        논문(論文) : 『향약집성방(鄕藥集成方),치문(齒門)』의 외치법(外治法)에 관한 연구(硏究)

        Objectives : This literature study about dental external treatments from Hyangyakjibsungbang?Tooth will invigorate applying the therapeutics in the field of Dentistry. Methods : In this study, the parts related to external treatment in the original text of Hyangyakjibsungbang?Tooth were reviewed. Results : Examples of frequently used external method were gargling, applying powder, and brushing one``s tooth. These methods are still used in modern dental treatment. Conclusions : Discovering new herbal medicines that provide better effects and developing more precise external treatments will advance dental treatment in Korean medicine.

        歷代醫學姓氏의 針과 鍼에 대하여

        From the part “歷代醫學姓氏(Historic Doctors)” in 『醫林撮要(Uirimchualyo)』, the following has been noticed and concluded. 1. Because acupuncture was originated from stony needle, the word “石(sok)” contains the meaning of needle, and from this point on, words like 石(sok), 砭石(pyumsok), 箴石(Jamsok), 鑱石(Chamsok) had been derived. 2. The word 砭石(pyumsok) used in 『Hwangjenaekyong(Yellow Emperor’s Canon of Medicine or Hwangdineijing)』should be interpreted as acupuncture in a verb form, not a noun form. 3. 鑱石(Chamsok) or 鑱鍼(Chamchim) was used for surgical treatment for tumor, by cutting open tumors and pressing the pus out. Therefore, 砭石(pyumsok), 鑱石(Chamsok) are the same kind of needles, and 鑱鍼(Chamchim) is the tool improved from 鑱石(Chamsok) used in the Bronze Age. 4. Kwakpak put a note on 鑱石(Chamsok) in 『山海經(Sanhaekyong)』 that reads “it is defined as 砥鍼Jichim) and treats tumor.” This let us know the shape of 石(sok), 砭石(pyumsok), 鑱石(Chamsok), 鑱鍼(Chamchim), and the stone that can be used as a surgical tool with slim & sharp shape is obsidian. 5. Because obsidian is only found around Mt. Baekdu and limited area in South Korea & Japan in Asia, it is closely related with the life & medical environment of the tribe “Mt. Baekdu”. 6. The development of 鑱鍼(Chamchim) was influenced by surgical treatment used in early stage of civilization, and its origin is traced upto Gochosun dynasty. Korea’s own traditional medical knowledge is derived from this surgical treatment skill. 7. Because the acupuncture is originated from Gochosun dynasty, 鍼(chim) was derived from 箴(Jam) of 箴石(Jamsok), 鍼(Chim) & 鍼(Chim) both were used for a time being, and finally settled into 鍼(Chim). 8. The word 針(Chim) showed up at Myung dynasty, and started to be used in Korea from early Chosun dynasty. 9. In the early Chosun dynasty, 鍼(Chim) was used for medical term, and 針(Chim) for non-medical term. In the mid Chosun dynasty, 針(Chim) was used as a term for tool, and 鍼(Chim) as a term for acupunctural medical treatment. 10. Under the order of King Sunjo, Dr. Yesoo Yang published 『醫林撮Uirimchualyo)』 added 『醫林撮要續集Sequel to Uirimchualyo)』 and added 『歷代醫學姓氏(Historic Doctors)』 again which eventually made totally 13 books of 『醫林撮要Uirimchualyo)』 In addition, many parts of 『醫林撮要續集(Sequel to Uirimchualyo)』 were quoted in 『東醫寶鑑Donguibogam)』 and influenced much in publishing 『Donguibogam』. 11. In 『歷代醫學姓氏(Historic Doctors)』 of 『醫林撮要Uirimchualyo)』 the same way in 『Donguibogam』 referred to 針(Chim) as a term for a needle, and 鍼(Chim) as a term for Acupuncture. 12. From the usage of 針(Chim) & 鍼(Chim), shown in 『鄕藥集成方(Hyangyakjipsungbang)』, 醫林撮要(Uirimchualyo)』 and 東醫寶鑑(Donguibogam)』, we can notice the spirit of doctors who tried to take over the legitimacy of Korean tradition, and their elaboration & historical view that expresses confidence on our own medical technology, through the wording 鍼(Chim).

        『어의촬요』로 본 고려시대의 의료

        『어의촬요(御醫撮要)』은 1226년(고종 13)에 최종준(崔宗峻)이 왕실에서 비전(祕傳)되어 왔던 어의들의 처방 중 다방(茶房)에서 전해오던 방문을 2권으로 간행한 고려시대 의서이다. 이 책은 유실되어 전해지지 않으나 『향약집성방(鄕藥集成方)』과 『의방유취(醫方類聚)』 등에 인용되어 일부 내용을 알 수 있다. 『어의촬요』의 인쇄 배포를 통해, 종래 어의로 대표되는 왕실이 독점한 의료지식이 좀 더 널리 퍼지게 되었다. 하지만 의학 지식의 증가와는 별개로 약물을 구하는 것이 더욱 큰 문제였다. 왕으로부터 약을 받은 유공권의 사례나 용뇌를 권력자에게 받은 이규보의 사례는 약물의 확보 여부가 권력임을 보여준다. 이러한 흐름은 단오의 옥추단(玉樞丹)이나 납일(臘日)의 전약(煎藥)과 같이 의례화된 세시 풍속을 통해 권력 과시를 보여주는 형태로 유지되었다. 그러나 이런 의학적 혜택에서 배제되었던일반 민중들은 향약이라는 대체재를 마련하면서 의료생활을 발전시켰다. Eouichwaryo(御醫撮要) was published by Choi jongjun(崔宗峻) in 1226. He compiled two volumes of the royal family s prescription, which had been handed down by the Tea Office(茶房). This book is not handed down due to its loss, but it is cited in Euibangyoochui(醫方類聚) and Hyangyakjipsungbang(鄕藥集成方) to reveal some of its contents. Through the printing and distribution of this book, the royal family s proprietary medical knowledge, represented by the royal physician, has become more widespread. However, apart from the increase in medical knowledge, finding drugs was a bigger problem. The case of Yu Gongkwon(柳公權), who received medicine from the king and the other case of Lee Gyubo(李奎報), who received the borneol(龍腦) from the powerful man, shows that securing drugs is power. This trend was maintained in the form of showing power through ritualized seasonal customs, such as the Okchudan of Dano(端午) and the decoction of medicinal herbs of Nabil(臘日). However, people who were excluded from these medical benefits developed their medical lives by preparing alternative medicines.

        鄕藥集成方・咽喉門의 外治法에 관한 硏究

        This literature review focuses on external treatments for the throat, as found in the classic Korean text, Hyang-Yak-Jib-Sung-Bang・Throat. Sections of the text related to external treatment of throat disorders were reviewed. Examples of frequently used external methods include gargling, application of powder, and brushing of the throat. These methods are still used in modern Korean Medicine treatments for Ear-Nose-Throat (ENT) disorders. This discussion is provided with intention to invigorate the ancient practices of throat treatments, by highlighting benefits and results of the therapeutic protocols described in Hyang-Yak-Jib-Sung-Bang・Throat. Discovering new effective herbal medicines and developing more precise external treatments will advance ENT treatment in Korean medicine. 본 연구는 『향약집성방‧인후』의 외치법에 대한 문헌 연구로 이비인후과 분야의 치료방법의 다양화에 도움이 된다고 생각한다. 연구 방법으로는 『향약집성방‧인후』에서 외치법과 관련된 부분을 검색하여 구체적인 외치방법과 약물 등에 대하여 분석하였다. 그 결과 자주 사용되는 외치법으로는 含漱法, 도포, 닦기 등으로 나타났다. 이러한 방법들은 요즘의 이비인후과 치료에도 그대로 적용되는 방법이다. 외치법에 사용 가능한 약물을 개발하여 더욱 효과 좋은 방법을 개발하고, 아울러 더욱 세밀한 외치법을 개발하여 향후 한방이비인후과의 치료에 도움이 되어야 한다.
