        How to Interpret a Functional or Motility Test : How to Perform and Interpret Timed Barium Esophagogram

        Timed barium esophagogram (TBE) is a simple and objective method for assessing the esophageal emptying. The technique of TBE is similar to usual barium swallow with some modifications, which include taking multiple sequential films at pre-decided time interval after a single swallow of a fixed volume of a specific density barium solution. While many authors have used height and width of the barium column to assess the esophageal emptying, others have used the area of the barium column. TBE is being used in patients with suspected or confirmed achalasia and to follow-up those who have been treated with pneumatic dilation or myotomy. This review discusses technique of performing TBE, interpretation and its utility in clinical practice.

        Development, optimization and pharmacodynamic assessment of olanzapine based lipidic SNEDDS for proficient management of psychosis

        The purpose of this study was to demonstrate the use of 32 factorial design for the development and optimization of lipid based self nanoemulsifying drug delivery system of olanzapine using response surface methodology and to establish a functional relationship between two operating variables of Acconon E (X1) and Cremophor EL (X2). The response variables selected were optical clarity in Millipore water (Y1), Particle size in Millipore water (nm) (Y2), Particle size in 0.1 N HCl (nm) (Y3), Particle size in PB 7.4 pH (nm) (Y4), emulsification time (sec) (Y5) and time for 85 % of drug released (T85 %) (Y6). Compatibility studies using the fourier transform infrared spectroscopy, differential scanning calorimeter and X-ray diffraction studies indicated absence of any incompatibility between olanzapine and excipients used to prepare SNEDDS. Transmission electron microscopy of optimized formulations demonstrated the spherical globules morphology with no signs of coalescence and precipitation of drug. The in vitro hemolysis study and histological study showed the nondamaging effect of the OLZ-SNEDDS. Pharmacodynamic evaluation (Behavioural study) indicates better efficacy of olanzapine loaded SNEDDS.

        Enhancement of in vitro dissolution and pharmacodynamic potential of olanzapine using solid SNEDDS

        The present research work focused on preparing a solidified lipidic self-nanoemulsifying drug delivery system (S-SNEDDS) by adsorbing liquid SNEDDS onto the surface of inert solid adsorbent (Aerosil-200) to improve the in vitro dissolution of poorly aqueous soluble drug, olanzapine and to evaluate their pharmacodynamic (Behavioral study) activity in Swiss albino mice. The liquid SNEDDS consisted of olanzapine, Acconon E and Cremophor EL. The powder flow properties were characterized using parameters viz. angle of repose (θ), Haussner’s ratio and Carr’s index. S-SNEDDS demonstrated good flow characteristics. The solid-state characterizations of S-SNEDDS by fourier transform infrared spectroscopy, differential scanning calorimeter and powder X-ray diffraction studies confirmed the amorphous behavior of olanzapine in the S-SNEDDS. Enhanced in vitro drug release and pharmacodynamic efficacy were attained from solidified- SNEDDS in comparison to olanzapine powder signifying the immense potential offered by S-SNEDDS and it could be a better alternative for oral administration of olanzapine.

        The Utility of 64 Channel Multidetector CT Angiography for Evaluating the Renal Vascular Anatomy and Possible Variations: a Pictorial Essay

        The increased use of laparoscopic nephrectomy and nephron-sparing surgery has prompted the need for a more detailed radiological evaluation of the renal vascular anatomy. Multidetector CT angiography is a fast and accurate modality for assessing the precise anatomy of the renal vessels. In this pictorial review, we present the multidetector CT angiography appearances of the normal renal vascular anatomy and a spectrum of various anomalies that require accurate vascular depiction before undergoing surgical treatment.

        CT-Guided Percutaneous Biopsy of Intrathoracic Lesions

        Percutaneous CT-guided needle biopsy of mediastinal and pulmonary lesions is a minimally invasive approach for obtaining tissue for histopathological examination. Although it is a widely accepted procedure with relatively few complications, precise planning and detailed knowledge of various aspects of the biopsy procedure is mandatory to avert complications. In this pictorial review, we reviewed important anatomical approaches, technical aspects of the procedure, and its associated complications.

      • Radiological Downstaging with Neoadjuvant Therapy in Unresectable Gall Bladder Cancer Cases

        Background: Gall bladder cancer (GBC) usually presents as unresectable or metastatic disease. We conducted a feasibility study to evaluate the effect of neoadjuvant therapy (NAT) on radiologic downstaging and resectability in unresectable GBC cases. Materials and Methods: Patients with locally advanced disease were treated with chemoradiotherapy [CTRT] ( external radiotherapy (45Gy) along with weekly concurrent cisplatin $35mg/m^2$ and 5-FU 500 mg) and those with positive paraaortic nodes were treated with neoadjuvant chemotherapy [NACT (cisplatin $25mg/m^2$ and gemcitabine $1gm/m^2$ day 1 and 8, 3 weekly for 3 cycles). Radiological assessment was according to RECIST criteria by evaluating downstaging of liver involvement and lymphadenopathy into complete response (CR), partial response (PR), stable disease (SD) and progressive disease (PD). Results: A total of 40 patients were evaluated from January 2012 to December 2014 (CTRT=25, NACT=15). Pretreatment CT scans revealed involvement of hilum (19), liver infiltration (38), duodenum involvement (n=22), colon involvement (n=11), N1 involvement (n=11), N2 disease (n=8), paraaortic LN (n=15), and no lymphadenopathy (n=6). After neoadjuvant therapy, liver involvement showed CR in 11(30%), PR in 4 (10.5%), SD in 15 (39.4%) and lymph node involvement showed CR in 17 (50%), PR in 6 (17.6%), SD in 4 (11.7 %). Six patients (CTRT=2, NACT=4) with 66.6 % and 83% downstaging of liver and lymphnodes respectively underwent extended cholecystectomy. There was 16.6 % and 83.3% rates of histopathological CR of liver and lymph nodes. All resections were R0. Conclusions: Neoadjuvant therapy in unresectable gall bladder cancer results in a 15% resectability rate. This approach has a strong potential in achieving R0 and node negative disease. Radiologic downstaging (CR+PR) of liver involvement is 40.5% and lymphadenopathy is 67.5%. Nodal regression could serve as a predictor of response to neoadjuvant therapy.

        Assessment of Meta-Heuristic and Classical Methods for GMPPT of PV System

        Numerous global peak searching mechanisms have been proposed to solve the problem of energy loss due to partial shading of solar photovoltaic (PV) plants but still there is a great need for an effi cient and fast global maximum power point tracker (GMPPT). A detailed review of GMPPT based on various meta-heuristic and classical methods along with the basics of partial shading phenomenon, proper positioning of the PV panels, distributed MPPT, Field MPPT etc. will be of great help to the researchers. Till date, it looks like that not a single literature has covered these issues at a single place and thus, this article fi lls this gap by providing a detailed assessment, tabulated gist of the various GMPPT along with the basics of key issues related to partial shading of PV plants. Various classical and bio-inspired meta-heuristic based GMPPT methods have been compared in this literature. It is expected that this paper will prove to be a valuable asset and a complete reference for the academicians and professionals for further research and proper selection of a GMPPT technique.
