The use of OverStitchTM for the treatment of intestinal perforation, fistulas and leaks

Gastrointestinal perforations, leaks and fistulas may complicate endoscopic and surgical procedures. Surgical repair is associated with significant morbidity. Therapeutic endoscopic tools and techniques have included the application of tissue sealants, clip closure, and stent placement. Endoscopic suturing is a rapidly evolving minimally invasive technique. The OverStitchTM (Apollo Endosurgery, USA) is currently the only available endoscopic suturing system. Although technically more difficult than clip closure, endoscopic suturing allows closure of larger defects. In some settings, outcomes similar to surgical management with less morbidity may be achieved. This review describes the OverStitchTM endoscopic suturing system and the published literature regarding its use for perforations, leaks and fistulas.
Mass Propagation of Vitex negundo L., in vitro

Shoot proliferation was obtained from shoot tips and nodal explants of Vitex negundo L. on MS medium supplemented with either BAP or KIN (0.1-2.0 mg/L) alone or in combination with NAA (0.1 mg/L). The concentrations of cytokinins combined with NAA produced multiple shoots from shoot tips and nodal explants. The highest mean percentage (84.3$\pm$8.0) of shoot multiplication's were observed on nodal explants in the presence of BAP (1.5 mg/L) and NAA (0.1 mg/L) followed by shoot tips (65.0$\pm$5.0). The regenerated shootlets were rooted on MS basal medium IAA, IBA, NAA (0.1-1.5 mg/L). The maximum number of roots (51.0$\pm$2.6) was achieved on the medium containing IBA (1.0 mg/L) followed by other auxins (NAA, IAA). The regenerated plants were successfully transferred to a mixture of vermiculate and soil. About 95% of the plantlets survived when transferred to the field.

An efficient protocol for in vitro organogenesis was achieved from callus-derived immature and mature leaf explants of Momordica charantia, a very important vegetable and medicinal plant. Calluses were induced from immature leaf explants excised from in vitro (15-day-old seedlings) mature leaf explants of vivo plants (45 days old). The explants were grown on Murashige and Skoog (MS) medium with Gamborg (B5) vitamins containing 30 g $1^{-1}$ sucrose, 2.2 g $1^{-1}$ Gelrite, and 7.7 lM naphthalene acetic acid (NAA) with 2.2 ${\mu}M$ thidiazuron (TDZ). Regeneration of adventitious shoots from callus (30-40 shoots per explant) was achieved on MS medium containing 5.5 ${\mu}M$ TDZ, 2.2 ${\mu}M$ NAA, and 3.3 ${\mu}M$ silver nitrate ($AgNO_3$). The shoots (1.0 cm length) were excised from callus and elongated in MS medium fortified with 3.5 ${\mu}M$ gibberellic acid ($GA_3$). The elongated shoots were rooted in MS medium supplemented with 4.0 ${\mu}M$ indole 3-butyric acid (IBA). Rooted plants were acclimatized in the greenhouse and subsequently established in soil with a survival rate of 90%. This protocol yielded an average of 40 plants per leaf explant with a culture period of 98 days.

On-line, voluntary control of human temporal lobe neurons

Daily life continually confronts us with an exuberance of external, sensory stimuli competing with a rich stream of internal deliberations, plans and ruminations. The brain must select one or more of these for further processing. How this competition is resolved across multiple sensory and cognitive regions is not known; nor is it clear how internal thoughts and attention regulate this competition. Recording from single neurons in patients implanted with intracranial electrodes for clinical reasons, here we demonstrate that humans can regulate the activity of their neurons in the medial temporal lobe (MTL) to alter the outcome of the contest between external images and their internal representation. Subjects looked at a hybrid superposition of two images representing familiar individuals, landmarks, objects or animals and had to enhance one image at the expense of the other, competing one. Simultaneously, the spiking activity of their MTL neurons in different subregions and hemispheres was decoded in real time to control the content of the hybrid. Subjects reliably regulated, often on the first trial, the firing rate of their neurons, increasing the rate of some while simultaneously decreasing the rate of others. They did so by focusing onto one image, which gradually became clearer on the computer screen in front of their eyes, and thereby overriding sensory input. On the basis of the firing of these MTL neurons, the dynamics of the competition between visual images in the subject??s mind was visualized on an external display.
Hydrodynamics of co-current downward liquid-liquid system with packing

The effect of packing and its voidage on the hydrodynamic characteristics of co-current downward liquidliquid system (CCDLLS) was studied using water and three different oils (exxsol, kerosene and white oil). Two flow regimes, homogeneously dispersed oil regime and heterogeneously dispersed oil, were observed with the total pressure drop showing a minimum at the regime transition velocity. Depending on the packing, frictional pressure drop either shows a minimum or monotonically increases with water velocity. Low-density oil results in higher holdup and correspondingly higher frictional pressure drop. Higher dispersed phase holdup is obtained by using a low voidage packing. Semi-empirical/empirical equations are proposed to predict the frictional pressure drop and phase holdup.
