배경 및 목적 정신질환은 신체 건강에 부정적인 영향을 미치며, 특히 우울증은 전 연령층에서 나타나는 보편적인 질환이다. 기존 연구는 주로 특정 연령층 내의 분석에 치중해 왔다. 본 연구는 연령대별 우울증 여부가 병원 이용행태(외래이용건수, 외래수납금액)에 미치는 영향의 차이를 비교 분석하고, 이를 바탕으로 중점적인 관리 체계가 필요한 세대를 식별하여 정책적 대응 방안을 제시하고자 한다.
방법론 분석을 위해 한국의료패널 2기(2019~2023) 데이터를 이용하며, 종속변수의 특성을 고려하여 랜덤효과 패널음이항모형과 랜덤효과 패널토빗모형을 적용한다. 분석 결과의 강건성을 확보하기 위해 PSM(Propensity Score Matching)과 IPW(Inverse prorbability Weight)를 통한 추가 분석을 수행한다. 주관적 정신건강 상태 및 우울감 변수를 활용한 민감도 분석으로 결과의 타당성을 종합적으로 검증하였다.
결과 만성우울증이 병원 이용에 미치는 영향은 청년층(20-39세)에서 가장 두드러지게 나타난다. 20-39세 집단은 우울증이 있을 때 외래이용건수가 59.3%, 외래수납금이 131.6% 증가하며 타 연령대 대비 가장 가파른 상승폭을 보인다. 40-49세 연령층은 청년층과 통계적으로 유의미한 차이를 보이지 않는 반면, 고령층(50-64세, 65-74세, 75+)의 경우 우울증으로 인한 의료 이용 증가 효과는 상대적으로 감소하는 경향을 보인다. 이러한 결과는 강건성 검정 및 민감도 검사에서도 일관되게 나타난다.
결론 및 시사점 본 연구는 우울증이 성인들 중 청년층의 의료이용행태를 가장 급격히 변화시킨다는 점을 실증적으로 분석했다. 이는 고령층 중심의 우울증 관리 정책을 청년층으로 확대할 필요가 있으며, 청년기 정신건강에 대한 조기 개입이 장기적인 의료 비용 절감의 핵심임을 시사한다.

Background and Objectives: Mental disorders adversely affect physical health, and depression is a prevalent condition observed across all age groups. Previous studies have primarily focused on analyses within specific age groups, leaving limited evidence on generational differences. This study aims to compare how depression affects healthcare utilization-measured by the number of outpatient visits and outpatient visits and outpatient expenditures-across age groups, and to identify generations that require prioritized management in order to inform targeted policy responses,
Methods: This study uses data from Phase Ⅱ of the Korea Health Panel Survey(2019-2023). Given the characteristics of the dependent variables, random-effects panel negative binomial models and random-effects panel Tobit models are employed. To ensure the robustness of the findings, additional analyses using propensity score matching (PSM) and inverse probability weighting (IPW) are conducted. Sensitivity analyses incorporating subjective mental health status and depressive symptoms are also performed to comprehensively validate the results.
Results: The impact of chronic depression on healthcare utilization is most pronounced among young adults aged 20-39. In this age group, depression is associated with a 59.3% increase in the number of outpatient visits and a 131.6% increase in outpatient expenditures, representing the steepest increases across all age groups. Individuals aged 40-49 do not exhibit statistically significant differences compared with young adults, whereas older age groups (50-64, 65-74, and 75 years and older) show a relatively attenuated effect of depression on healthcare utilization. These patterns remain consistent across robustness checks and sensitivity analyses.
Conclusions and Policy Implications: This study provides empirical evidence that depression leads to the most substantial changes in healthcare utilization among young adults. The findings suggest that depression management policies, which have traditionally focused on older populations, should be expanded to include young adults. Early intervention in mental health during young adulthood may play a critical role in reducing long-term healthcare utilization and associated costs.
