        한국인 구순열 및 구개열 환자 1008례에 대한 임상적 고찰

        The congenital cleft lip and palate is relatively high incidence among koreans. So, it has been studied by many authors because impropr manaement resulted maldevelopment, speech disturbance, psychological stress of parents and social maladjustment of patients. The authors retrospectively analysed reviewed and 1008 cases of cleft lip and palate patients who were treated at the department of plastic and Rec onstructive Surgery, National Medical center form April 1978 to september 1989. Clinical analysis of these 1008 cases of cleft lip and palate were summerized as followings. The result of 6-11 were obtained from 701 cases which clinical records was perfect among 1008 cases. 1. The ratio of cleft lip, cleft lip with palate, cleft palate alone was 2:1.76:1. 2. The male was affected 1.9 times more than the female in cleft lip and 2.8 times in cleft lip with palate but, the female was affected 1.06 times more than in cleft palate alone. 3. The complete cleft lip occurs 1.6 times more than incomplete cleft lip and the ratio of Left, Right and bilateral cleft lip was 2.5 : 1 : 0.3. 4. The incomplete cleft palate occurs 2.9 times more than complete cleft palate. 5. The complete cleft lip with palate occurs 5.8 times more than incomplete cleft lip with palate and the ratio of LT., Rt, and bilateral cleft lip with palate was 1.9 : 1 : 0.86. 6. Other congenital anormaly was 5.7% and the tongue tie was the most frequent one. 7. Family history was present in 8.1% among the cleft patients. 8. The gestational history during 1st trimester was mostly hyperemesis gravidarum. URI and drug medication. 9. Regional distribution was Seoul and Kyungki : 39.1%, Honam : 23.1%, Youngnam : 16.8% and Choungcheong : 15.4% 10. The distribution of cleft patient in the family was 33%, 32% and 20% in 1st, 2nd and 3rd siblings in sequence. 11. Mothers in the group aged 25 top 30 years had the greatest incidence of children with clefts (46%) 12. Now, the cleft patients tend to undergo the operation at appropriate times, but some of them lose the proper moment due to economic problem. 13. In the past, triangular flap method was used in large percent of in complete cleft lip patients, but recently Millard method is being used mostly. 14. Triangular flap method was used mostly in treatment of complete cleft lip in the past, but recently, Millard method is in common use. 15. The trend of the operation for bilateral cleft lip has been changed from the two stage to one stage operation during the past 12 years. 16. The nasal deformity of the cleft lip was corrected by minor operation and/or nasal splint in field of the cheiloplasty.

        근접부 전층피부 이식술을 이용한 경한 하안검 외번증의 교정

        Postoperative ectropion of the lower eyelid is one of the most common complications in lower blepharoplasty. The causes are many, including excess removal of skin, sequelae of a traumatic cicatrix due to hematoma and damage from cauterization, weakened canthal tendon, inflammation of the fat pocket beneath the inferior septum and atonicity or flaccidity of the lower eyelid. For correction of the lower eyelid ectropion, horizontal shortening of lower eyelid, tightening of lateral canthal lid and FTSG are frequently used. The authors corrected 2 cases of mild postoperative lower eyelid ectropion using of secondary blepharoplasty in which suture plication of orbicularis oculi muscle, cepharolateral rotation & tightening of lower eyelid skin flap and small FTSG from remnant lateral marginal skin of lower eyelid skin flap were included.

        반지에 의한 수지 결출상(Ring Avulsion Injury) : 3 cases report

        The ring avulsion injury is that accidental taction upon a ring shears the skin and varying amount of tissue from the fingers, furthermoer, it is often accompanied with dislocation of joints or facture of phalangeal bones. In the past, this injury has led to reconstruction by distant flap or primary amputation(or Ray amputation) with results that were often less desirable. Recently, the decvelopment of microsurgical technique makes it possible to get more acceptable result in reconstruction of severe ring avulsion injury functionally and cosmetically. The advantages of microsurgical reconstruction for ring avulsion injury is that it makes available to get more functional digit without staged operation. We experienced three patients with ring avulsion injury(Carroll Classification Class Ⅳ) among 600 patients who underwent micosurgical replantation of hands and digits. They were operated with micosurgical anastomoses and two cases of them were successfully replanted leading to useful finger. Average TAM of each digit was 217'(one 210', the other 225') and average 2PD was 8.5mm(one 7mm, the other 10mm)

        Correcting Shape and Size Using Temporary Filler after Breast Augmentation with Silicone Implants

        Breast contour deformities, lack of volume and asymmetry are common confronted problems after breast augmentation with implants. These problems can be corrected by using temporary fillers or autologous fat grafts. The purpose of this study was to introduce our experience using long lasting temporary filler (Aquafilling®) for the correction of unfavorable results after breast augmentation with silicone implants. Two non-pregnant, non-breastfeeding women unsatisfied with previous breast augmentation with silicone implants were recruited for this study. All procedures were performed under local anesthesia with sedation. Efficacy and safety assessments were carried out at follow-up visits (1, 3, and 6 months). The study showed that Aquafilling® could provide satisfactory improvement in breast shape and volume. Also it showed that the corrected volume and shape were lasting without affecting the breasts’ original volume. Patients reported high satisfaction as Aquafilling® was generally well tolerated with no inflammatory reactions or serious adverse events. We recommend that Aquafilling® as a new option for the correction of minor problems after breast augmentation surgery with implants. However, further follow-up studies are required to observe long-term results.
