자연분만 후 발생한 비골신경마비 환자에 대한 Muscle Energy Technique(MET)의 병행치료 효과 증례보고

Objectives: The purpose of this study is to evaluate the clinical effects of Muscle Energy Technique (MET) for peroneal nerve palsy after normal delivery. Methods: Two patients with peroneal nerve palsy were treated with acupuncture, moxibustion, cupping and MET. MET was performed in piriform, gluteus medius, anterior tibial and adductor muscles. To evaluate the effect of MET, we analyzed Ankle dorsiflexion range of motion (ROM), Manual Muscle Test (MMT), Numerical Rating Scale (NRS) and Ankle Hindfoot Scale (AHS). Results: In Case 1, ROM score was changed from -5 to 20, and MMT score was changed from 0 to 4. NRS score was changed from 5 to 1, and her AHS score was changed from 54 to 94 after treatment. In Case 2, ROM score was changed from 0 to 20, and her MMT score was changed from 1 to 5. NRS score was changed from 4 to 1, and her AHS score was changed from 64 to 97 after treatment. Conclusions: MET may be a useful treatment for patients who, shortly after childbirth or while breastfeeding, strongly refuse to treat the irritation.
교통사고로 유발된 경항통 환자의 근에너지기법(MET)의 효과에 대한 임상적 연구

Objectives: The purpose of this study is to find out the effects of muscle energy techniques on nuchal pain caused by traffic accidents. Methods: The 20 patients were divided into 2 groups: group A was treated except muscle energy techniques and group B was treated with muscle energy techniques. Both groups were treated with acupunture treatment, physical theraphy and herbal medication. Patients were evaluated by McGill Pain Questionnaire-Short Form(SF-MPQ), Pain Disability Index(PDI) and Neck Disability Index(NDI) in traffic accident patients. Results: 1. Both Group were significantly decreased in sensory of SF-MPQ, VAS, PDI and NDI after 7 days of treatment. 2. Group B compared with the Group A was significantly decresed in VAS, PDI after 7 days of treatment. Conclusions: We found out that muscle energy techniques is considered to be effective and useful on nuchal pain caused by traffic accidents.

Objectives : The purpose of this study was to explore the effectiveness of Korean medicine treatment and the Muscle Energy Technique on foot drop through a case study with a patient with common peroneal neuropathy. Methods : A patient with severe foot drop after an ankle sprain was hospitalized and treated with acupuncture and traditional medicine and the Muscle Energy Technique. The effect of the treatment was evaluated with the Numeric Rate Scale(NRS), dorsiflexion angle evaluation, and the Manual Muscle Test (MMT). Results : The results of the NRS, dorsiflexion angle evaluation and MMT showed greatly improvementwith results changing from 8 to 2, 0 to 15, and Grade 2 to Grade 4, respectively. Conclusions : This study suggests that Korean medicine treatment and the Muscle Energy Technique could be effective treatments for foot drop with common peroneal neuropathy. However further clinical studies are needed to establish a definite conclusion.
한방치료와 근에너지기법(MET)을 적용한 이상근 증후군 치험 1례

This study was performed to report the effectiveness of oriental medical treatment and muscle energy techniques on a patient with piriformis syndrome. The patient was diagnosed as piriformis syndrome and treated with acupuncture, herb medicine, physical therapy and muscle energy techniques. We have evaluated curative efficacy with measuring changes of visual analogue scale and physical examination according to period of hospitalization. After treatment, pain and visual analogue scale decreased. Physical examinations were recuperated nearly to normal findings and ambulation was improved. These results suggest that oriental medical treatment and muscle energy techniques were effective on patient with piriformis syndrome.
대요근에 대한 근에너지기법을 이용한 자세이완기법 적용: 사례연구

Background: Positional release muscle energy technique (PRMET) is a method joined positional release technique and muscle energy technique. Methods: Subjects those who have low back pain from the acute to chronic phase, were applied PRMET method on psoas major muscle and measured the changes in pain and disfunction. Results: PRMET method is effective for reducing pain and disfunction on psoas major muscle. Conclusions: The advantages of PRMET method are minimized patient inconvenience, shortening of treatment time and effective for improvement. In the future research, methods need to be improved so that this can be applied to other muscles.

Background: Proprioception in the ankle joint is important for maintaining balance. There is a correlation between joint position sense (JPS), balance and ankle dorsiflexion range of motion (DF-ROM). Objectives: The purpose of this study was to compare the effects of talocrural joint mobilization (TJM) and muscle energy technique (MET) of plantar flexor muscle (PF) on improving joint position sense (JPS) and static balance. Design: Cross-over randomized trial research. Methods: Sixteen participants (male 10, female 6; 20’s of their age) without ankle instability were recruited. In a randomized, three conditions, no intervention, talocalcaneal mobilization, and PF-MET were all applied to 16 participants. For TJM, the Kaltenborn grade 3 joint gliding method, in which the talus moves posteriorly perpendicular to the tacrocrural joint plane, were used. In the PF-MET, MET method were applied gastrocnemius muscle and soleus muscle with 25% of 1 Repeat Maximum of each muscle. The all participants performed PF-MET and TJM for 18 minutes. DF-ROM measured the weightbearing lunge test. JPS measured using the active joint angle reproduction test. Static balance was measured displacement of center of pressure parameter. Results: PF-MET and TJM had significant differences in DF-ROM and AP displacements. PF-MET significantly increased at 5° and 15° of plantar flexion and 5° of dorsiflexion, and COP velocity significantly decreased in JPS compared to TJM. Conclusion: PF-MET and TJM are effective in increasing DF-ROM. However, PF-MET has a more positive effect on improving JPS and static balance than on talocrural joint mobilization.

Objectives The purpose of this study is to investigate the clinical application of Mckenzie exercise and muscle energy techniques(MET) to three flat-back posture patients who had Lumbar Disc Herniation. Methods Three patients were diagnosed as Straightening of lumbar lordosis and lumbar disc herniation through X-ray and MRI examination. This study was measured by Numerical Rating Scale(NRS), Oswestry Disability Index(ODI) and physical examinations including lumbar spine range of movement, straight leg raising test. Results In this study, after the continuous Mckenzie exercise and muscle energy techniques(MET), patient s low back pain was improved. NRS and ODI were also decreased. And three patients showed significant improvement in physical examinations. Conclusions These results suggest that Mckenzie exercise and muscle energy techniques(MET) were effective treatments on patients who had flat-back posture.

Objectives :Muscle energy techniques(MET) are used commonly for lengthening shortened musculature, improving joint function and range of motion. This article reviews articles on MET published for recent 11 years to consider effectiveness of MET. Methods :We searched Pubmed, KSI, KERIS, KMBASE, and national assembly library using “Muscle Energy Techniques” and classified the articles into subsets of posture, pain relief, increasing the range of movement, lengthening shortened muscles and inactivation of myofascial trigger points(MTrPs). Also we evaluated their values according to the Jadad Scale. Results :A total of 20 articles reconfirmed that MET reduce pain, increases range of motion(ROM), lengthens shortened muscles(especially hamstring) and inactivates MTrPs. But domestic articles scores 0 to 1 points in Jadad Scale. Researchers should make an effort to write clinically more reliable randomized controlled trials(RCTs). Conclusions :MET may be applied on various musculoskeletal problems, and further studies analysing its effect are needed.

PURPOSE: This study compared the effect of the muscle energy technique (MET) and stretching technique on ankle dorsiflexion passive range of motion, balance, and gait ability of stroke patients with limited ankle dorsiflexion. METHODS: Forty-four post-stroke patients participated. The participants were randomized into the MET group (METG; n = 22) and the stretching group (STG; n = 22). The METG was subjected to the MET to relax the dorsiflexion, while the STG was subjected to the dorsiflexion stretching technique. Both groups completed standard neurological physical therapy for 30 min per session. The intervention was conducted five times a week over 3 weeks for a total of 15 times. All participants underwent ankle dorsiflexion passive range of motion measurement and Berg Balance Scale score determination and completed a 10-m walking test and the timed up and go test before and after the intervention. RESULTS: After the 3-week intervention, both groups showed significant improvement after the intervention (p < .05). METG participants showed greater improvements in ankle dorsiflexion passive range of motion and 10-m walking test results compared to STG participants (p < .05). CONCLUSION: Both interventions improved ankle dorsiflexion passive range of motion, balance, and gait ability in stroke patients with limited ankle dorsiflexion. Moreover, the MET was superior to ankle dorsiflexion passive range of motion on the 10-m walking test.
관절가동범위 증진을 위한 이완 기법의 적절한 수축강도와 수축시간은?

Purpose: The purpose of this study was to review articles in order to establish optimal contraction intensity and duration in the performance of relaxation techniques for maximal increase in range of motion. Methods: The Cochrane, EBSCO, Embase, Medline, ProQuest, PubMed, ScienceDirect, and Scopus databases were used to search articles from 1990 to January 2016. The search terms were "contract relax," "hold relax," "muscle energy technique (MET)," and "proprioceptive neuromuscular facilitation (PNF) stretching." Only experimental human studies (randomized controlled trials) that compared the effects of varying intensity and duration of isometric contraction were included. Non-English language and unpublished studies were excluded. Results: A total of 2,156 articles were initially identified, with only five eventually meeting the inclusion and exclusion criteria. Three studies compared the effects of varying intensity in isometric contraction and two studies compared the effects of varying duration in isometric contraction with regard to range of motion (ROM). Two articles suggested that submaximal voluntary isometric contraction was more effective than maximum voluntary isometric contraction (MVIC) in the improvement of ROM. One article showed that a longer contraction time led to greater increases in ROM. Conclusion: Submaximal voluntary isometric contraction was recommended during contract-relax exercises in healthy people. Lack of evidence makes it difficult to suggest the optimal duration of isometric contraction during relaxation techniques. For future research, high-quality evidence will be needed to establish the optimal contraction intensity for maximum improvement of ROM.
