        Archaeogeophysical investigation using magnetic and GPR surveys at Tal-Baltus, Menofia Governorate, Egypt

        An integrated geophysical survey using land magnetic and ground penetrating radar (GPR) methods was conducted to investigate the archaeological findings and ancient cultural relics at an undiscovered 100 m × 60 m in Tal-Baltus in El-Sadat city, Menofia Governorate, Egypt. The study area at Tal-Baltus was chosen among several archaeological hills in the Menofia governorate due to its archaeological significance in the Greco-Roman history of Egypt. The total area was first surveyed using the magnetic method, and then two small promising sites within this area were selected for the GPR survey. The obtained magnetic results showed the presence of remarkably high anomalies with different shapes of irregular geometry. Therefore, they are interpreted as ruins of old storage rooms related to an ancient harbor-shaped structure. Besides, numerous scattered pillars and column heads were also delineated and matched with remains of granite blocks in abundance in the study area, taking the same trend as the delineated magnetic anomalies. In addition, the GPR results highlighted several hyperbolas with variable amplitudes and sizes, which have been interpreted as the shallow foundation of a potential ancient harbor made of limestone. The comprehensive interpretation of the integrated magnetic and GPR surveys strongly suggests that the study area may be a part of an ancient harbor in addition to some other ancillary room-shaped structures used for cargo storage purposes and scattered portions of walls and pillars dating back to the Greco-Roman era.

        Procedural outcomes of laparoscopic caudate lobe resection: A systematic review and meta-analysis

        A systematic review was conducted in compliance with PRISMA statement standards to identify all studies reporting outcomes of laparoscopic resection of benign or malignant lesions located in caudate lobe of liver. Pooled outcome data were calculated using random-effects models. A total of 196 patients from 12 studies were included. Mean operative time, volume of intraoperative blood loss, and length of hospital stay were 225 minutes (95% confidence interval [CI], 181–269 minutes), 134 mL (95% CI, 85–184 mL), and 7 days (95% CI, 5–9 days), respectively. The pooled risk of need for intraoperative transfusion was 2% (95% CI, 0%–5%). It was 3% (95% CI, 1%–6%) for conversion to open surgery, 6% (95% CI, 0%–19%) for need for intra-abdominal drain, 1% (95% CI, 0%–3%) for postoperative mortality, 2% (95% CI, 0%–4%) for biliary leakage, 2% (95% CI, 0%–4%) for intra-abdominal abscess, 1% (95% CI, 0%–4%) for biliary stenosis, 1% (95% CI, 0%–3%) for postoperative bleeding, 1% (95% CI, 0%–4%) for pancreatic fistula, 2% (95% CI, 1%–5%) for pulmonary complications, 1% (95% CI, 0%–4%) for paralytic ileus, and 1% (95% CI, 0%–4%) for need for reoperation. Although the available evidence is limited, the findings of the current study might be utilized for hypothesis synthesis in future studies. They can be used to inform surgeons and patients about estimated risks of perioperative complications until a higher level of evidence is available.

        Early and Late Functional Outcomes of Anal Sphincter-Sparing Procedures With Total Mesorectal Excision for Anorectal Adenocarcinoma

        Purpose: The study aims to assess the functional outcome of anal sphincter-sparing procedures (SSP) with total mesorectal excision (TME) for anorectal adenocarcinoma. Methods: In a multicentric, prospective, single-group study in the period between December 2012 and November 2017, 93 patients presented with anorectal adenocarcinoma were included in the study. Sixty-nine patients underwent SSP with TME. SSP included the combined approach of transabdominal TME with intersphincteric resection (ISR) or transanal transabdominal TME. Using the per anal examination scoring system (PASS), postoperative anal function was assessed after 1 year. Results: Bowel motility time was 50±19 hours. The time needed for narcotic analgesia was 54±18.8 hours. Mean hospital stay was 15.4±10.25 days. Incidence of evident fecal incontinence after ISR is 10.6% (7 of 67 cases). The PASS findings of 69 cases are as follows: extremely hypotonic 8.6% (6 cases), slightly hypotonic 26.1% (18 cases), normal tone 58% (40 cases), slightly stenotic 3 cases (4.3%), or occluded 2.9% (2 cases). Urinary dysfunction occurred in 1 case (1.4%). Temporary diversion was performed in 61 patients (87.1%). Conclusion: Sphincter preservation with TME for anorectal adenocarcinoma helps avoid permanent stoma and provides reasonable functional outcomes. PASS is a new application for postoperative assessment of anal function.

        A Four Leg Shunt Active Power Filter Predictive Fuzzy Logic Controller for Low-Voltage Unbalanced-Load Distribution Networks

        Recently evolved power electronics’ based domestic/residential appliances have begun to behave as single phase non-linear loads. Performing as voltage/current harmonic sources, those loads when connected to a three phase distribution network contaminate the line current with harmonics in addition to creating a neutral wire current increase. In this paper, an enhanced performance three phase four leg shunt active power filter (SAPF) controller is presented as a solution for this problem. The presented control strategy incorporates a hybrid predictive fuzzy-logic based technique. The predictive part is responsible for the SAPF compensating current generation while the DC-link voltage control is performed by a fuzzy logic technique. Simulations at various loading conditions are carried out to validate the effectiveness of the proposed technique. In addition, an experimental test rig is implemented for practical validation of the of the enhanced performance of the proposed technique.

        Vaginal Laser Therapy for Stress Urinary Incontinence: A Systematic Review of Prospective Randomized Clinical Trials

        The most common type of urinary incontinence in women is stress urinary incontinence (SUI) which negatively impacts several aspects of life. The newly introduced vaginal laser therapy is being considered for treating SUI. This systematic review aimed to evaluate the efficacy of vaginal laser therapy for stress urinary incontinence in menopausal women. We searched the following databases: MEDLINE (via PubMed), EMBASE, Cochrane Library databases, Web of Science, clinical trial registry platforms, and Google Scholar, using the MeSH terms and keywords [Urinary Incontinence, Stress] and [(lasers) OR laser]. In our systematic review, prospective randomized clinical studies on women diagnosed with SUI as per the International Continence Society’s diagnostic criteria were included. The Cochrane Risk-of-Bias assessment tool for randomized clinical trials was used to evaluate the quality of studies. A total of 256 relevant records in literature databases and registers and 25 in additional searches were found. Following a review of the titles, abstracts, and full texts, four studies involving 431 patients were included. Three studies used CO2-lasers, and one used Erbium: YAG-laser. The results of all four studies revealed the short-term improvement of SUI following both the Erbium: YAG-laser and CO2-laser therapy. SUI treatment with CO2-laser and Erbium: YAG-laser therapy is a quick, intuitive, well-tolerated procedure that successfully improves incontinence-related symptoms. The long-term impact of such interventions has not been well established as most trials focused on the short-term effects.
