The purpose of this study is to clarify what the colonial medical system was and how it was developed as one of the core medical systems in Korean medical history. The author analysed the formation and reorganization of the modern medical system in late Great Han Empire and early Japanese colonial period(1876-1919) in order to achieve this purpose. Firstly a detailed analysis was carried out to clarify the characteristics of the modern medical system that the Chosun dynasty tried to establish. Then, an inquiry into the colonial medical system from Residency-General period was made on four fields : hospitals, education, legislation and the police.
After Korea opened her ports to foreign countries in 1876, there growed up the national needs to be a wealthy country and to have a powerful army. The Western medicine was considered as one of the means to meet these needs. Western medicine was regarded serviceable to increase national production and maintain a powerful army. Especially, the notion of sanitation was highlighted on as an effective tool in a way of actualizing a wealthy and powerful country by virtue of taking preventive measures against disease and promoting the health of the people.
Yu Gil-jun(兪吉濬), one of the members of the Civilized Party, discussed diseases, governmental agencies, and legislation toward the modern medical system in the 1890s. Efforts were made to set up a better health system during Gap-o Reformation(甲午改革) in which he participated. The Sanitary Control Agency(SCA) was created as a sub-department of administrative ministry with a view to take charge of sanitary administration in Korea. Also the SCA enacted the preventive law against epidemics and organized the Infectious Disease Prevention Commission in which the western doctors took initiatives while cholera prevailed throughout the country.
The appearance of critical discourse about Oriental medicine that had been a major part of Korean medical system characterize the medical discourse of the Great Han Empire. Also there was a claim that we should emphasize on accommodating the Western medicine as our primary concern. So the Western medicine as well as the Oriental medicine was discussed in that period. Still the Oriental medicine continued playing an important role using its geographic and physical characteristics in Korea. Both Oriental and Western medicines respectively took their own advantages of rather than deprived of each other's field in Great Han Empire.
It was the Government Medical School that accomplished its purpose of accepting the Western medicine in Great Han Empire. The curriculums in the medical school had been transformed from having both Oriental and Western medicines to focusing on the Westerns after having Japanese teacher. But still there was the certain belief that we should have the Oriental medicine as a main part of a medical system. So the Oriental medical doctors continuously got hired in government-run hospitals that played the alternative role in the modern medical system together with the medical school. Furthermore, the Great Han Empire approved of the Oriental medicine as an official one. So the Oriental medical doctors were accepted as legally licensed medical doctors regarding qualification as a part of the modern medical system.
While Japan invaded Korea in an aggressive way, it promoted the Western Medicine Only Policy differentiating from that of Great Han Empire. The Great Han Empire tried to promote both Oriental and Western medicines at the same time. Japan had already promoted its own Western Medicine Only Policy and achieved the unified medical system on the basis of that policy. And Japan would try to change Korea's medical system over its accomplishment. Furthermore, Japan reorganized Korea's medical system in order to strengthen its ruling power. Japan specifically established the medical center Daehan Hospital(大韓醫院) supervising all the medical affairs including the sanitary police in 1907 and dispatched the police advisory doctors(警務顧問醫) taking in charge of sanitation to each province. Douzinkai(同仁會), a private society of Japanese doctors, supported the Residency-General by dispatching its members as the police advisory doctors during the reorganizing process.
The reorganized medical system by the Residency-General aimed at two objectives : to conciliate the Koreans by the practice of Western medicine and to control them through the sanitary police. In order to appease the Korean people, Japan built many government-run hospitals and expanded the Western medical services. Also military doctors were involved in the sanitary police system to enforce control over Korea.
After merging of Korea to Japan, the medical school attached to Daehan Hospital was renamed as the medical training center annexed to Chosun Government-General Hospital Then it promoted to Gyengseong Medical College(京城醫學專門學校) in April 1916. From the beginning, the school aimed at training general practitioners rather than educating highly qualified doctors including specialized researchers. Their policy of medical education agreed with Japan's general educational policy, intending to eternalize her ruling over Korea. Japan intended to establish a colonial medical system subordinating to Japan in medical research and educational training.
The Government-General enacted a new regulation that authorizes Western medical doctors only, the Oriental medical doctors, who had been practically playing major roles in Korea, were not qualified officially but were defined as alternative assistant. Based on this legislative regulation, colonial medical system was established upon Western medicine. The Government-General increased the number of doctors by issuing licence to those who pass the licence examination. Anyone could become a doctor by passing the licence examination without receiving any formal education, which entailed the danger of authorizing unqualified personnels. Although the Oriental medical doctors were not officially authorized, but there were definite needs. So an exceptional rule was added on the regulation for Oriental medical practitioners. In a certain transitional period, the Oriental medical doctors could have a license according to the exceptional law. Both way were considered to be temporary policies because they did not focus on medical education in a long run.
One of the special features of the medical system by Government-General was that the military doctors and police heavily involved in it. Jahye Hospital(慈惠醫院) was a good example that showed up the main characteristic of the medical system driven by the military doctors. While Jahye Hospital was being established, Douzinkai, initiated by private doctors, was excluded even though it had actively supported the Residency-General before. The other distinctiveness of the medical system by Government-General explained why the sanitary police system was consolidated. The police usually tended to limit the individual rights and control people. By magnifying their characteristic, the police became easily involved in sanitation and epidemic controls in Government-General medial system. It was the sanitary police that played the major role in sanitary affairs and epidemic controls.
Although the number of public or government-run hospitals increased and legislation associated with medical affairs was consolidated, the characteristic of the core medical system depending on the sanitary police did not change over the colonial period. It means that the medical policy laid stress on protecting lives in passive way rather than improving people's health in active way. It also showed that Koreans were merely the subject under the control of sanitary police through out over the colonial period. Therefore, those who were in charge of running the colonial medical system considered medicine not as a right or a interest of people but as a tool of governing or ruling Korean people. And others were accustomed to being the colonized public familiar with not spontaneity and agreement but with obedience and submission.
