        Thulium laser vaporesection of the prostate: Can we operate without interrupting oral antiplatelet/ anticoagulant therapy?

        Purpose: Thulium vaporesection of the prostate (ThuVARP) is a new and safe approach for patients receiving anticoagulant therapy in whom transurethral resection of the prostate (TURP) may possess a high bleeding risk. We aimed to demonstrate the efficacy and safety of ThuVARP in patients receiving oral antiplatelet/anticoagulant (OAP/OAC) therapy. Materials and Methods: A total of 103 patients who underwent ThuVARP between 2011 and 2013 were enrolled in the study. Patients were divided into 2 groups. Group A consisted of 47 patients who underwent low molecular weight heparin (LMWH) bridging and group B consisted of 56 patients who were operated on while receiving OAP/OAC therapy. Results: The drop in hemoglobin levels in the pre- and postoperative periods was significantly higher in group A than in group B. When subgroups were analyzed, the mean drop in hemoglobin was significantly lower in the warfarin and ticlopidine subgroups of group B than in group A. International Prostate Symptom Scores were significantly lower 3, 12, 18, and 24 months after surgery in group A than in group B. Quality of life scores, maximal flow rate values, and postmicturition residual urine volumes (mL) were similar between the 2 groups. A total of 38 and 41 patients in groups A and B, respectively, had no complications. Conclusions: Our study showed the safety profile of continuing different OAP/OAC therapies in terms of bleeding problems in patients undergoing ThuVARP. We strongly recommend abandoning LMWH bridging and maintaining the OAP/OAC regimen patients are already receiving.

        A Novel Approach to Motion Control Design for Functionally Related Systems

        When a complex task has to be performed by multiple systems, it imposes functional dependencies between the states and/or outputs of the systems. These functional dependencies create a system of ’virtually’ interconnected subsystems, even though they may be physically separated. The component subsystems within the overall system we call ‘functionally related systems’. This paper deals with motion control design for functionally related systems. The design is based on identifying functions. The control design is then performed by projecting the system dynamics into the function space. The main goal is to obtain unit control distribution matrix in the function space and enforce a desired dynamics for each of the identified functions.

        Radiation exposure during different percutaneous renal puncture techniques: A YAU endourology & urolithiasis study

        Purpose: Radiation exposure is affected by C-arm fluoroscopy device positioning during percutaneous renal puncture. Our aim was to compare the exposure of surgeon's lens, hand and chest with a fluoroscopy protocol replicated in different C-arm positions. Materials and Methods: A standardized fluoroscopy protocol was created using water-equivalent solid phantoms to replicate a surgeon and patient. 111 mGy radiation (360 s) was applied in standard fluoroscopy mode (91 kVp, 2.7 mA/mAs). Dosimeters were placed on lens, chest and hand of surgeon and patient phantom models. 7 different C-arm positions were created: 0°, mediolateral (ML) +90°, ML -90°, ML +30°, ML -15°, craniocaudal (CC) +30°, CC +15°. Measurements were evaluated separately for different positions. Results: The highest radiation exposure was measured on patient dosimeter (2.97 mSv). The highest exposure on surgeon was recorded on finger dosimeter in all C-arm positions; highest dose was recorded in ML +90° position (2.88 mSv). In finger dosimeters, lowest exposure was recorded in 0° position (0.51 mSv). The lowest exposure of all positions was measured in chest dosimeter in ML -90° position (0.24 mSv). Conclusions: In positions where X-ray generator of the C-arm was facing towards the surgeon, radiation exposure measured in all dosimeters was higher compared to positions where the generator was facing away. The hand radiation exposure in all positions was higher than chest and lens. Special care must be taken to avoid facing the X-ray generator tube and hands should be as well-protected as chest and eyes with special protective gear.

        Genes Involved in the Biosynthesis and Transport of Acinetobactin in Acinetobacter baumannii

        Pathogenic bacteria survive in iron-limited host environments by using several iron acquisition mechanisms. Acinetobacterbaumannii, causing serious infections in compromised patients, produces an iron-chelating molecule, called acinetobactin,which is composed of equimolar quantities of 2,3-dihydroxybenzoic acid (DHBA), L-threonine, and N-hydroxyhistamine, tocompete with host cells for iron. Genes that are involved in the production and transport of acinetobactin are clusteredwithin the genome of A. baumannii. A recent study showed that entA, located outside of the acinetobactin gene cluster,plays important roles in the biosynthesis of the acinetobactin precursor DHBA and in bacterial pathogenesis. Therefore,understanding the genes that are associated with the biosynthesis and transport of acinetobactin in the bacterial genome isrequired. This review is intended to provide a general overview of the genes in the genome of A. baumannii that are requiredfor acinetobactin biosynthesis and transport.

        On the attenuation of the axisymmetric longitudinal waves propagating in the bi-layered hollow cylinder made of viscoelastic materials

        The paper studies the attenuation of the axisymmetric longitudinal waves propagating in the bi-layered hollow cylinder made of linear viscoelastic materials. Investigations are made by utilizing the exact equations of motion of the theory of viscoelasticity. The dispersion equation is obtained for an arbitrary type of hereditary operator of the materials of the constituents and a solution algorithm is developed for obtaining numerical results on the attenuation of the waves under consideration. Specific numerical results are presented and discussed for the case where the viscoelasticity of the materials is described through fractional-exponential operators by Rabotnov. In particular, how the rheological parameters influence the attenuation of the axisymmetric longitudinal waves propagating in the cylinder under consideration, is established.

        The influence of the rheological parameters on the dispersion of the flexural waves in a viscoelastic bi-layered hollow cylinder

        The paper investigates the influence of the rheological parameters which characterize the creep time, the long-term values of the mechanical properties of viscoelastic materials and a form of the creep function around the initial state of a deformation of the materials of the hollow bi-layered cylinder on the dispersion of the flexural waves propagated in this cylinder. Constitutive relations for the cylinder's materials are given through the fractional exponential operators by Rabotnov. The dispersive attenuation case is considered and numerical results related to the dispersion curves are presented and discussed for the first and second modes under the first harmonic in the circumferential direction. According to these results, it is established that the viscosity of the materials of the constituents causes a decrease in the flexural wave propagation velocity in the bi-layered cylinder under consideration. At the same time, the character of the influence of the rheological parameters, as well as other problem parameters such as the thickness-radius ratio and the elastic modulus ratio of the layers’ materials on the dispersion curves, are established.

        Effects of Omega-3 Supplementation on Adipocytokines in Prediabetes and Type 2 Diabetes Mellitus: Systematic Review and Meta-Analysis of Randomized Controlled Trials

        Background: The objective of this systematic review and meta-analysis was to determine the effects of omega-3 supplementation on adipocytokine levels in adult prediabetic and diabetic individuals. Methods: We searched PubMed, Medline, EMBASE, Scopus, Web of Science, Google Scholar, Cochrane Trial Register, World Health Organization Clinical Trial Registry Platform, and Clinicaltrial.gov Registry from inception to August 1, 2017 for randomized controlled trials. Pooled effects of interventions were assessed as mean difference using random effects model. We conducted a sensitivity, publication bias and subgroup analysis. Results: Fourteen studies individuals (n=685) were included in the meta-analysis. Omega-3 supplementation increased levels of adiponectin (0.48 μg/mL; 95% confidence interval [CI], 0.27 to 0.68; P<0.00001, n=10 trials), but effects disappeared after sensitivity analysis. Tumor necrosis factor α (TNF-α) levels were reduced (–1.71; 95% CI, –3.38 to –0.14; P=0.03, n=8 trials). Treatment duration shorter than 12 weeks was associated with greater reduction than longer treatment duration. Levels of other adipocytokines were not significantly affected. Publication bias could generally not be excluded. Conclusion: Eicosapentaenoic acid and docosahexaenoic acid supplementation may increase adiponectin and reduce TNF-α levels in this population group. However, due to overall study heterogeneity and potential publication bias, a cautious interpretation is needed.

      • On TCP Performance over Multi-Hops Satellite Constellations

        TCP is the most widely used transport protocol on today's Internet. A large part of its success is due to its ability to probe for unused bandwidth while also backing off its transmission rate upon detection of network congestion. Despite the fact that several mechanisms have been presented in recent literature to improve TCP, there remain some vexing attributes that impair TCP's performance. In this paper, we address the issue of the efficiency and fairness of TCP in multi-hops satellite constellations. We mainly focus on the effect of the change in flow count on TCP behavior. In case of handover occurrence, a TCP sender mat be forced to share a new set of satellites with other users resulting in a change of flow count. Our work argues that the TCP rate of each flow should be dynamically adjusted to the available bandwidth when the number of flows, competing for a single link, changes over time. We show through simulations that our fair share scheme improves fairness by guaranteeing a fair progression in window size for each flow, decreases the number of losses, and makes better utilization of the bottleneck link.
