PURPOSE: This study examined the effects of combined conventional exercise therapy plus respiratory exercise program with combined conventional exercise therapy plus aqua exercise program in the pulmonary function of subacute stroke patients. METHODS: The respiratory exercise program group underwent inspiration and expiration training using the Threshold IMT and Threshold PEP three days per week for four weeks. The aquatic exercise program group had aquatic aerobics, halliwick, and bad ragaz ring training three days per week for four weeks. Before and after the experiment, Pony fx was used to examine the FVC, FEV1, FEV1/FVC, VC, and MVV. RESULTS: Significant improvement was found after the experiment in the FVC. FEV1, VC, MVV, excepting FEV1/FVC, of the pulmonary function in the aquatic exercise program group. After the experiment, significant improvement was found in FVC, FEV1, FEV1/FVC, VC, and MVV of pulmonary function in the respiratory rehabilitation therapy group. No significant difference in FVC, FEV1, FEV1/FVC, VC, and MVV of pulmonary function was observed in the inter-group comparison. CONCLUSION: Significant improvement was found after the experiment in both the aquatic exercise program group and the respiratory exercise program group. No difference in pulmonary function was noted in the inter-group comparison. Therefore, combining general exercise therapy and an aquatic or respiratory exercise program is expected to be effective for the pulmonary function of acute stroke patients. These results are expected to provide basic data to help research intervention of aquatic and respiratory exercise programs for subacute stroke patients.

Twelve patients underwent surgical interventions for aortic aneurysms consecutively from September 1986 to September 1990 in the Department of Thoracic and Cardiovascular Surgery, Yeungnam University Hospital. They were all men ranging in age from 25 to 68 years with the mean age of 48 years. Five patients of them had aneurysms involving ascending aorta, one aneurysm involving both ascending and descending aorta, four aneurysms involving descending thoracic aorta, and the others had aneurysms involving abdominal aorta. According to morphological classification, six cases of them were in dissection and the others in fusiform aneurysms. All the cases were pathologically true aneurysms and four of them were in acute status. According to causal classification, seven patients of them had atherosclerosis, three cystic medial necrosis, and the others had syphilis and trauma respectively. Six patients with ascending aortic aneurysm had annuloaortic ectasia with aortic regurgitation and underwent ascending aortic graft and aortic valve replacement[Bentall`s operation]. The others with descending thoracic and abdominal aortic aneurysms underwent Dacron graft replacement. Both hemorrhage necessitating reopening of the chest and wound dehiscence were observed in three patients and hoarseness in one patient. Cerebrovascular accident occurred three and a quarter years postoperatively in one patient and he died two days later. The others were followed up via OPD and have been doing well postoperatively.

We have performed a prospective study to assess the efficacy of suction drainage in 45 patients with primary spontaneous pneumothorax and 15 patients with secondary spontaneous pneumothorax, treated by closed chest tube drainage with underwater seal during the period Jan. 1990 to Dec. 1990 at the Department of Thoracic and Cardiovascular Surgery, Yeungnam University Hospital. The patients were divided randomly into two groups, 28 cases receiving suction and the other 32 cases no suction. The success rate was 82.1% for the former and 87.5% for the latter with the overall success rate of 85.0% and there was no significant difference in success rate between the two groups According to the causes and the extent of pneumothorax, the difference between the success rates of the two groups was also not significant statistically. But according to the duration of air leakage, suction group under 2 days showed a high success rate[46.4%] and the same group with 3-4 days, a relatively low success rate[21.4%] compared with that of the former. We conclude that the suction treatment is somewhat valuable in shortening the tubing time in patients with small amounts of air leakage, but it doesn`t seem to increase the success rate in all patients.
