Indication expansion of new drugs is a key strategy that allows the broadening of therapeutic areas based on a single molecule; it contributes to the strategic recovery of development costs and to the extension of clinical utility. This study systematically analyzed actual patterns of indication expansion and their influencing factors for 419 new active‑ingredient (NME) drugs approved by the US FDA between 2012 and 2021. Among the 419 drugs, 138 (32.94 %) underwent at least one indication expansion, while 281 (67.06 %) did not. The number of expansions was distributed as follows: one expansion in 16.71 % (n = 70), two expansions in 7.16 % (n = 30), and three or more expansions in 9.07 % (n = 38). Among the drugs with indication expansions, the proportion was 31.1% for small molecules (85/273) by modality and 52.8% for oncology (67/127) by therapeutic area. Among products designated as orphan drugs (ODD), 33.0% (72/218) exhibited expansions. The proportions were 34.7% (77/222) for co-developed products and 47.3% (89/188) for products from major top-20 pharmaceutical companies. In the analysis of influencing factors, significant differences were observed in therapeutic area, modality, and company scale in relation to overall occurrence of indication expansion. Specifically, the oncology field showed the highest expansion rate (p < 0.0001), and modality was significantly associated with expansion occurrence (p = 0.0002). Drugs developed by the Major 20 companies had a significantly higher likelihood of expansion (p < 0.0001). In the logistic regression analysis with indication expansion within 38 months of initial approval as the dependent variable, therapeutic area, modality, and company scale emerged as significant factors. The oncology area had a significantly higher likelihood compared to other areas (p < 0.001); the antibody/recombinant protein modality had an odds ratio of 2.42 (95% CI: 1.41–4.16, p = 0.006) versus small molecules; and drugs developed by Major 20 companies had an odds ratio of 3.31 (95% CI: 1.98–5.55, p < 0.001). The Cox regression analysis of time to first indication expansion found that, compared to oncology, other therapeutic areas experienced delayed expansions, while drugs developed by Major 20 companies reached expansion sooner. In the modality analysis, antibodies had a 1.96‑fold (p = 0.0003) faster time to expansion compared with small molecules. In the three‑group analysis comparing the number of indication expansions (1, 2, ≥3), significant distributional differences appeared for therapeutic area, modality, ODD status, and co‑development status. Within therapeutic area, the ≥3‑expansion group contained a relatively high share of oncology drugs (p < 0.0001). For modality, the share of small molecules decreased as the number of expansions increased, whereas the share of antibodies increased.
This study is meaningful in that it categorized influencing factors into drug‑ and company‑level characteristics and analyzed them by both the number and timing of indication expansions for NME drugs. Whereas previous literature often focused on individual cases or single therapeutic areas, the present research considered multiple factors and empirically presented trends and patterns of indication expansion, thereby broadening our understanding of the phenomenon in new‑drug development.
