암환자 가족의 삶의 질에 관한 연구 : 돌봄 부담감과 내적 성장의 매개효과 중심으로

This study intends to investigate the main and mediating effects which caregiving appraisal and positive reappraisal exert on quality of life (QOL) of primary family caregivers of cancer patient considering the relationship with social support. The ultimate goal of this study is to give a contribution to find various ways in improving QOL of family caregivers who take care of a person with cancer.
The processes of this study are as follows. First, the variables which research model were chosen on the basis of stress-appraisal-coping theory through reviews of the previous studies. This study sees that caregiving needs of the cancer patient are the stressor and social support to the family caregiver is helpful resources. In the study, appraisal variable is divided into the feeling of burden as negative appraisal and growth as positive reappraisal. The direct and indirect affective relationships between these variables are presented through the path model. Second, a survey was conducted upon 295 primary caregiver of patient with cancer at National cancer center. Collected data were analyzed by SPSS 12.0 and SEM (Structural Equation Modeling) method using AMOS 5.0.
The summary of the result is as follows. First, as a result of a confirmatory factor analysis, 4 latent variables of this research model are persuasively explained by the measurement model. Second, the entire model including measurement and structural model shows sufficient fit index of CFI(.951), TLI(.940) and RMSEA(.062). Third, the results of analysis of direct effects among variables are as follows. The ‘Social support’ has statistically significant direct effect on the ‘feeling of burden’ and ‘growth’. The ‘feeling of burden’ has statistically significant direct effect on the ‘growth’ and ‘QOL-mental and physical’. The ‘growth’ has statistically significant direct effect on the ‘QOL-mental’. Fourth, the results of analysis of mediating effects of the ‘social support and QOL’ and ‘feeling of burden and QOL’ are as follows. The effects of ‘social support’ on ‘QOL-mental’ are significantly mediated by the ‘feeling of burden’ and ‘growth’. The effects of ‘social support’ on ‘QOL-physical’ are significantly mediated by the ‘feeling of burden’. The effects of ‘feeling of burden’ on ‘QOL-mental’ are significantly mediated by ‘growth’.
Through this research, these implications in social work study and practice are found: (1) this study extended the scope of study in the caregiver’s health area from negative sides into positive ones by using growth variables as positive reappraisal of caregiving in research model, which has not been tried on the Korean family caregivers of the cancer patient. (2) The effects of positive reappraisal on QOL-mental can provide a foundational necessity for social workers to help family caregivers find positive meaning in their caregiving experience. This approach of social work practice will improve QOL of family caregivers. (3) This study present a framework including social support, negative appraisal, positive reappraisal, and QOL variables available to social work practice and explaining affective relationships among these variables in various aspects.

국문 초록 (Abstract)

암 질환은 환자뿐 아니라 가족들에게도 직접적 간접적으로 큰 영향을 미치며 삶에 변화를 가져오게 한다. 이러한 영향은 그들이 가진 자원과, 상황에 대한 평가, 재평가를 통해 신체적, 정신적 삶의 질에도 영향을 주게 된다. 본 연구는 암환자를 돌보는 주 돌봄 제공자를 대상으로 사회적 지지, 돌봄 부담감, 내적 성장, 삶의 질을 파악하고, 이들 변수간의 관계와 함께 삶의 질에 대한 사회적 지지, 돌봄 부담감, 내적 성장의 직접효과와 매개효과를 검증한 것이다.
본 연구의 대상자는 2008년 8월 한달 간 국립암센터에 입원한 암환자 또는 외래를 이용하는 암환자의 가족으로 하고, 한 환자당 한 가족 일원만을 대상으로 하였다. 본 연구에 동의한 참여자는 303명이었으며 그 중 295부를 실제 분석에 사용하였다. 조사도구로는 사회적 지지 척도, 돌봄 부담감 척도, 내적 성장 척도, 삶의 질 척도를 함께 사용하였다. 수집된 자료는 SPSS 12.0을 이용하여 기술적 통계 분석하였고, 연구모형의 가설을 구조방정식으로 검증하기 위해 AMOS Version 5.0 프로그램 사용하였다.
주요 결과를 살펴보면, 첫째, 연구모형의 타당성 검증을 위하여 확인적 요인분석을 하였을 때, 연구모형을 구성하는 4개의 잠재변인에 대해서 각 측정 변인들의 요인 부하량이 모두 .40 이상으로 잠재변인의 개념을 잘 설명하고 있는 것으로 확인 되었다. 둘째, 설정한 연구모형의 모형 적합성을 검증하였을 때 적합도 지수가 CFI(.951), TLI(.940), RMSEA(.062)으로 좋은 적합도를 보이는 것으로 나타났다. 셋째, 각각 변인의 직접효과에 대한 가설 검증결과 사회적 지지의 돌봄 부담감, 내적 성장에 대한 직접효과, 돌봄 부담감의 내적 성장, 삶의 질에 대한 직접효과, 내적 성장의 정신적 삶의 질에 대한 직접효과가 통계적으로 유의하였다. 넷째, 돌봄 부담감, 내적 성장의 매개효과에 대한 가설 검증 결과, 사회적 지지가 삶의 질에 미치는 영향에 대한 돌봄 부담감의 매개효과, 사회적 지지와 돌봄 부담감이 정신적 삶의 질에 미치는 영향에 대한 내적 성장의 매개효과가 통계적으로 유의하였다.
이상의 결과에서 볼 때, 첫째, 사회적 지지가 암환자 가족의 삶의 질에 영향을 미치는 중요한 요인임을 알 수 있었으며, 특히 사회적 지지가 신체적, 정신적 삶의 질에 직접 영향을 주기보다는 부정적인 돌봄 부담감을 완충시키고, 내적 성장을 야기하는 간접적인 효과를 통하여 신체적, 정신적인 삶의 질 향상을 이끄는 것으로 볼 수 있었다. 둘째, 돌봄 부담감은 삶의 질에 가장 크게 영향을 미치는 중요한 변수임을 알 수 있었으며, 특히 신체적, 정신적 삶의 질에 대한 사회적 지지의 영향을 중재하는 매개변인 역할을 한다는 것을 알 수 있었다. 셋째, 내적 성장은 사회적 지지, 돌봄 부담감, 정신적 삶의 질과 각각 직접적인 관계가 있는 변수임을 알 수 있었으며, 특히 정신적 삶의 질에 대한 사회적 지지와 돌봄 부담감의 영향을 중재하는 매개변인 역할을 한다는 것을 알 수 있다. 넷째, 본 연구에서는 돌봄 부담감과 삶의 질의 관계에서 내적 성장의 중요한 매개효과에 대해서 발견하였다. 즉, 돌봄 부담감이 정신적 삶의 질과 직접관계일 때 부담감이 클수록 삶의 질이 나쁘지만, 내적 성장이 매개하는 경우는 부담감이 큰 경우라도 오히려 내적 성장을 크게 하여 정신적 삶의 질 수준을 높일 수 있음을 볼 수 있다는 대안이 도출되었다.
본 연구의 결과를 근거로 하여 암환자 가족이 겪는 객관적 부담에 대해서는 사회적 자원을 제공하고 강화하는 사회적 지지를, 주관적인 평가 영역인 부담감과 내적 성장에 대해서는 상담 및 인지적 교육 프로그램을 포함하는 팀 접근을 제공함으로써 암환자 가족의 삶의 질을 증진시킬 수 있을 것이다.
