        A Case Report of a Chronic Schizophrenia Treated with Combined Treatment of Korean and Western Medicine

        Objectives: Schizophrenia is difficult to treat effectively and the antipsychotics used have many side effects. However, few studies have focused on the combined treatment of Korean and Western medicine as an alternative. In this study, we reported an inpatient with chronic schizophrenia who was treated by a combination of Korean and Western medicine. Methods: We experienced a case of a diagnosed schizophrenia patient as whose chief complaint was avolition, diminished emotional expression and hallucination. The patient was treated with Western medicine and Korean traditional treatment including acupuncture, moxa and herbal medicine. The Brief Psychiatric Rating Scale (BPRS) and the Positive and Negative Syndrome Scale (PANSS) were used for assessment. Results: After treatment, symptoms involving avolition, diminished emotional expression and hallucination were improved; furthermore, the scores of the BPRS and the PANSS were decreased by approximately 50%, respectively. In addition, there were no notable side effects. Conclusions: The combined treatment of Korean and Western medicine can be an effective and well-tolerated treatment modality for patients with chronic schizophrenia.

        A Case Report of Migraine with Typical Aura on Korean Medical Treatments

        Objectives: The objective of this case report was to investigate the effect of Korean medicine therapy on migraine patients with typical aura. Methods: Acupuncture and chuna manual therapy were applied to outpatients once in 1 to 2 weeks, a total of 9 times, for about 11 weeks. A patient was diagnosed with ganyang-toutong and taeeumin dry-heat symptomatic pattern. Herbal medicine (Chongsanggeontong-tang Gagambang (淸上?痛湯加減方)) was prescribed for 15 days up to the 3rd visit, then another herbal medicine (Yeoldahanso-tang (熱多寒少湯)) prescribed for 15 days up to the 6th visit. MSQol and MIDAS were used as a tool at the start and after 9th treatment. The intensity and frequency of headache, frequency of aura and neck pain were examined on every visit. Results: According to the results, Korean traditional medicine described above had effect on the intensity and frequency of headache, frequency of aura and neck pain as well as an increase in the figure of MSQoL from 62 to 81 and decrease of MIDAS Disability from 18 to 7, Duration from 60 to 30, Pain score from 8 to 2. Conclusions: Migraine with typical aura classified as ganyang-toutong and taeeumin dry-heat symptomatic pattern can be alleviated by Korean traditional medicine.

        Effects of Galgunhaejutang on Alcohol Consumption in C57BL/6 Mice

        Objectives : Galgunhaejutang extract (GHT) is a Traditional Korean medical herbal mixture that has been tradtionally used to treat alcohol consumption. This study was preformed to evaluate the effects of GHT on alcohol consumption in C57BL/6 mice. Methods : Sixty three C57BL/6 mice were alcohol dependence-induced by limited access paradigm. Water, GHT 0.688 g/kg (GHT-L), or GHT 3.45 g/kg (GHT-H) were administrated for 10 days. The amounts of alcohol consumption for 2 hours, water consumption for 22 hours, food intake for 24 hours and body weight were measured. Results : There weren't significant differences in 2 hours of alcohol consumption, 22 hours of water consumption, 24 hours of food intake and body weight for ten days between vehicle group and GHT-L or GHT-H group. Conclusions : Further studies employing multi-dose and long term administration of GHT (more than 10 days) might be of benefit.

        Trends of Tuina Therapy on Depression and Its Efficacy -based on CNKI-

        Objectives: The purpose of this study is to investigate the research about using tuina therapy for depression and to determine its efficacy. Methods: All relevant articles were searched in the China National Knowledge Infrastructure using the terms 'tuina' and 'depression'. Results: Forty-one studies were selected, 34 randomized controlled trials (RCTs) and 7 prospective studies. The Chinese Classification of Mental Disorders and Hamilton Depression Rating Scale were used most frequently as diagnostic criteria and an assessment tool, respectively. Conduction exercise therapy was used in all studies; in contrast, Zheng Xing exercise therapy was used in only 1 study of depression patients with neck vertebrae disease. In 9 RCTs that showed scores of more than 2 points on a modified Jadad scale, tuina therapy had significant antidepressant efficacy compared with conventional treatment. However, more high-quality studies are required. Conclusions: Tuina therapy has a valid therapeutic effect on depression according to studies published in China; if evidence accumulates from high-quality studies, it can be considered a non-pharmacologic treatment for depression in Korean medicine, as well.

        Complementary and Alternative Medicine in Clinical Practice Guideline for Insomnia

        Objectives: The aim of this review was to investigate whether evidence of complementary and alternative medicine (CAM) was reflected in clinical practice guidelines (CPGs) for insomnia based on relevant clinical trials. Methods: We conducted a systematic search on domestic and international CPG databases and medical databases. In addition, we conducted manual searches of relevant articles. Three authors independently searched and selected relevant studies; any disagreement was resolved by discussion. We extracted and analyzed the following data: published language, country, development group, participants, interventions, presence or absence of recommendations for CAM, level of evidence, grade of recommendation for CAM, and methods of development. Results: We identified 8,241 records from domestic and international databases, and 22 CPGs were included. Eleven of the 22 CPGs mentioned CAM interventions including herbal medicine, relaxation, acupuncture moxibustion, Tai Chi, meditation, hypnosis, biofeedback, Tuina, and external herbal medicine. However, most of the CPGs indicated 'no recommendation' or 'weak recommendation' for CAM interventions. Only Valeriana dageletiana Nakai and relaxation were considered to have experimental evidence. Valeriana dageletiana Nakai was recommended for improvement of sleep latency, sleep maintenance, total sleeping time, and sleep cycle. Relaxation was recommended as effective intervention for relieving physical and psychological arousal. Conclusions: Despite systematic reviews and randomized controlled trials on CAM for insomnia, most of the CPGs for insomnia did not reflect the evidence obtained. Further CPGs for insomnia should be developed by considering the current advanced studies in the field of CAM.

        Meditation for Alzheimer's Disease: Systematic Review and Meta-Analysis

        Objectives: Interest in the use of complementary and alternative treatments to treat dementia. Meditation is used to treat various symptoms of physical and psychological diseases. Some studies suggest that meditation might have positive effects on cognitive functions, especially attention, in the elderly. However, how meditation affects Alzheimer's disease (AD) patients remains unclear. In this review, we assessed the effectiveness of practicing meditation in combination with standard care in AD. Methods: We searched the CCRCT, MEDLINE, EMBASE, AMED and CINAHL databases on 30 May 2017. We included randomized controlled trials (RCTs) that used meditation in adult patients diagnosed with AD. We allocated patients to a meditation combined with standard care or a standard care-only group. Results: The two RCTs met the inclusion criteria. A total of 98 patients were included in the meditation with standard care and standard care-only groups in this review. All meditation programs in the included trials were based on practicing mindfulness. The results of our meta-analysis indicatedthat adjunctive mindfulness meditation programs exerted favourable but non-significant effects on cognitive function on the Mini Mental State Examination (MMSE) (MD=4.68, 95% CI -0.11 to 9.46; Z=1.92, p=0.06). Only one study assessed depression, anxiety, quality of life and stress. No adverse events related to meditation were reported in the included studies. Conclusions: Insufficient data iscurrently available to determine the effectiveness of practicing meditation on patients diagnosed with AD. Hence, further RCTs with high methodological quality and larger sample sizes are needed to effectively estimate the effects of meditation on AD.

        The Involvement of AMPA Receptor in the Antidepressant-like Effects of the Portulaca Oleracea L. Extract in Mice

        Objectives : The development of natural drugs with antidepressant effects is important and needed. This study was performed to investigate the antidepressant-like effects of the distilled water extract of Portulaca oleracea L. (POL) in a mouse model and to investigate the role of ${\alpha}$-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid (AMPA) receptors in producing these antidepressant-like effects. Methods : The forced swim test (FST) and tail suspension test (TST) were used to investigate the behavioral anti-depressive-like effects of POL in mice. Additional behavioral experiments with 2,3-dihydroxy-6-nitro-7-sulfamoyl-benzo[f]quinoxaline-2,3-dione, an AMPA receptor antagonist, were undertaken to determine the involvement of the antidepressant-like properties of POL in AMPA receptor throughput. Results : Oral administration of the POL extract (100 mg/kg) 1 h prior to testing significantly reduced the immobility times in the FST and TST. The antidepressant-like effects of the POL extract were not increased in a dose-dependent manner. Pre-treatment with NBQX significantly attenuated the reduction in immobility time induced by the POL extract in the FST. Conclusions : The distilled water extract of POL has antidepressant-like effects, which may be related to AMPA receptor. Pre-treatment with NBQX significantly attenuates the reduction in immobility time induced by the POL extract in the FST.

        Report on Two Cases of Treatment of Anxiety Disorder with Panic Attacks-on the Basis of Breath-Counting Meditation (Anapanasati)

        Objectives: The purpose of this study was to evaluate the effect of breath-counting meditation on anxiety disorder patients with panic attack. Methods: Two anxiety disorder patients with panic attack were treated with Breath-counting Meditation, acupunctures, and herb-medications. The patients have been predicted for anxiety disorder with panic attack through Dignosis and Statistical Manual (DSM-IV), Panic Disorder Severity Scale (PDSS), BDI (Beck Depression Inventory), and BAI (Beck Anxiety Inventory) on their 1st and 6th treatments. Results: After the treatments, both the physical and psychological symptoms have decreased. Conclusions: This study suggests that the breath-counting meditation is an effective way for treating patients who are suffering from anxiety disorder with panic attack.

        A Study of Psychiatric Impotance on the Openning of Nae-ui-won(內醫院)

        내의원(內醫院)은 병원과 약국을 겸비한 일종의 궁중의료기관으로 의사를 비롯하여 여러의원들이 임금과 왕비 및 왕자들의 약을 조제하여 달이던 곳으로, 일명 내국(內局) 또는 약원(藥院)이라고 내의원 앞에 예시되어 있다. 그러나 내의원은 현재 그 내용이 전해지지 않고, 단지 관광지의 유물로만 여겨지고 있다. 현재의 내의원은 창덕궁(昌德宮)에 위치하고 있다. 내의원은 창덕궁의 돈화문(敦化門)을 지나, 인정전(仁政殿)을 거쳐 희정당(熙政當) 좌측에 있으며, 왕비의 거처인 대조전에 가장 가까이 자리잡고 있다. 내의원은 조선왕조 세종25년(1443)에 개원하게 되었으니, 이때는 정치, 문화적으로 안정이 이루어져 가는 상황이었다. 또한 당시 왕이나 왕비 기타 왕족들의 여러 가지 정신적인 갈등으로 인한 질병이 많았으며, 이로인해 전문적이고 독립적인 왕실의료기관의 필요성에 의해 내의원이 독립적인 개원이 이루어졌으리라 사료된다. 이로인하여 저자가 내의원에 관한 자료조사와 실제답사를 통하여 다음과 같은 사실을 알수 있었다. 1) 내의원의 개원은 조선왕조 초기 중앙의료질서 성립을 통한 봉건왕조의 절대왕권확립을 의미함으로 사료된다. 2) 내의원의 위치가 창덕궁의 중심부에 위치하고, 특히 어전(御殿)과 내전(內殿) 가까이에 위치함은 왕실의 질병을 신속하고도 중요하게 다루기 위함으로 사료된다. 3) 특히 의인을 내전에서도 대조전(大造殿) 가까히 둔것은 왕실의 질병중 왕비의 정신신체질환의 치료를 위함으로 사료된다. 4) 내의원의 진료형태는 왕진(往診)이었으며, 이는 당시 의원신분이 중인층이었음으로 사료된다. 5) 내의원의 이용중에서 평민에게는 왕명에 의해서만 이루어졌음으로 사료된다.

        Qualitative Analysis of the Hwa-byung Recovery Process and Influencing Factors

        Objectives: To describe the progress and influencing factors of Hwa-byung. Methods: Data were collected from semi-structured interviews. The progress of and recovery from Hwa-byung were examined using the collected experiences of eight middle-aged women who suffered from Hwa-byung. For analysis of the data collected, a Consensual Qualitative Research (CQR) methodology was used. Results: Of the eight participants who thought they had Hwa-byung, six were actually Hwa-byung patients and two were non-Hwa-byung patients. Four years later, four of the six Hwa-byung patients recovered, while the Hwa-byung of one patient persisted, and the remaining patient had circular Hwa-byung. Over time, the symptoms of Hwa-byung declined substantially. However, the patients recognized that their disease continuously persisted. Moreover, they thought their symptoms had worsened. Many types of changes were observed during the recovery period, including environmental, physical, cognitive, and emotional changes, as well as changes in attitudes toward others. Recovery factors included the reduction of stimuli, social support, exercise, treatment, control of rumination, and some cognitive changes. Conclusions: To recover from Hwa-byung, it is necessary for patients to experience changes in environment, cognition, and emotion, and to employ self-management in everyday life.
