STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.
Locator Attachment를 이용한 하악 임플란트 유지형 피개의치 임상 증례

Over the past decades, conventional complete dentures were used for various patients although they have incomplete function. Overdenture using dental implants could help the improvement of denture function. In this case, a fully edentulous patient was treated with a conventional complete denture on the maxilla and implant retained overdenture retained by 2 freestanding implants placed in the anterior region of the mandible, following recommendations from the 2002 McGill. Locator attachments were used for retention of the mandibular overdenture. Patient was satisfied with the final maxillary full denture and mandibular implant retained overdenture in esthetic and functional aspect.
치조골 흡수가 심한 하악 완전 무치악 환자에서 짧은 임플란트를 이용한 고정성 보철 수복 증례

A short dental implant is considered as possible solution in difficult clinical situations for the placement of a regular length implant. Using a short implant avoiding more invasive surgical procedures simplifies the treatment plan and shortenes the duration of treatment. In this case, 71-year-old female came up with discomfort from her old mandibular denture. As she had fully edentulous mandible and got a negative feedback from removable denture, implant-supported fixed prosthesis was planned. Six short implants were placed on her mandible with severe alveolar bone loss. After 1 year follow up period, implants were well retained with any other abnormal findings. The patient was satisfied with her prostheses and satisfactory outcomes were attained in terms of both esthetic and functional clinical results.
Dental CAD-CAM system을 통한 고정성 전악 임플란트 수복 증례

임플란트를 사용한 수복치료에서는 보철물을 위한 이상적인 위치에 임플란트가 식립되어야 한다. 치조골이 부족한 환자의 경우에는 최종 보철물을 위한 적절한 연조직 및 경조직 지지를 얻기 위해서 수술적인 처치를 포함한 구강내 처치를 필요로 하게 된다. CAD-CAM 기술을 활용한 보철물 설계는 위와 같은 수술적 처치의 필요성에 대해 임상가가 사전적으로 평가해 볼 수 있는 청사진을 제시해준다. 본 증례에서는 CAD-CAM system을 통해 환자의 치료 전구내상황, 수술적 처치에 대한 예상, 일련의 임시 보철물을 반영하여 최종 보철물을 계획하고 제작하였다. 최종 보철물을 반영하여 제작한 수술용 가이드를 사용하여 임플란트를 식립함과 동시에 치조골 증대술을 시행하였다. 이후 CAD-CAM을 활용하여, 맞춤형 지대주와 1차 및 2차 잠정 수복물을 디자인하고 제작하였으며, 환자의 구내에서 평가를 거친 2차 잠정 보철물과 동일한 형태의 최종 보철물을 제작하였다. 저작, 발음, 심미 기능에 있어서 환자와 술자 모두 만족할만한 결과를 얻을 수 있었다.
