      • 구강 내 균형장치 및 한방치료를 적용한 턱관절장애 환자에 대한 증례보고

        Objectives: The aim of our study was to report clinical improvement of a patient who suffered from Temporomandibular Disorder (TMD) being treated with Korean Medical treatments and Intraoral Balancing Appliance (IBA). Methods: During the admission period, the patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy and FCST (Functional Cerebrospinal Therapy) every day. And we observed patient's condition by Numeric Rating Scale (NRS), Pain Disability Index (PDI), EQ-5D-5L (Five-level EuroQol-5 dimensions), and assessment about range of motion(ROM) for temporomandibular joint and cervix. Results: After treated for 4 weeks, the patient's NRS, PDI, EQ-5D-5L and ROM were improved. Conclusions: Korean Medical treatments including FCST are estimated to be effective for patients with TMD. But the case was only one, so more cases and further research is needed to prove the effectiveness of the treatment.

        3D-CT를 이용한 흉추의 극돌기와 횡돌기 사이의 관계에 대한 연구

        Objectives : There are many theories about the anatomical relationships between the thoracic spinous and transverse processes. However, these studies were all conducted on cadavers. Thus, there might be differences when applied to living individuals. Therefore, the aim of this study was to determine whether the theories were similar when measured in living individuals. Methods : We conducted studies with 10 individuals aged 20-30 using 3D-CT. Four different measurements were taken between the thoracic spinous and transverse processes. To facilitate the relationship analysis, the spinous process positioned between the traverse process of the segment and the lower segment was set to 0.5. The spinous process located at the height of the traverse process of the lower segment was set to 1. The spinous process located between the traverse process of the lower segment and the two segments below was set to 1.5. Results : Therefore, based on the aforementioned settings, T1–T4 were 1, T5–T7 were 1.5, T8–T10 were 1, and T11 and T12 were 0.5. This indicated that the spinous processes of T1–T4 and T8–T10 were equal in height to the transverse processes of the lower segment, and that T5–T7 were at the midpoint of the height of the transverse processes between the lower segment and two segments below, and T11, T12 corresponded to the midpoint between the traverse process of the segment and the lower segment. Conclusions : Neither ‘Geelhoed s rule’nor ‘the rule of three’was accurate when applied to living individuals. However, this study had some limitations, so further research is needed.

        근위 경골 절골술 후 한방 재활치료 프로토콜을 적용한 재활치료의 임상적 효과: 증례 보고

        In this study, we propose a rehabilitation protocol involving Korean medicine for patients after high tibial osteotomy (HTO) and report its effectiveness. Three patients underwent HTO using the rehabilitation protocol involving Korean medicine. We estimated the outcome evaluating physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After the treatment, we observed that knee movement and muscular strength were improved. In addition, the numeric rating scale, pain disability index, Lysholm knee score, and walking state of patients were improved. Therefore, a rehabilitation protocol involving Korean medicine can be effectively used for patients after HTO. The limitation of this study was the insufficient number of cases and the difficulty of follow-up because of the characteristics of this operation and the long periods of time that are needed for each phase of the protocol. Further studies are needed to design a rehabilitation protocol involving Korean medicine. (J Korean Med Rehabil 2018;28(3):107-118)

        무릎뼈 골절 후 한방 재활치료의 임상적 효과: 증례 보고

        The aim of this study was to propose our rehabilitation protocol involving Korean medicine for patients with patellar fracture and report its effectiveness. Further, for the same patellar fracture patients, we created different protocols for those who underwent surgical treatment and those who required conservative treatment. Among patients who received inpatient treatment at our hospital for three years from August 2017 to August 2020, we included three patients who underwent surgical treatment for patellar fracture and one patient who required conservative treatment for patellar fracture. The degree of improvement of the patients was evaluated using physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After each treatments, we observed that the range of motion of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state improved in the patient groups. Even with the same patellar fracture, we observed that different rehabilitation treatments involving Korean medicine should be applied to patients who underwent surgical treatment and those who required conservative treatment. However, further studies are needed to design a rehabilitation protocol involving Korean medicine. (J Korean Med Rehabil 2020;30(4):203-217)

      • 구강 내 장치를 이용한 한의 임상연구 고찰

        Objectives: The purpose of this review is to analyse the trends of clinical studies using oral appliance in Korean Medicine. Methods: We reviewed Korean Medicine papers by searching Korean web databases 'Korea Traditional Knowledge Portal (KTKP)', 'Korea Medical Information Portal (OASIS)', 'Academic Research Information Service (RISS)', 'Scientific and Technological Information Integration Service (NDSL)'. We classified the papers by the year of publishment, the title of journals, the type of study and contents. Results: 1. Clinical studies using oral appliance have been increasingly published since 2011. 2. Most of the studies were published in the Journal of TMJ Balancing Medicine. 3. 33 research papers were divided in to 6 original articles, 27 case reports. But almost presented a low level of evidence. 4. In the content of the studies, oral appliance were used for various diseases such as nervous system, musculoskeletal system and visceral system. Conclusions: We expect that various studies and clinical application will be actively carried out with the development of technology related to oral appliance.

        원위 요골 골절에 대한 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고

        The objective of this study was to propose a rehabilitation protocol for distal radius fracture in Korean Medicine and to report its effectiveness. Three patients who had undergone distal radius fracture were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the wrist joint, numeric rating scale and quick the Disabilities of the Arm, Shoulder and Hand (DASH) score. After the treatment, we observed that wrist movement, numeric rating scale and quick DASH score were improved. A rehabilitation in Korean Medicine can be effectively used for distal radius fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine. (J Korean Med Rehabil 2018;28(3):97-106)

        근골격계 수술 후 한방재활치료의 효과: 후향적 관찰연구

        Objectives This study aimed to evaluate the effectiveness of postoperative Korean rehabilitation treatment for patients with musculoskeletal disorders. Methods The medical records of patients undergoing Korean medical rehabilitation after a musculoskeletal surgery, from January 1, 2016, to December 31, 2019, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale (NRS) and questionnaire for each surgical site. When the same patient was re-admitted after discharge, it was treated as the same case if it was within 7 days from the discharge date; otherwise, it was treated separately. If the period of hospitalization was within 7 days or the chief complaint was not related to musculoskeletal surgeries, the case was excluded. Results In total, 480 cases were included in this study. Acupuncture (99.8%), moxibustion (95.6%), cupping (97.5%), Chuna manual therapy (81.5%), Korean medical physiotherapy (61.0%), Daoyin exercise therapy (26.3%), and herbal medicine (87.7%) were used as the Korean medical rehabilitation treatments. A significant improvement was noted when comparing the NRS and questionnaire results for each surgical site at the time of admission and discharge. Conclusions Korean medical rehabilitation can be effectively used for patients who have undergone musculoskeletal surgeries, to relieve pain and enable returning to daily activities. However, further research with a high level of evidence is necessary to support this finding. (J Korean Med Rehabil 2020;30(3):151-162)

        Sauve-Kapandji 수술 후 한방재활치료에 대한 증례보고

        Objectives : The purpose of this study was to report the clinical progress of a patient exposed to a Sauve-Kapandji procedure after being diagnosed with dislocation of distal radioulnar joint and was treated using Korean medicine rehabilitation treatment. Methods : During the admission period, the patient was treated with acupuncture, cupping, herbal medicine, and Chuna therapy. The clinical progress was assessed by using range of motion(ROM), manual muscle test(MMT), numeric rating scale(NRS), and pain disability index(PDI). Results : After receiving the above treatments, the active ROM and motor grade of the elbow, wrist, and finger joints were improved; the NRS and PDI were decreased. Conclusions : Although this is a single case report, Korean medicine rehabilitation treatment, including Chuna manual therapy, might be an effective intervention for a patient after being exposed to a Sauve-Kapandji procedure.

        섬유근통에 대한 국내 임상 연구 동향 및 중재 방향

        Objectives The purpose of this review was to analyze the trend of clinical research related to fibromyalgia and to suggest the direction of intervention Methods We reviewed the clinical research by searching seven Korean web databases and classified the studies by the year of publication, academic field, and study design. Results A total of 75 clinical studies were selected. Except for 2002, more than one study was annually published; however, the number was below 10. In the academic field, there were 34 in the medical field, 21 in the Korean medicine field, and 15 in the nursing science field. By study design, there were 62 observational studies, mostly case reports and case-control studies. There were 13 experimental studies, including non-random control studies and one group pre and post-test design studies. Conclusions Fibromyalgia-related clinical research requires a greater number of studies, and a study design with a high level of evidence should be conducted along with various observational and intervention studies. Additionally, multidisciplinary and integrated intervention plans and research are needed.

        몽골과 한국 전통의학의 비교 연구

        Objectives The purpose of this study was to investigate the development process and describe the diagnosis methods, theories and treatments of Mongolian traditional medicine and Korean traditional medicine through literature records and prior studies. Methods Literature records and previous studies on traditional medicine of both countries were collected through various sites in Mongolia (Esan, Mongoliajol, Kok, Yumpu, Scribd, Science and Technology Foundation [STF]) and Korea (Koreanstudies Information Service System [KISS], Korea Institute of Science and Technology Information [KISTI], National Digital Science Library [NDSL], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS]). Also the English database was searched through PubMed. In the case of Mongolian traditional medicine, medical books published in Mongolia were mainly referenced and used for research. Results Studying the development process, basic concepts and the system of diagnosis and treatment of the two traditional medicine, several commonalities and differences were revealed. Conclusions This study showed that the scope of diagnosis methods between Mongolian and Korean traditional medicine were slightly different, and that the medical terminology for the diagnosis method had slightly different contents from each other. Although there were many similarities in treatments of Mongolian and Korean traditional medicine, the Chuna therapy is found in Korean traditional medicine only. The basic theories constituting traditional medicine were the same, but the five-element theory used by the two countries differs in the following two factors. Mongolia uses elements of air and space as the theory of five elements, while Korea uses elements of wood and iron. (J Korean Med Rehabil 2021;31(4):87-103)
