본 연구는 빈곤 여성노인의 요실금 중증도를 전ㆍ후기 연령군으로 구분하여 관련요인을 파악하고자 실시하였다. 분석자료는 2009년 전국 253개 보건소 방문건강관리사업에 등록되어 낙상예방운동, 영양, 요실금, 약물복용, 우울, 인지기능저하 완화 교육프로그램에 참여한 재가 노인 중 여성노인 4,036명의 기초자료를 이용하였다. 분석은 빈곤 여성노인을 전기와 후기 연령군으로 구분하여 요실금 중증도의 관련요인을 다중회귀분석을 이용하여 분석하였다. 연구결과, 전기노인은 허약상태, 만성질환의 수, 왼손 악력, 시각 장애 등 신체기능적 상태를 나타내는 여러 요인들이 관련성을 보인 반면, 후기노인은 허약상태만이 관련성을 보여 전기와 후기노인의 요실금 모두 허약상태와 매우 긴밀한 관계가 있는 것으로 나타났다. 노인이 신체적, 정서적 기능이 감소되었거나 기능의 쇠퇴가 진행 중인 허약상태일 경우 연령과 관계없이 일반 노인에 비해 요실금이 발생할 가능성이 더 높으며 요실금이 있는 노인은 허약상태가 유발되거나 심화될 가능성이 증가할 수 있다. 따라서, 노인의 요실금 예방과 관리에 있어 허약상태에 대한 사정과 중재가 필수적이며 허약노인에게 적합한 요실금 관리도 반드시 제공되어야 할 것이다. The purpose of this study was to investigate the severity of urinary incontinence and relevant factors according to age in low-income elderly women. The participants were female elders over 65 years of age registered in public health centers. Data were collected by interviewing the elders participating in educational programs for fall prevention, nutrition, urinary incontinence, medication compliance, depression and recognition function enhancement. For data analysis, descriptive statistics and multiple regression analysis were performed. Multiple regression analysis shows that frail status, the number of chronic diseases, grip strength(left hand), and visual impairment were significantly associated with the severity of urinary incontinence in the young-old(65 to 74 years of age), while only frail status was associated in the old-old(75years of age or older). These findings indicate that frail status is common and crucial factor related to the severity of urinary incontinence of female elders regardless of age. Therefore, the intervention for preventing and getting over urinary incontinence for female elders should be focused on the prevention of progress to frailty.

지역사회 재가 허약노인의 낙상두려움 관련요인 성별 비교

본 연구는 허약노인의 성별에 따른 낙상두려움, 건강상태, 규칙적 운동 횟수 등을 비교하고 낙상두려움에 영향 미치는 요인을 규명하고자 하였다. 본 연구는 253개 보건소의 2009년 맞춤형 방문건강관리사업 중에서 허약노인 사례관리를 위해 수집된 65세 이상 노인들의 기초자료를 이용하였다. 연구대상자는 총 3903명(남 259명, 여 3644명)으로, 평균 연령은 남성이 75.17세, 여성이 평균 77.54세였으며, 남성의 75.3%, 여성의 85.4%가 낙상 두려움이 있다고 응답하였다. 저소득층 허약노인의 낙상두려움에 영향을 미치는 요인은 남성의 경우 정적 균형감, 과거 낙상경험이었으며, 여성 노인의 경우 정적 균형감, 복합적 이동능력, 우울과 과거 낙상경험, 교육수준으로 나타났다. 본 연구결과를 토대로 허약노인의 낙상두려움 여부 및 영향요인을 주기적으로 평가하여 고위험군을 발견하고, 낙상 및 낙상두려움 발생을 예방하기 위해 이들을 대상으로 정서적 접근을 포함한 다요인적 통합 프로그램을 성별에 따른 맞춤형으로 개발, 제공할 것을 제안한다. Purpose: This study was to investigate the factors influencing the fear of falling in frail elderly according to gender. Methods: The participants were the elderly over 65 years who were registered for case management for frail elderly of 253 public health centers. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 18.0. Results: Prevalence of fear of falling and the influential factors were different according to gender. 75.3% of the male elderly, 85.4% of the female elderly had the fear of falling. The predictors for men's fear of falling were static balance ability, experiences of previous falls, whereas for women static balance ability, TImed up and go, depression, experiences of previous falls, educational status were significant. Conclusion: Fear of falling and the influential factors of the frail elderly according to gender should be assessed regularly to find the high risk group. On the basis of that, prevention program for fear of falling and fall need to be developed and provided, which should be gender sensitive.

간호·간병통합서비스병동 노인입원환자의 낙상위험지각, 낙상관련 효능감 및 낙상예방행위에 대한 연구

본 연구는 간호·간병통합서비스병동에 입원한 노인입원환자의 낙상위험지각의 수준 차이에 따른 낙상관련 효능감과 예방행위를 파악하기 위한 서술적 조사연구이다. 자료 수집은 충청북도 J시, C시에 소재한 5개의 간호·간병통합서 비스병동에 입원한 65세 이상의 노인 환자를 대상으로 2021년 10월 20일부터 12월 30일까지 설문조사한 자료 148부를 SPSS 28.0프로그램을 이용하여 분석하였다. 연구결과, 낙상위험상태 수준과 낙상위험상태지각 수준이 일치하지 않을 경우, 낙상관련 효능감은 유의한 차이를 보이지 않았으나, 낙상예방행위는 유의한 차이를 나타났다. 낙상위험상태와 이에 대한 지각의 일치할 때 적합한 낙상예방행위 수행하게 되므로 실제 낙상위험수준과 함께 지각수준을 사정한 후 위험수준에 대한 정확한 인식을 토대로 낙상예방교육을 수행할 필요가 있다. This descriptive research study was undertaken to determine the perception of fall risk, fall-related efficacy, and fall prevention behavior among elderly individuals using the Comprehensive Nursing Care Services. We further endeavored to understand the fall-related efficacy and fall prevention behavior according to the extent of fall risk perception. Data were collected from 148 elderly patients aged 65 years or older, admitted to 5 centers located in J and C cities in the North Chungcheong Province between October 20 and December 30, 2021, and availing the Comprehensive Nursing Care Services. Data were analyzed using SPSS 28.0. Our results revealed that when the levels of fall risk and perception of fall risk were unmatched, there was no significant difference in fall-related efficacy according to fall risk, and the fall-related efficacy remained consistent, However, a significant difference was obtained for fall prevention behavior. Our data also showed that appropriate fall prevention measures were performed when the fall risk and perception of fall risk are well matched. Therefore, it is necessary to conduct fall prevention education based on the accurate recognition of the risk level after assessing the perception level, along with the actual fall risk level.
