Study Design: Multicenter, prospective study. Purpose: To investigate the effects of diabetes mellitus (DM) on surgical outcomes in patients with cervical myelopathy. Overview of Literature: To date, few studies have investigated the influence of postoperative blood glucose or glycated hemoglobin (HbA1c) levels on surgical outcomes. Methods: The participants were patients who underwent surgery for the treatment of cervical spondylotic myelopathy and ossification of the posterior longitudinal ligament. The 61 cases were evaluated preoperatively and 1 year postoperatively using the Japanese Orthopaedic Association (JOA) scores and the JOA Cervical Myelopathy Evaluation Questionnaire (JOACMEQ). The study variables included fasting blood glucose and HbA1c levels measured preoperatively and at 1 week, 4 weeks, and 1 year postoperatively; the F-wave conduction velocity, latency, rate of occurrence, and M-wave latency in the ulnar and tibial nerves were measured preoperatively and at 1 year postoperatively. The patients were divided into a group without diabetes (N group, 42 patients) and a group with diabetes (DM group, 19 patients). We then assessed the associations between the surgical outcomes and each of the study variables. Results: JOA scores significantly improved in both groups; however, no significant between-group differences were found. There was no significant improvement in the JOACMEQ scores, which assessed cervical function, upper and lower limb function, and bladder function in both groups. We then subdivided the DM group into those with a good control of HbA1c after 1 year (DMG group, 12 patients) and those with HbA1c deterioration after 1 year (DMB group, seven patients), prior to comparing the surgical outcomes. The JOACMEQ scores for upper and lower limb function significantly improved in the DMG group (p<0.01). Compared with the DMB group, there were no significant increases in upper or lower limb function scores in the DMG group. Conclusions: Poor glycemic control might prevent postoperative functional recovery of the spinal cord.

In the very high temperature reactor (VHTR), which is a next generation nuclear reactorsystem, ceramics are used as a fuel coating material and graphite is used as a core structuralmaterial. Even if a depressurization accident occurs and the reactor power goes up instantly,the temperature of the core will change only slowly. This is because the thermal capacity ofthe core is so high. Therefore, the VHTR system can passively remove the decay heat of thecore by natural convection and radiation from the surface of the reactor pressure vessel. Theobjectives of this study are to investigate the heat transfer characteristics of natural convectionof a one-side heated vertical channel with inserted porous materials of high porosityand also to develop the passive cooling system for the VHTR. An experiment was carried outusing a one-side heated vertical rectangular channel. To obtain the heat transfer and fluidflow characteristics of the vertical channel with inserted porous material, we have alsocarried out a numerical analysis using a commercial Computational Fluid Dynamics (CFD)code. This paper describes the thermal performances of the one-side heated vertical rectangularchannel with an inserted copper wire of high porosity.

<P>α2-Heremans-Schmid glycoprotein (human fetuin) is one of numerous serum proteins produced in the liver. Recently, the biological functions of fetuin, such as calcification and insulin resistance, have been clarified. However, these effects appear to be indirect, occurring through binding to other molecules. When equal amounts of fetuin in sera were treated with chymotrypsin, resistance to the protease treatment was observed in patients with pancreatic cancer, but not in normal volunteers. To investigate the molecular mechanism behind this resistance, gel-filtration chromatography was performed. The results revealed that high molecular types of fetuin showed a resistance to protease treatment. When fetuin was purified from sera of patients with pancreatic cancer and normal volunteers, certain types of proteins, including haptoglobin (which binds to fetuin derived from pancreatic cancer patients), were identified using mass spectrometry. Furthermore, the oligosaccharide structures of fetuin analyzed with lectin microarray differed between pancreatic cancer patients and normal volunteers. This macro/micro heterogeneity of fetuin might contribute to pancreatic cancer resistance to chymotrypsin treatment.</P>
Groundwater supply under land subsidence constrains in the Nobi Plain

Groundwater overdraft resulted in land subsidence throughout the Nobi Plain, central Japan. To cope with the growing water demand in the region, a numerical model was used to determine the maximum withdrawal capacity of two confined aquifers without causing undesirable consequences. Results were validated against field data and by analytical solutions. The analysis focused in Aburashima, a site expected to experience a rapid development in the forthcoming years. Calculations showed the water availability in the upper aquifer is limited. Moreover, seasonal fluctuations in heads reduce its extraction capacity up to 44%. In contrast, storage is substantially higher in the deep aquifer. Larger quantities and lower extraction costs make this layer a more reliable source for water supply. Findings from this study will be used by authorities to update the current legislation on groundwater abstraction. Nevertheless, it is argued that to achieve a long-term sustainability, policies should not limit solely to control regulations but also to economical strategies and the expansion of the infrastructure system.
