        Limberg flap reconstruction for sacrococcygeal pilonidal sinus disease with and without acute abscess: Our experience and a review of the literature

        Background The efficacy of Limberg flap reconstruction for pilonidal sinus with acute abscess remains unclear. This study aimed to compare outcomes after Limberg flap reconstruction for pilonidal sinus disease with and without acute abscess. A secondary objective was to perform a review of the literature on the topic. Methods A retrospective chart review was conducted of all patients who underwent excision and Limberg flap reconstruction for pilonidal sinus from 2009 to 2018. Patient demographics, wound characteristics, and complication rates were reviewed and analyzed. Results Group 1 comprised 19 patients who underwent Limberg flap reconstruction for pilonidal sinus disease without acute abscess and group 2 comprised four patients who underwent reconstruction for pilonidal sinus disease with acute abscess. The average defect size after excision was larger in group 2 than group 1 ($107.7{\pm}60.3cm^2$ vs. $61.4{\pm}33.8cm^2$, respectively). There were no recurrences, seromas or cases of flap necrosis postoperatively. There was only one revision surgery needed for evacuation of a postoperative hematoma in group 1. There were comparable rates of partial wound dehiscence treated by local wound care, hematoma, need for revision surgery and minor infection between group 1 and group 2. Conclusions Limberg flap reconstruction for pilonidal sinus in the setting of acute abscess is a viable option with outcomes comparable to that for disease without acute abscess. This practice will avoid the pain and cost associated with a prolonged local wound care regimen involved in drainage of the abscess prior to flap reconstruction.

      • Baby Ghosts: Child Spirits and Contemporary Conceptions of Childhood in Thailand

        The currently popular practice of propitiating, or ‘adopting’, child spirits in Thailand reveals an ambivalent attitude towards childhood. According to Buddhist scholars on childhood, Buddhist conceptions of children do not differentiate children in significant ways from adults in terms of their relative purity or innocence, as both children and adults possess countless lifetimes of karma; children are thus agentive beings, although not yet fully realised as adults. The child ghosts reflect the complex, competing conceptions of childhood, where they are both valuable resources to be deployed in the assistance of their families, and vulnerable beings in need of adult caretaking. Child ghosts are markers of both material and sentimental resources for their adoptive parents, or ‘guardians’. This article explores representations of child ghosts in popular media, and investigates child ghost propitiation practices through interviews with child ghost guardians. In addition, an overview is provided of the various categories of child ghosts, including kumanthong, kuman-thep, kuman-phrai, luk-krok, and rak-yom.

      • Hydrolytic and phosphorolytic metabolism of cellobiose by the marine aerobic bacterium Saccharophagus degradans 2-40T.

        <P>Saccharophagus degradans 2-40 is a marine gamma proteobacterium that can produce polyhydroxyalkanoates from lignocellulosic biomass using a complex cellulolytic system. This bacterium has been annotated to express three surface-associated 관-glucosidases (Bgl3C, Ced3A, and Ced3B), two cytoplasmic 관-glucosidases (Bgl1A and Bgl1B), and unusual for an aerobic bacterium, two cytoplasmic cellobiose/cellodextrin phosphorylases (Cep94A and Cep94B). Expression of the genes for each of the above enzymes was induced when cells were transferred into a medium containing Avicel as the major carbon source except for Bgl1B. Both hydrolytic and phosphorolytic degradation of cellobiose by crude cell lysates obtained from cellulose-grown cells were demonstrated and all of these activities were cell-associated. With the exception of Cep94B, each purified enzyme exhibited their annotated activity upon cloning and expression in E. coli. The five 관-glucosidases hydrolyzed a variety of glucose derivatives containing 관-1, (2, 4, or 6) linkages but did not act on any 관-linked glucose derivatives. All but one 관-glucosidases exhibited transglycosylation activity consistent with the formation of an enzyme-substrate intermediate. The biochemistry and expression of these cellobiases indicate that external hydrolysis by surface-associated 관-glucosidases coupled with internal hydrolysis and phosphorolysis are all involved in the metabolism of cellobiose by this bacterium.</P>
