<P>The main objective of this study was to model the contribution of shelf sediments in the Arctic Ocean to the total mass of neutral organic contaminants accumulated in the Arctic environment using a standardized emission scenario for sets of hypothetical chemicals and realistic emission estimates (1930–2100) for polychlorinated biphenyl congener 153 (PCB-153). Shelf sediments in the Arctic Ocean are shown to be important reservoirs for neutral organic chemicals across a wide range of partitioning properties, increasing the total mass in the surface compartments of the Arctic environment by up to 3.5-fold compared to simulations excluding this compartment. The relative change in total mass for hydrophobic organic chemicals with log air–water partition coefficients ≥0 was greater than for chemicals with properties similar to typical POPs. The long-term simulation of PCB-153 generated modeled concentrations in shelf sediments in reasonable agreement with available monitoring data and illustrate that the relative importance of shelf sediments in the Arctic Ocean for influencing surface ocean concentrations (and therefore exposure via the pelagic food web) is most pronounced once primary emissions are exhausted and secondary sources dominate. Evaluating Internet Pricing Schemes: A Three-Dimensional Visual Model

Traditional Internet pricing schemes are coming under continual pressure to adapt to, and encourage, a changing mix of Internet applications and consumer usage patterns. Much research effort over the last decade has been focused on developing more efficient and attractive charging schemes. However, none of the proposed models has been widely deployed. This raises questions regarding the inhibiting factors and missing pieces that make pricing the Internet such a challenge. In this paper, we discuss the problems with current Internet pricing schemes, review the history of Internet pricing research over the last ten years, and summarize the key features and motivations of the most significant models. We develop a novel visual approach to comparing and evaluating such schemes using a three-dimensional (3D) metric encompassing technical efficiency, economic efficiency, and social impact. We address and discuss the important factors that have inhibited the deployment of the reviewed models and suggest productive areas of focus for future Internet pricing research.
A Primary Care Approach to Myelodysplastic Syndromes

Myelodysplastic syndromes (MDS) are probably the most common hematologic malignancies in adults over the age of 60 and are a major source of morbidity and mortality among older age groups. Diagnosis and management of this chronic blood cancer has evolved significantly in recent years and there are Food and Drug Administration-approved therapies that can extend patients’ life expectancy and improve quality of life. Primary care physicians (PCPs) are often involved in the process of diagnosis and follow-up of MDS patients, especially those in low-risk groups. They can therefore play an important role in improving patient care and quality of life by ensuring early referral and participating in supportive management. There is also a shortage of oncologists which increases the importance of the role of PCPs in management of MDS patients. In the face of limited resources, PCPs can improve access and quality of care in MDS patients. This article provides an overview of the common manifestations, diagnostic approaches, and therapeutic modalities of MDS for PCPs, with a focus on when to suspect MDS, when a referral is appropriate, and how to provide appropriate supportive care for patients diagnosed with MDS.
An Overview and Update of Chronic Myeloid Leukemia for Primary Care Physicians

Chronic myeloid leukemia (CML) accounts for approximately 15% of adult leukemias. Forty percent of patients with CML are asymptomatic, in whom the disease is detected solely based on laboratory abnormalities. Since the introduction of tyrosine kinase inhibitor therapy in 2001, CML has become a chronic disease for the majority of patients. Primary care physicians may be the first to recognize a new diagnosis of CML. In patients with known CML, the primary care physician may be the first to detect disease progression or adverse effects to therapy. This article provides an overview of the clinical presentation, diagnostic approach, and treatment considerations of CML.
