        Malaysian pharmacy students’ perspectives on the virtual objective structured clinical examination during the coronavirus disease 2019 pandemic

        Purpose: This study investigated pharmacy students’ perceptions of various aspects of virtual objective structured clinical examinations (vOSCEs) conducted during the coronavirus disease 2019 pandemic in Malaysia. Methods: This cross-sectional study involved third- and fourth-year pharmacy students at the International Islamic University Malaysia. A validated self-administered questionnaire was distributed to students who had taken a vOSCE a week before. Results: Out of the 253 students who were approached, 231 (91.3%) completed the questionnaire. More than 75% of the participants agreed that the instructions and preparations were clear and helpful in familiarizing them with the vOSCE flow. It was found that 53.2% of the respondents were satisfied with the flow and conduct of the vOSCE. However, only approximately one-third of the respondents believed that the tasks provided in the vOSCE were more convenient, less stressful, and easier to perform than those in the conventional OSCE. Furthermore, 49.7% of the students favored not having a vOSCE in the future when conducting a conventional OSCE becomes feasible again. Internet connection was reported as a problem hindering the performance of the vOSCE by 51.9% of the participants. Students who were interested in clinical pharmacy courses were more satisfied than other students with the preparation and operation of the vOSCE, the faculty support, and the allocated time. Conclusion: Students were satisfied with the organization and operation of the vOSCE. However, they still preferred the conventional OSCE over the vOSCE. These findings might indicate a further need to expose students to telehealthcare models.

        Characterization and Performance Evaluation of Advanced Aircraft Electric Power Systems

        A model of an advanced aircraft electric power system is developed and studied under variable-speed constant-frequency (VSCF) operation. The frequency of the generator's output voltage is varied from 400-Hz to 800-Hz for different loading scenarios. Power conversions are obtained using 12-pulse power converters. To reduce the harmonic contents of the generator output waveforms, two high-pass passive filters are designed and installed one at a time at the generator terminals. The performance of the two passive filters is compared according to their losses and effectiveness. The power quality characteristics of the studied VSCF aircraft electric power system are presented and the effectiveness of the proposed filter is demonstrated through compliance with the newly published aircraft electrical standards MIL-STD-704F.

      • Application of Hamilton variational principle for vibration of fluid filled structure

        Vibration investigation of fluid-filled three layered cylindrical shells is studied here. A cylindrical shell is immersed in a fluid which is a non-viscous one. Shell motion equations are framed first order shell theory due to Love. These equations are partial differential equations which are usually solved by approximate technique. Robust and efficient techniques are favored to get precise results. Employment of the wave propagation approach procedure gives birth to the shell frequency equation. Use of acoustic wave equation is done to incorporate the sound pressure produced in a fluid. Hankel's functions of second kind designate the fluid influence. Mathematically the integral form of the Lagrange energy functional is converted into a set of three partial differential equations. It is also exhibited that the effect of frequencies is investigated by varying the different layers with constituent material. The coupled frequencies changes with these layers according to the material formation of fluid-filled FG-CSs. Throughout the computation, it is observed that the frequency behavior for the boundary conditions follow as; clamped-clamped (C-C), simply supported-simply supported (SS-SS) frequency curves are higher than that of clamped-simply (C-S) curves. Expressions for modal displacement functions, the three unknown functions are supposed in such way that the axial, circumferential and time variables are separated by the product method. Computer software MATLAB codes are used to solve the frequency equation for extracting vibrations of fluid-filled.

        Implementation of waste silicate glass into composition of ordinary cement for radiation shielding applications

        The aim of this work is to study the radiation shielding properties of cement samples with waste glassincortated into its composition. The mass attenuation coefficient (MAC) of the samples were experimentally determined to evaluate their radiation shielding ability. The experimental coefficient wasevaluated using NaI detector for gamma energies between 59.53 keV and 1408.01 keV using differentradioactive point sources Am-241, Eu-152, Co-60, and Cs-137, and the gamma transmission parametershalf-value layer, mean free path, and transmission factor were calculated. The theoretical coefficient ofthe composites was determined using Geant4 and XCOM software. The results were also comparedagainst Geant4 and XCOM simulations by calculating the relative deviation between the values todetermine the accuracy of the results. In addition the mechanical properties (including Compressive andporosity) as well as the thermogravimetric analysis were tested for the present samples. Overall, it wasconcluded that the cement sample with 50% waste glass has the greatest shielding potential for radiationshielding applications and is a useful way to reuse waste glass.

        Effect of Aqua Therapy Exercises on Postmastectomy Lymphedema: A Prospective Randomized Controlled Trial

        Objective To investigate the effect of aqua therapy resistance exercise on arm volume, pain, and shoulder range of movements in post-mastectomy lymphedema. Methods This was a single-blind randomized controlled trial. Fifty eligible breast cancer survivors (median, 10 years after surgery) with lymphedema (median, 21% inter-limb difference) were assigned randomly to group A (n=25) or control group B (n=25). The study group underwent 60 minutes of aqua therapy exercise comprising of warm-up for 10 minutes, 40 minutes of strengthening exercises, and 10 minutes of cooling down, three times a week for 8 weeks. The control group underwent 60 minutes of land-based exercise three times a week for 8 weeks. Arm volume calculated by measuring the arm circumference, shoulder flexion, and abduction range of motion (ROM), and pain using a visual analog scale (VAS) were assessed at baseline and after 8 weeks of treatment. Results There was a statistically significant difference in limb volume, shoulder flexion and abduction ROM, and VAS scores in favor of the study group (p<0.001) after 8 weeks of intervention. The mean±standard deviation for limb volume, shoulder flexion, abduction, and pain score were 2,108.71±200.97 mL, 169.68°±4.54°, 150.44°±3.92°, and 3.16±1.1 in the study group and 2,256.41±186.94 mL, 147.36°±5.32°, 131.32°±4.38°, and 5.68±0.94 in the control group, respectively. Conclusion Adding aqua therapy resistance exercise to routine physical therapy might be more effective in decreasing the limb volume and pain intensity and improving ROM of the shoulder in postmastectomy lymphedema.

        Testicular Biopsy Histopathology as an Indicator of Successful Restoration of Spermatogenesis after Varicocelectomy in Non-obstructive Azoospermia

        Purpose: We aimed to evaluate the efficacy of using testicular biopsy histopathology as an indicator of the success of loupe-assisted subinguinal varicocelectomy in non-obstructive azoospermia (NOA) patients.Materials and Methods: In a 2-year period, a prospective study was carried at Minoufiya University Hospital on 20 NOA patients with clinical bilateral varicoceles. These patients underwent loupe-assisted subinguinal varicocelectomy with simultaneous testicular biopsy. All patients were evaluated by determining their hormonal profile and performing semen analyses and scrotal Doppler and transrectal ultrasonography. Two semen analyses showing azoospermia were performed before the surgery and two semen analyses were received at 3 and 6 months post-operatively for follow-up.Results: The mean age was 29.9±6.7 years, and the mean follow-up duration was 17.3±8.3 months. We noted the restoration of spermatogenesis in six men (30% of all patients). Testicular biopsy results were as follows: hypospermatogenesis in 7 patients, maturation arrest in 3, and Sertoli cell-only syndrome in 10. The improvement in the sperm counts of these patients ranged from 3 million to 15 million/mL. Sperms were recovered in the hypospermatogenesis (6 patients, 85.5%) patients only, but other patients with testicular biopsy results of Sertoli cell-only or maturation arrest did not show any improvement in their semen parameters. Conclusions: Testicular biopsy results showed that hypospermatogenesis patients have a better chance of improvement in their semen analysis after varicocelectomy in contrast to NOA patients with Sertoli cell-only syndrome or maturation arrest.

        Environmentally benign Scouring of Cotton Knits Using Locally Produced Acid Pectinase Enzyme

        The main task of the present study is to pre-screen selected fungal strain (Fusarium oxysporum M1-EGY) isolatedfrom local agriculture soil and optimize cultivation conditions for an acid pectinase production. Experimental resultsdemonstrated the optimum conditions for achieving the highest enzyme activity (14.80 U/ml) were found to be: incubationperiod (8 days) at pH (5), temperature (30 oC), and shaking rate (150 rpm), apple pectin carbon source (2 %), and ammoniumphosphate as nitrogen source (0.2 gN/l). The positive impacts of utilizing the produced crude acid-pectinase in eco-friendlyscouring of various cotton knits as well as their subsequent bleaching and reactive dyeing have been confirmed. Additionally,the use of local resources for an acid pectinase production would encourage and promote local production of textile enzymesand their potential applications in Egyptian textile sector, taking in consideration efficiency, textile quality, environmental andeconomy concerns.

        Effect of Intermittent Fasting on Glucose Homeostasis and Bone Remodeling in Glucocorticoid-Induced Osteoporosis Rat Model

        Background: The present study examined the effect of intermittent fasting (IF) on bone mineral content (BMC) and bone mineral density (BMD) and the markers of bone remodeling in a glucocorticoid-induced osteoporosis (GIO) rat model. Methods: Forty male rats were allocated to 4 groups (N=10 per group): control group of normal rats; control+IF group (normal rats subjected to IF for 16-18 hr daily for 90 days); dexamethasone (DEX) group: (DEX [0.5 mg i.p.] for 90 days); and DEX+IF group (DEX and IF for 90 days). By the end of the experiment, BMD and BMC in the right tibia were measured. Serum levels of the following were measured: glucose; insulin; triglycerides (TGs); total cholesterol; parathyroid hormone (PTH); osteoprotegerin (OPG); receptor activator of nuclear factor-κB (RANK); bone-resorbing cytokines, including bone deoxypyridinoline (DPD), N-terminal telopeptide of collagen type I (NTX-1), and tartrate-resistant acid phosphatase 5b (TRAP-5b); and bone-forming cytokines, including alkaline phosphatase (ALP) and osteocalcin (OC). Results: DEX administration for 90 days resulted in significantly increased serum levels of glucose, insulin, TGs, cholesterol, PTH, OPG, DPD, NTX-1, and TRAP-5b and significantly decreased BMD, BMC, and serum levels of RANK, OC, and ALP (all P<0.05). IF for 90 days significantly improved all these parameters (all P<0.05). Conclusions: IF corrected GIO in rats by inhibiting osteoclastogenesis and PTH secretion and stimulating osteoblast activity.
