자폐범주성장애를 대상으로 시각적 단서를 활용한 작업수행의 훈련효과: 예비연구

◉ 목적 : 자폐범주성장애 아동의 작업수행 향상을 시각적 단서를 이용한 훈련과 이용하지 않은 작업수행 훈련의 효과를 비교하고자 하였다. ◉ 연구방법 : 연구디자인은 개별실험 연구 설계인 교차중재설계(alternating treatment design)를 사용하였다. 연구대상자는 자폐범주성장애 진단을 받고 경기도 소재 정신건강의학과 의원에서 작업치료를 받는 만 16세 남아였다. 중재방법으로 회기 당 시각단서의 유무별로 운동화 끈묶기 훈련을 교차로 실시하였다. 종속 변인의 측정은 운동화 끈으로 리본을 묶는 작업수행 과정을 비디오로 촬영한 후, 기간기록(duration recording)으로 교차중 재과정 5회, 일반화 과정 4회 동안 수행시간과 수행의 질을 측정하였다. ◉ 결과 : 모든 중재 후 대상자는 작업수행시간이 단축되었고, 시각적 단서가 있는 중재가 시각적 단서가 없는 중재 보다 적은 시간이 걸렸다. 수행의 질 역시 시각적 단서가 있는 중재점수가 더 높았다. 그러나 과제수행률이 증가하는 중재후반에는 시각적 단서가 없는 중재가 더 적은 시간이 소요되었다. 대상자는 중재가 끝난 후에도 수행기술을 유지하였다. ◉ 결론 : 시각적 단서를 이용한 치료가 시각적 단서를 이용하지 않은 치료에 비해 자폐범주성장애를 가진 청소년의 작업수행 질의 향상과 시간을 단축시키는 것으로 나타났다. 그러나 시각적 단서를 이용한 치료는 초기중재 기간이 지난 후엔 오히려 방해요인으로 작용하여 이러한 중재는 초기에 보다 효과적임을 알 수 있었다. ◉ Purpose : This research was conducted to compare whether visual cue affected occupational performance of adolescent who had diagnosed autistic spectrum disorder. ◉ Method : This research implemented alternating treatment design as single experimental research study. The subject was 16-year-old adolescent with autistic spectrum disorder visiting psychiatric hospital of Gyunggi-do. Therapeutic intervention to improve occupational performance was to tie his shoelaces with using visual cue and without by turns. To measure dependent variable, sessions tying his shoelaces was taken by video and was done with duration recording to evaluate accomplishment time and quality of performance during 5 sessions of alternating treatment and 4 sessions of generalization. ◉ Result : After intervention, the subject reduced occupational performance time and specially sessions using visual cue cut more time than not using visual cue. Futhermore, when being with visual cue, quality of performance intervention score was higher. However, from fifth session intervention with no visual cue took less time. The subject maintained performance skill after intervention. ◉ Conclusion : This study showed treatment with visual cue for adolescent diagnosed autistic spectrum disorder makes reduce performance time and improve quality of performance than without. However, intervention with visual cue affected as disturbance factor after early sessions so, this intervention is more effective at beginning stages.

◉ 목적 : 본 연구는 감각통합치료가 지적장애아동의 문제행동의 개선에 미치는 영향에 대해 알아보고자 하는데 목적이 있다. ◉ 연구방법 : 본 연구는 과제이탈, 주의산만, 상동행동을 가지는 지적장애 9세 남아를 대상으로 실시하였다. 총 20회기, 주 3회, 40분, 1:1 감각통합치료를 통한 문제행동을 다중기초선 설계(multiple-based line)로 A-B설계를 사용하였으며 사건기록법으로 측정하고 분석하였다. ◉ 결과 : 대상아동은 감각통합치료를 통해서 과제이탈행동 23회에서 11.30회로 11.7회가 감소하였고, 주의산만은 22.57회에서 10.62회로 11.95회 감소하였고, 상동행동은 18.6회에서 7.4회로 11.2회 감소하였다. ◉ 결론 : 본 연구는 감각통합치료가 지적장애아동의 문제행동감소에 효과가 있는 것으로 나타났다. ◉ Purpose : This study was to describes the effects of problem behaviors on sensory integration therapy with intellectual disabilities. ◉ Method : The participant of the study was 9 years old boy with intellectual disabilities with out-of seat, attention deficit, stereotype behaviors. the intervention for the study consisted of total 20 sessions with 40minutes 3 times a week of each session in 1:1 sensory integration therapy. the participants was evaluated with even recording and multiple based line at A-B designed. ◉ Result : The children’s out-of seat behavior declined form 14.75times to 11.30 times, attention deficit declined from 16.28times to 10.62times, stereotype behavior declined from 18.10times to 7.04times, through sensory integration therapy. ◉ Conclusion : Sensory integration therapy might be helpful child with intellectual disabilities on problem behaviors.
