The current study examines both instrument and care management by reviewing foreign long-term care systems particularily focusing on Japan and the USA. Despite a short history of public social insurance system, South Korea launched a fifth long-term care insurance from 2008 which is primarily modeled on the Japan’s long-term care insurance. Since its inception, several problems are emerged such as lack of professional care workers, inappropriatedness of instrument, and poor facilities. Although much progess has been made about the creation of home helper and increase in long-term care facilities, the development of appropriate instrument designed for eligibility still needs to be improved. America’s MDS(Minimum Data Set) has been served as multi-facet purposes. MDS has not only used for determining eligibility, but also improving qualities of care and developing indicators of quality of each long-term care facilities. Two suggestions have been made. First, More research about instrument tool needs to be done to make an accurate decision about eligibility. Second, Geriatric care manager might be trained to plan cares and advocate the elderly clients’ unmet needs.
