        Perinatal and Socio-Economic Risk Factors of Autism in Children attending a Tertiary Care Hospital in Bangladesh

        Autism is a neurodevelopmental disorder where genetic factors play causal role. This case-control study explored the association between perinatal and socioeconomic status with risk of autism. It was a case control study among children who came from different areas of the country during the period of 2005 to 2015 for assessment in the outdoor facility of Bangabandhu Sheikh Mujib Medical University. One hundred children were diagnosed autism by Autism Diagnostic Observation Schedule (ADOS)-Generic. Among them 79 were boys and 21 were girls. Another one hundred children without autism were taken as control, 65 of them were boys and 35 were girls. Perinatal and socioeconomic risk factors were analyzed in these two groups, using medical records. Mean age of the ASD children was 3.51+1.58 year and control was 3.80+1.53. Male predominance was observed in ASD children. After univariate analysis three significant variables identified to pose as risk for autism. These were parental advanced level of education (P=<0.001), urban residence (P=<0.001) and neonatal jaundice (P=<0.001). Variables like male sex, occupation as service holder and low birth weight at birth approached significance at the 0.05 level. Results suggested that parental profile as well as perinatal environment are associated with risk of autism and this factors act independently.

        Case report of cerebral creatine deficiency syndrome with novel mutation of SLC6A8 gene in a male child in Bangladesh

        Cerebral creatine deficiency syndrome (CCDS) is a disorder where a defect is present in transport of creatine in the brain. Creatine plays an essential role in the energy metabolism of the brain. This is a genetic disorder, autosomal recessive or X linked, characterized by intellectual disability, speech and language delay, epilepsy, hypotonia, etc. Until recently very few number of cases have been reported. Here we report a case of 1.5-year-old boy who had epilepsy (epileptic spasm and generalized tonic clonic seizure), intellectual disability, microcephaly, hypotonia and speech delay. His magnetic resonance imaging of brain showed cortical atrophy and electroencephalography showed burst suppression pattern. The diagnosis was confirmed by clinical exome sequencing which showed novel mutation of SLC6A8+ in exon 9, suggestive of X linked recessive CCDS. The patient was then treated with glycine, L-arginine and creatine monohydrate with multiple antiepileptic drugs.

        Effectiveness of Developmental Therapy in Children with Cerebral Palsy

        Cerebral palsy (CP) describes a group of motor syndromes resulting from disorders of early brain development. Physiotherapy is an important part of the management of children with CP. In Bangladesh a single approach therapy has been adopted in any medical institutes as developmental therapy where a developmental therapist assesses the child and provides physiotherapy, occupational therapy, cognitive stimulation, feeding therapy, speech and language therapy to the children according to the need by a holistic approach. There are limited published evidences on effectiveness of developmental therapy among Bangladeshi children with cerebral palsy. So this study was designed to measure the effectiveness of developmental therapy in children with cerebral palsy. A quasi-experimental study was conducted in the department of Pediatrics of Bangabandhu Sheikh Mujib Medical University from February 2009 to May 2010 on 135 children with CP aged 6 months to 3 years. The motor, mental and behavioral functions of the selected children were initially measured by Bayley Scales of Infant Development II. They were reassessed after 3 months and 6 months following developmental therapy. Only 62 (45.9%) children have completed the 2 follow up sessions and hence were evaluated against their baseline motor, mental and behavior score. The study result showed that motor, mental and behavior functions were significantly improved after 6 months following developmental therapy. Nearly half (46.8%) of the children improved on motor skills, 40.3 on mental skills and about 69% on behavior skills. Epilepsy and duration of therapy at home interfered developmental outcome. This study has demonstrated a beneficial effect of single approach developmental therapy for children with cerebral palsy.

        The status of NORMs in natural environment adjacent to the Rooppur nuclear power plant of Bangladesh

        The Rooppur Nuclear Power Plant (RNPP), the first nuclear power plant in Bangladesh with a capacity of2.4 GWe, is under construction on the bank of the river Padma, at Rooppur in Bangladesh. Measurementof background radioactivity in the natural environment adjacent to RNPP finds great importance forfuture perspectives. Soil and sediment samples collected from upstream and downstream positions ofthe Padma River (adjacent to RNPP) were collected and analyzed by HPGe gamma-ray spectrometry forprimordial radionuclides. The average activity concentrations (in Bqkg 1) of 226Ra, 232Th and 40K radionuclides in soil samples were found to be 44.99 ± 3.89, 66.28 ± 6.55 and 553 ± 82.17 respectively. Respective values in sediment samples were found to be 44.59 ± 4.58, 67.64 ± 7.93, 782 ± 108. Relevantradiation hazard indices and dosimetric parameters were calculated and compared with the worldaverage data recommended by US-EPA. Analytical results show non-negligible radiation hazards to thesurrounding populace. Measured data will be useful to monitor any change of background radioactivityin the surrounding environment of RNPP following its operation for the generation of nuclear energ
