Erosive pustular dermatosis of the scalp (EPDS) is a rare inflammatory disorder of the elderly, characterized by sterile pustules and chronic erosions on the scalp, leading to cicatricial alopecia. Variety types of trauma have been reported as the predisposing factors of EPDS. A 38-year-old woman presented with a 6-year history of progressive erythematous patch with erosions and crusted pustular lesions on her scalp. The lesion initially had begun at the frontal area mimicking seborrheic dermatitis, and developed to the entire scalp. Several treatment methods including systemic and topical steroids, antibiotics and herbal medicine have been tried, but recurrence have continued. She had no history of any other medical problems or trauma history. Skin biopsy revealed mixed dermal infiltrate of lymphocytes and neutrophils. KOH test, bacterial and fungal cultures showed negative results. Systemic steroids and cyclosporine were tried and, after 2 months, the erosive and pustular lesions almost resolved. No recurrence has been observed during 6-month with topical tacrolimus. There is no single diagnostic finding or test that confirms EPDS, so the exclusion of other conditions should be needed to make a diagnosis. Herein, we present a rare case of EPDS occurred in a young woman without any trauma history. Clinicians need keep EPDS in mind when confronting relapsing inflammatory dermatosis of the scalp to prevent permanent cicatricial alopecia.

Erosive pustular dermatosis of the scalp (EPDS) is a rare disorder of the elderly characterized by multiple pustular lesions with erosions and crusting that result in scarring alopecia. EPDS typically develops in aged or sun-damaged skin with a history of trauma. Histopathologically, EPDS is nonspecific, showing atrophic epidermis and chronic inflammation. Bacteriological and mycological investigations of EPDS are generally negative. Although herpes zoster is a common disorder in elderly people, previously reported cases of EPDS were rarely associated with herpes zoster. We present a rare case of EPDS following herpes zoster treated successfully with topical tacrolimus. (Ann Dermatol 22(2) 232~234, 2010)

Erosive pustular dermatosis of the scalp is a rare disorder of the elderly and it is characterized by pustular, erosive and crusted lesions of the scalp with progressive scarring alopecia. The predisposing factors have been reported to be trauma, including mechanical trauma, skin grafting, surgery and prolonged exposure of a bald scalp to UV light. Yet the laboratory data, the bacteriological and mycological investigations and the histopathologic findings of the previously reported cases were generally negative and not diagnostic. We herein present a case of erosive pustular dermatosis of the scalp that occurred after mechanical trauma on the previous skin graft site. This type of case has not been previously reported in the Korean dermatologic literature. (Korean J Dermatol 2009;47(6):702∼705)
