A case of cutaneous myeloid sarcoma

Myeloid sarcoma is an extramedullary tumor composed of myeloid blasts, such as immature granulocytic, monocytic or erythroid precursors, and can distort the tissue architecture. Myeloid sarcoma can occur de novo, coincide or precede with hematologic malignancies such as acute myeloid leukemia or myeloproliferative disorders, and frequently involve in the skin, lymph nodes, gastrointestinal tract, bone, soft tissue, and testis. Cutaneous manifestations can be diverse and nonspecific as generalized pruritus, or erythematous macules and nodules. Rare, but having severe consequences unless it is correctly diagnosed, therefore, it is important to include into differential diagnosis list. A 76-year-old male initially presented with pruritic erythematous macules which gradually grew into huge plaque on forearm. There were no abnormal laboratory findings except for the skin lesions and he diagnosed with myeloid sarcoma after skin biopsy. After referral to hemato-oncology department, patient was diagnosed with acute myeloid leukemia after bone marrow biopsy. He had blast crisis after chemotherapy and eventually died with sudden cardiac arrest in 2 months after diagnosis. Herein, we report a rare case of cutaneous myeloid sarcoma with abrupt aggravation of clinical course.

Background: Various drugs can cause diverse cutaneous adverse drug reactions (CADR). Factors have been implicated in CADR, including the dosage, duration of use, physiological status and genetic background of the patient. In addition, many authors have suggested the activation of human herpes virus (HHV) 6, 7, Epstein-Barr virus (EBV), and cytomegalovirus (CMV) in patients with drug-induced hypersensitivity syndrome (DISH). However, aside from DIHS, there are scarce data regarding the relationship of HHV 6, 7, EBV, and CMV with the overall CADR. Objectives: We investigated the presence of HHV-6,7, EBV and CMV in the blood samples and tissue biopsy of patients diagnosed with CADR. Methods: Blood samples and tissue biopsy were taken from 26 patients with CADR diagnosed on the basis of drug use and clinical findings. The presence of HHV-6,7, EBV and CMV was confirmed using real-time polymerase chain reaction (PCR). Results: Real-time PCR demonstrated negative results for HHV 6, 7, EBV, and CMV DNA in the blood samples of all patients. However, the tissue sample of one patient with DISH was positive for HHV 6. Only one patient was positive for the EBV capsid antigen IgM, none of the patients was positive for serum anti-CMV IgM. Conclusion: There is still no clinical proof of a relationship between reactivation of HHV 6, 7, EBV, and CMV and overall CADR, except DISH. Inclusion of a larger number of samples and diverse timing of sampling would be needed to confirm our results.

Cutaneous and subcutaneous metastases from the neoplasm of visceral organs are uncommon and have been estimated to occur in only 0.7%∼9% of patients. Among these cases, breast cancer is the most frequently observed malignancy. The incidence of cutaneous metastasis of breast cancer is 23.9%. The most common clinical manifestation of cutaneous metastasis of breast cancer is non-fixed painless erythematous nodules on the anterior chest. However, cutaneous metastasis of breast cancer shows a wide range of clinical manifestations and can mimic benign dermatologic lesions such as erythema annulare centrifugum, contact dermatitis, cellulitis, and erysipelas. We report a 52-year-old woman who presented with diffuse purpuric macules and patches on the right trunk. Finally, based on the clinical and pathologic findings, a diagnosis of cutaneous metastasis of breast cancer was made, and the patient expired 1 year later. (Korean J Dermatol 2021;59(2):140∼143)

The currently available coronavirus disease 2019 (COVID-19) vaccines in South Korea include mRNA (Moderna<sup>®</sup> and Pfizer<sup>®</sup>) and adenoviral vector (AstraZeneca<sup>®</sup> and Janssen<sup>®</sup>) vaccines. Dermatologic side effects of COVID-19 vaccines range from local injection site reactions to systemic eruptions, including morbilliform rashes or erythema multiforme. Pernio-like lesions, one of the most common cutaneous manifestations of COVID-19, have been rarely reported post-vaccination. Herein, we report four cases of pernio-like lesions, which were detected in a single tertiary hospital within 2 months, after the first dose of mRNA-1273 (Moderna<sup>®</sup>) vaccination was administered. In this study, we discuss the clinical and pathological features of our cases and compare them with those of previously reported cases of pernio-like lesions after COVID-19 vaccination. It is pivotal to realize that perniolike lesions can be a possible side effect of COVID-19 vaccination and that the number of patients experiencing this side effect is bound to be quite high in real-world clinical settings.
A case of molluscum contagiosum treated by ingenol mebutate gel

In most of cases, Molluscum Contagiosum (MC) is treated by curettage. However, for children, it is often difficult as they fear it. In these cases, topical therapy can be applied as another option. It has been reported that treatment with adapalene, tretinoin, imiquimod, etc. for average 8 months only acquires 50 to 60 percent of cure rate. Ingenol mebutate (IM) is a new topical drug for the field treatment of actinic keratosis, which improves symptoms in short treatment once a day for a couple of days. Recently, it was reported that IM was also effective in the treatment of MC. When applied to the MC lesion, the roof of MC was disrupted, followed by the extraction of molluscum body. Also, it is possible to promote cellular immune reaction against viral infection. We herein report a 5-year old patient with disseminated MC lesions over the whole body for 7 months who was successfully treated by IM.
A case of kerion celsi mistaken for ruptured epidermal cyst

Kerion celsi is the severe inflammatory infection, caused by a vigorous T-cell mediated host response to the dermatophyte infection. In this case, it can be mistaken for other disease such as cellulitis, impetigo, furuncle, epidermal cyst. M. canis is most likely transmitted by contact of animal hosts such as cats and dogs with humans. We report the 75-year-old female who presented with a 2-month history of tender erythematous scaly plaques with pustules, crusts on Rt. scalp area. At first, from the histopathologic findings of the lesion, the patient was diagnosed epidermal cyst because of cystic-like structure in mid-dermis. But, the fungal culture revealed M. canis as the causative fungus. Herein we report a educational case of kerion celsi mistaken for ruptured epidermal cyst.
