        Results of a Standard versus an Accelerated Ponseti Protocol for Clubfoot: A Prospective Randomized Study

        Background: The aim of this study was to compare the results of the standard once-weekly Ponseti casting technique to an accelerated twice-weekly regimen in our population cohort. Methods: A prospective randomized controlled study was conducted with a total of 100 consecutive patients (158 feet) being enrolled for the study. Fifty patients were randomized to each group and followed up for at least one year. Results: Initial mean Pirani score was 4.67 ± 0.73 in the standard group and 4.35 ± 0.76 in the accelerated group, and the score decreased to 0.34 ± 0.38 and 0.35 ± 0.31, respectively. Initial mean Dimeglio score was 11.75 ± 2.75 in the standard group and 10.51 ± 2.57 in the accelerated group, and the score decreased to 0.79 ± 0.77 and 0.79 ± 0.71, respectively, immediately after casting. The average number of casts required to correct all the deformities was 6.3 ± 1.2 in the standard group and 6.1 ± 1.4 in the accelerated group (p = 0.45). Average time spent in cast was 58.2 ± 8.3 days in the standard group and 39.5 ± 5.2 days in the accelerated group (p < 0.001). Percutaneous Achilles tendon tenotomy was done in 86.42% in the standard group and in 84.41% in the accelerated group (p = 0.72). Final results were assessed by using a modified functional rating scoring system: 55.55% clubfeet had excellent results and 44.45% had good results in the standard group, whereas 66.23% clubfeet had excellent results and 33.77% had good results in the accelerated group. None amongst the two groups had fair or poor results. Conclusions: These results suggest that the accelerated Ponseti technique significantly reduces the correction time without affecting the final results and that it is as safe and effective as the traditional Ponseti technique.

        Molecular characterization and differential expression of an aromatic heptaketide producing type III plant polyketide synthase from Himalayan rhubarb

        Rheum australe (Himalayan Rhubarb, Polygonaceae), an endangered medicinal and vegetable herb owes its age-old remedying properties to the bio-active phyto-constituents viz. anthraquinones, stilbenoids, chromones and dietary flavonoids. Polyketide pathway primarily involving the intricate Type III polyketide synthases (PKSs) contributes to the biosynthesis of these phyto-constituents. In the present study, we perform a homology-based approach to isolate an 1176 bp full-length cds sequence of the RaALS gene showing an equitable level of sequence similarity to related Type III PKSs at both nucleic acid and amino acid levels. In silico characterization revealed the presence of highly conserved amino acid residues found in nearly all Type III PKSs including the conserved active-site residues, signature motif and cyclization pocket residues with an exception of Ile256 and Gly258. Docking studies established major interactions between the starter acetyl-CoA and RaALS. Copy number analysis suggested slender evolution in Type III PKS in R. australe having a single copy of RaALS gene. qRT-PCR analyses revealed corroboration between the higher expression of RaALS in leaves followed by stem and root with that of the metabolite concentration. Expression studies further showed a direct increase of RaALS transcripts with the growing metabolite accretion in relation to altitude suggesting a probable involvement of specific Type III PKS in biosynthesis of the major phyto-constituents. Furthermore, abiotic stressors viz. methyl jasmonate, salicylic acid and UV light enhanced RaALS transcription hinting towards its role in defense mechanism in R. australe and highlighting the significance of RaALS as a prospective target for metabolic engineering.

        Facile synthesis of strontium selenide supported copper sulfide hybrid nanosheets as an efficient electrode for high-performance OER

        To drive clean and sustainable fuel production via water electrolysis, development of high-performing, cost-eff ective elec- trocatalysts rich in earth elements without relying on precious metals or costly materials is crucial. In this study, strontium selenide (SrSe), copper sulfi de (CuS), and composite SrSe@CuS via a traditional coprecipitate method under alkaline con- ditions are synthesized. Characterization techniques including X-ray diff raction, Transmission electron microscopy, Field emission scanning electron microscopy, and Brunauer–Emmett–Teller surface area analysis are employed to analyze the structure, morphology, and surface characteristics. The larger surface area of 123  m 2  g−1 and lower crystalline size (46.43 nm) of SrSe@CuS nanosheets show more active sites for oxygen evolution reaction. The oxygen evolution activity displayed overpotentials of 290 mV, a lower tafel slope of 67 mV  dec −1, and Lower charge transfer resistance (RCT) values of SrSe@ CuS nanosheets (1.82 Ω) surpassing the individual SrSe and CuS nanosheets. Notably, the SrSe@CuS nanosheets exhibited remarkable stability, maintaining an oxygen evolution reaction (OER) activity of 10 mA  cm −2 for over 50 h and sustaining a negligible loss in performance even after 50,000 cycles of repetitive cyclivoltammetry scans. These fi ndings highlight their potential applicability in energy conversion and storage systems.
