Breast Magnetic Resonance Imaging Indications in Current Practice

Although mammography is the primary imaging modality for the breast, it has its limitations especially with dense breast parenchyma. Breast magnetic resonance imaging (MRI) has evolved into an important adjunctive tool as it is currently the most sensitive technique for breast cancer detection. Despite this high sensitivity, overlap in the appearances of some benign and malignant breast lesions results in additional unnecessary intervention with negative results. These false positives, in addition to high cost and limited availability, necessitate establishing proper indications for breast MRI. The literature was here reviewed for recent clinical trials, meta-analyses and review papers which have studied this important subject. PubMed; the US national library of medicine, was utilized to review the literature in the last twenty years. Using the obtained information, current uses of breast MRI are discussed in this paper to determine the indications which are relevant to clinical practice.

Background Plastic surgery is considered one of the most competitive specialties globally. In Saudi Arabia, it was established in 2001 in the central province (Riyadh) and then expanded in 2009 to include more eastern and western provinces. Previous research has identified several factors that impact pursuing a career in plastic surgery. Methods This cross-sectional study was conducted through a five-page, five-section questionnaire distributed among senior medical students and interns using online platforms between August 2021 and January 2022. The overall number of questions was 25, which tackled different aspects, including sociodemographic characteristics (i.e., age and gender); the duration of the elective rotation; supervisor during the elective; encouraging, discouraging, and influencing factors; and the rating of participants’ desire to become a plastic surgeon. Results After elective training, the most common factor that influenced medical students to choose a plastic surgery career was exposure to a variety of cases (favorable to agree: 80.4%), followed by involvement in clinical activities (i.e., rounding, clinics, and case presentations) (favorable to agree: 70.6%). On the contrary, the most common factor discouraging students from choosing a plastic surgery career after elective training was plastic surgery being a competitive specialty for matching (favorable to agree: 62.7%), followed by the intense workload (favorable to agree: 56.9%). Conclusions Medical students were more likely than medical interns to explore a career in plastic surgery. Exposure to a variety of cases, inspiring mentors, and a decent amount of time spent in the operating room were the most influential factors.

Background: Alternaria panax is the causative agent of black spot disease in Panax notoginseng, which causes significant yield loss. However, the molecular mechanisms underlying its pathogenicity remain mostly unknown. Objective: We sequenced the transcriptome of A. panax during infecting P. notoginseng leaves using next-generation RNA-seq to understand the molecular aspects of black spot disease. Methods: In this study, we sequenced the A. panax transcriptome during infecting P. notoginseng leaves through next-generation sequencing to explore the pathogenesis genes that may be responsible for black spot disease on P. notoginseng. Result: The de novo transcriptome assembly of A. panax produced 23,036 unigenes, of which 18,096 genes were functionally annotated by at least one protein database. GO enrichment analysis and KEGG pathways of differentially up-regulated genes suggest that most genes are associated with metabolic processes, catalytic activity, starch, and sucrose metabolism during infection. Many pathogenesis-associated genes, including genes encoding secreted proteins, candidate secreted effectors, cell wall degrading enzymes, transcription factors, and transporters, were up-regulated in A. panax during infection. In addition, the secondary metabolite biosynthesis genes, including cytochrome P450, and nonribosomal peptide synthetases, were also identified in this study. Conclusions: Differential gene expression analysis has confirmed that A. panax infection was mainly present in the middle and final stages. The findings show that these pathogenesis-associated genes in A. panax may be critical for the P. notoginseng black spots disease.

Fraudulence, cheating, and deception can occur in the commercial real estate (CRE) industry, besides the difficulty in searching for and transferring properties while ensuring the operation is processed through an authoritative source in a trusted manner. Nowadays, real estate transactions use neutral third parties to sell land. Indeed, properties can be sold by the owners or third parties multiple times or without a proper deed. Moreover, third parties request a large amount of money to mediate between the seller and buyer. Methods: We propose a new framework that uses a private blockchain network and predefined BPMN instances to enable the fast and easy recording of deeds and their proprietary transfer management controlled by the government. The blockchain allows for multiple verifications of transactions by permitted parties called peers. It promotes transparency, privacy, trust, and commercial competition. Results: We demonstrated the easy adoption of blockchain for land registration and transfer. The paper presents a prototype of the implemented product that follows the proposed framework. Conclusion: The use of Blockchain-based solutions to resolve the current land registration and transfer issues is promising and will contribute to smart cities and digital governance.
