxel and tamoxifen (PT); paclitaxel and MPA (PM); carboplatin and tamoxifen (CT); carboplatin and MPA (CM); paclitaxel and carboplatin (PC); paclitaxel, carboplatin, and tamoxifen (PCT); and paclitaxel, carboplatin, and MPA (PCM). MTT assay was performed, and quantitative real-time RT-PCR and methylation assay using estrogen receptor (ER)-, ER-, progesterone receptor (PR)-A, PR-B, and PR-AB were performed. Results: Compared with the non-treated (NT) group, Caov3 cell viability decreased significantly in all groups with the highest decreases in the T and PCT groups (P <0.05). Viability increased significantly in PT and PM groups compared with P group, and in CM group compared with C group (P <0.05). Additionally, compared with PC group, viability decreased significantly in PCT group and increased significantly in PCM group (P <0.05). Expressions of ER-, ER-, and PR-B to PR-AB were not different between the groups. In methylation analyses of ER-, ER-, PR-A, and PR-B genes, methylation of promoters and gene bodies of PCT group were different from the other groups. Conclusion: Combinations of tamoxifen or MPA have influences on platinum-based chemotherapy efficacy in HGSC cells regardless of ER and PR expression. Methylation of ER and/or PR might have influences on HGSC cell proliferation under a combination of platinum-based chemotherapy and tamoxifen.

      • Clinical manifestations and survival rate in women with endometrial cancer according to endometriosis

        Objective: This study aimed to evaluate clinical manifestations and survival rate according to endometriosis in Korean women with endometrial cancer using the nationwide cohort study. Methods: We selected 11,497 women newly diagnosed with endometrial cancer between 2008 and 2016 from the Korean Health Insurance Review and Assessment Service databases. Women with endometrial cancer were defined as women with five or more diagnostic codes for endometrial cancer and no diagnostic code for any other cancer. Endometriosis was defined as presence of a diagnostic N80X code with associated procedures that included fulguration, ovarian cystectomy, adnexectomy, and hysterectomy within 60 days before or after an initial diagnostic code. Results: The data from 2,142 women (18.6%) with endometriosis and 9,355 women (81.4%) without endometriosis were extracted for analysis. Age at diagnosis of endometrial cancer was lower in women with endometriosis compared to those without endometriosis (years, 52.3±8.9 vs 54.6±9.8, P < 0.001). Compared with women without endometriosis, frequency of those with endometriosis were higher between 30 and 49 years (37.4% vs 28.3%) and lower between 60 and 79 years (18.3% vs 28.8%). Frequencies of type 2 diabetes were not different between the groups. Adjuvant radiation therapy after surgery was performed in less frequency in women with endometriosis compared to those without endometriosis (20% vs 23.4%, P < 0.001). However, rate of adjuvant chemotherapy was not different between the groups. Moreover, overall survivals were not different between the groups. Conclusion: Women with endometriosis occur endometrial cancer in relatively younger age. Endometriosis is not associated with survival of endometrial cancer

      • Impact of caffeine on the expression of hormonal receptors in trophoblast cell lines

        목적: We investigated the effects of caffeine on the expression of human chorionic gonadotropin (hCG) and progesterone receptor (PGR), which perform critical roles in the maintenance of pregnancy. 방법: HTR-8/SVneo cells and JEG-3 cells were stimulated for 24 h using caffeine at final concentrations of 0.01-1000 microM/L. The expression of hCG-beta and PGR-B and AB was measured with quantitative real-time RT-PCR and western blotting. 결과: In both cell lines, hCG-beta mRNA and protein expressions did not differ in any study groups compared to the control. In HTR-8/SVneo cells, the ratio of PGR-B to PGR-AB mRNA levels did not differ in all study groups compared with the control. However, it decreased in JEG-3 cells treated with 10-1000 microM/L caffeine. In both cell lines, PGR-A protein expression was not detected and PGR-B protein expression did not differ between the study groups and the control. 결론: This study demonstrated that caffeine was not associated with hCG expression in human trophoblast cell lines. The influence of caffeine on the expression of PGRs was also different according to type of human trophoblast cell lines.

        Intramedullary Skeletal Kinetic Distractor를 이용한 하지 신연 골형성술에서의 기계고장

        Ilizarov 외고정 장치를 이용한 신연 골형성술이 가지는 단점을 극복하기 위하여 개발된 내부 연장 장치를 적용한 사례들이 늘어나고 있다. 그리고 다수의 성공적인 결과들이 보고되어 신연 골형성술의 새로운 지표를 열어가고 있다. 하지만 기계고장이나 연장속도 조 절 어려움 등의 내부 연장 장치 고유의 문제점 또한 존재한다. 저자들은 ISKD® (Orthofix Inc.) 기구를 이용한 신연 골형성술 과정 중 에 발생한 기계고장을 경험하였고, 이를 외고정 장치를 이용한 신연 골형성술로 전환하여 목표 연장길이를 얻어낸 사례를 보고하고자 한다. In an effort to overcome the drawbacks of distraction osteogenesis using the Ilizarov external fixator, intramedullary lengthening devices have been developed and applied for long bone distraction osteogenesis. Several successful cases have been reported, leading to the next generation of distraction osteogenesis. However, intramedullary lengthening devices have their own problems, such as device failure, difficulty of control of the lengthening degree. The authors report on a case of device failure during distraction osteogenesis using ISKD® (Orthofix Inc.) in the distraction phase, and the strategy that involved switching to a lengthening over nail system, which uses the Ilizarov external fixator, in order to obtain the target length.

        The Korea Ocean Research and Development Institute (KORDI) has developed AUVs(BORAM AUV, ISiMI AUV, ISiMI100 AUV) for Swallow Sea, and accumulated high technique and experimence. Since 2012, ISiMI6000 AUV has been designed to explorer a sea-floor topography and hydrothermal deposit at 6,000m depth. ISiMI6000's hull is designed based on Myring shape to minimize of fluid resistance. It is designed to maintain neutral buoyancy. Its whole length is 4.5m, diameter is 0.6m and weight is 749kg. ISiMI6000's maximum forward speed is 4kts and has over 1.5kts of rise/decent speed. It's battery is selected based on 10 hours of pure explorer time for one diving. Inside of ISiMI6000 is composed a frame structure and two pressure-resistant containers for embedding power system and control system. The pressure-resistant containers are made of titanium-aluminum alloy and designed to endure up to 800bar. Frame structure is designed based of Open-frame method and made of aluminium. Mounted equipment is as follow-Main thruster, linear actuators for control pin, information measuring device(Side Scan Sonar, Altimeter, CTD, GPS and etc.), communication device(ATM, RF modem), position tracking device(DVL, AHRS, IMU), camera/light, weight drop system, radio beacon/xenon flash and etc. In this paper, design goals and requirements of ISiMI6000 is described and design results are presented accordingly.

        Meigs' syndrome is defined as the triad of a benign ovarian tumor with ascites and pleural effusion that resolves after resection of the tumor. Although Meigs' syndrome mimics a malignant condition, it is a benign disease. Most cases of Meigs' syndrome are represented by fibroma, thecoma, cystadenoma, and granulosa cell tumor. Ovarian fibroma is the most common cause of Meigs' syndrome and constitutes approximately 2% to 5% of all surgically removed ovarian tumors. Of these diagnosed cases of ovarian fibroma, only 1% have been associated with the clinical picture of Meigs' syndrome. Pseudo-Meigs' syndrome is defined by the identical clinical manifestation with Meigs' syndrome but is associated with other benign ovarian tumors or malignant tumors. This paper presents a unique case of pseudo-Meigs' syndrome.: A large adnexal mass, which presented with ascites and right-sided pleural effusion, was initially considered to be Meigs' syndrome but turned out to be an endometrioid adenocarcinoma on histopathology.

      • 해저보행로봇을 위한 통신 시스템 설계

        현재 한국해양연구원에서 개발 중인 해저보행로봇은 프로펠러 방식으로 추력을 얻는 기존의 해저로봇과는 달리 여러 개의 관절로 이루어진 다리를 이용하여 해저면에 근접해서 보행과 유영으로 이동하는 새로운 개념의 해저로봇이. 다본 논문에서는 Crabster라고 불리는 해저로봇과 이를 보조하는 완충기, 그리고 선상시스템으로 구성되는 다관절 해저로봇 시스템을 위해 설계된 통신 시스템을 소개한다. 구성 요소 사이의 통신은 기가비트 이더넷 광변환기에 연결된 광케이블을 통하여 이루어지, 며내부 통신은 기가비트 이더넷을 주 네트워크로 사용한다. 이더넷 이외의 통신 방식을 갖는 장비들은 입출력 컴퓨터나 이더넷 컨버터를 통하여 주 네트워크에 연결되며, 특히 아날로그 카메라의 경우에는 비디오 인코더를 통하여 연결된다. 이러한 구조의 장점은 전체 통신 시스템의 구조가 간단해지기 때문에 통신 하드웨어와 소프트웨어의 구현이 쉽다는 것이다. 또한 저가의 소형 상용 광변환기를 그대로 사용할 수 있으며, 탑재 장비가 변경되거나 추가될 경우에는 통신 시스템의 변경을 최소화할 수 있는 이점이 있다 . 설계된 통신 시스템의 핵심 부분만으로 구축된 테스트베드로 실험한 결과는 본 연구에서 제안한 통신 시스템 구조가 설계된 모든 장비가 연결되었을 경우에도 충분네한트 워크 대역폭과 100㎐의 제어 주기 이내의 네트워크 지연을 보여주었다
