This paper presents the results of a novel study of Vino de Tea (pine heartwood wine) from the island of La Palma (Canary Islands, Spain). The aim is to investigate its differential characteristics and contribute to typifying it within the Denomination of Origin ‘Wines of La Palma’. The analysis is based on a mixed approach combining a qualitative ethnographic approach (22 interviews) and quantitative physic-chemical methods in a sample of 16 wines. The ethnographical study revealed the tradition and techniques of elaboration behind Vino de Tea. In turn, the physicchemical analysis revealed by chromatography a significant presence of α-terpineol due to elaboration in pine barrels or casks. These samples were compared with a Greek Retsina wine, revealing the differences between these two traditional wines. These data suggest that Vino de Tea is a unique product with distinctive characteristics and potential health benefits, owing to its significant content of α-terpineol resulting from its fermentation in Canary pine casks.

This paper shows the relationship, in steel fiber-reinforced concrete, between fiber orientation and fatigue response through the combined use of computed tomography (CT), digital image processing (DIP) software and wedge splitting test (WST). The WST cubes were extracted from conventional 150 × 150× 600 concrete prisms and a groove and notch were carved on different faces in such a way that in half of the test specimens the fibers are oriented mostly perpendicular to the breaking surface and, in the other half, the fibers are mostly oriented parallel to the breaking surface. Fiber orientation was obtained using a CT device and DIP software from a miniprism extracted from the previously mentioned concrete prisms. The results show that there is a strong correlation between the crack-sewing fiber orientation on the one hand and fatigue life and crack opening rate per cycle on the other hand. Cubes with a higher percentage of fibers perpendicular to the crack surface (i.e., with a higher efficiency index) show a longer fatigue life and a lower crack opening rate per cycle, while cubes with a lower efficiency index show a shorter fatigue life and a higher crack opening rate per cycle.

Malakoplakia is a rare granulomatous inflammatory disorder. Its diagnosis depends on histopathological findings; however, high-quality literature regarding proper medical/surgical treatment is lacking. A 38-year-old diabetic female patient was admitted to the emergency room with a history of lower gastrointestinal hemorrhage. Colonoscopy revealed a lesion in the descending colon, and abdominal computed tomography revealed a splenic flexure mass involving the lower pole of the spleen and upper pole of the left kidney. Biopsies confirmed the diagnosis of malakoplakia. After completing antibiotic treatment, a restaging computed tomography revealed a discrete mass increase; hence, the patient underwent laparoscopic <i>en bloc</i> colectomy and partial nephrectomy. Postoperatively, the patient developed a pancreatic fistula, which was successfully treated with percutaneous drainage and antibiotics. The presence of pathognomonic Michaelis-Gutmann inclusions on histopathology is frequently reported as the key to diagnosing malakoplakia. Herein, we present a successful, minimally invasive surgical treatment for colonic malakoplakia.

Purpose: Healthcare-associated infections (HAIs) in urological patients have special features due to specific risk factors. Our objective was to evaluate the characteristics and risk factors for HAIs in patients hospitalized in a Urology ward. Materials and Methods: We evaluated prospectively, from 2012 to 2015, the incidence, types and risk factor for HAIs, microbiological and resistance patterns. Results: The incidence of HAIs was 6.3%. The most common types were urinary infections (70.5%) and surgical site infections (22.1%). Univariate analysis showed an increased risk of HAIs among patients with American Society of Anesthesiologists physical status classification system III–IV (odds ratio [OR], 1.39; p<0.001), immunosuppression (OR, 1.80; p=0.013), previous urinary infection (OR, 4.46; p<0,001), and urinary catheter before admission (OR, 1.74; p<0.001). The surgical procedures with the highest incidence of HAIs were radical cystectomy (54.2%) and renal surgery (8.7%). The most frequently isolated microorganisms were Escherichia coli (25.1%), Enterococcus spp. (17.5%), Klebsiella spp. (13.5%) and Pseudomonas aeruginosa (12.3%). Enterococcus sp was the most common microorganism after radical cystectomy and in surgical site infections, E. coli showed resistance rates of 53.5% for fluoroquinolones, 9.3% for amikacin. The percentage of extended-spectrum betalactamase producing E. coli was 24.7%. Klebsiella spp. showed resistance rates of 47.8% for fluoroquinolones, 7.1% for amikacin and 4.3% for carbapenems. Enterococcus spp showed resistance rates of 1.7% for vancomycin and; P. aeruginosa of 33.3% for carbapenems and 26.2% for amikacin. Conclusions: Comorbidities, previous urinary infections, and urinary catheter are risk factors for HAIs. The microorganisms most commonly isolated were E. coli, Enterococcus and P. aeruginosa. Prospective monitoring may decrease the incidence of infections.
