        Extending Fatigue Life of Out-of-plane Gusset Joint by Bonding CFRP Plates Under Bending Moment

        Carbon fiber reinforced plastic, CFRP, plates bonding method is one of the new repair methods for fatigue cracks. In this method, propagation of fatigue crack will be delay because of the suppression of crack opening by crack bridging and stress reduction by composite effect of bonding CFRP plates. In this study, to clarify the effect of bonding CFRP plates on fatigue life extension, bending fatigue tests were carried out. First, fatigue tests on the specimens in as-welded condition were conducted until fatigue crack propagated to 10mm away from the weld toe. Then, fatigue crack was covered by CFRP plates and the specimens were re-tested until fatigue crack penetrated through the base plate. As the results of bending fatigue tests, the fatigue test data of the specimens repaired by bonding CFRP plates showed much longer fatigue life compared with that in as-welded condition, under stress ratio, R=0 and -infinity.

        Prediction of Bending Fatigue Life of Cracked Out-of-Plane Gusset Joint Repaired by CFRP Plates

        Carbon fi ber reinforced polymer (CFRP), plates bonding repair method is one of the simple repair methods for cracked steel structures. In this study, the infl uence of width of CFRP plates on bending fatigue life of out-of-plane gusset joint strengthened with CFRP plates was investigated from the experimental and numerical point of view. In the bending fatigue test of cracked out-of-plane gusset joint strengthened with CFRP plates, the eff ect of width of CFRP plates on crack growth life was clarifi ed experimentally. Namely, it was revealed that the crack growth life becomes larger with increasing the width of CFRP plates. In the numerical approach, the stress intensity factor ( SIF ) at the surface point of a semi-elliptical surface crack was estimated based on the linear fracture mechanics. Furthermore, the extended fatigue life of cracked out-of-plane gusset joint strengthened with CFRP plates was evaluated by using the estimated SIF at the surface point and the empirical formula of the aspect ratio of semi-elliptical crack. As the results of numerical analysis, the estimated fatigue life of the specimen strengthened with CFRP plates showed the good agreement with the test results.

        Continuation of Enteral Nutrition and Relief from Vomiting by Administration of a New Formula: a Case Report

        Gastro-esophageal reflux (GER) is a common and serious complication in patients receiving enteral nutrition, making continuation of enteral nutrition difficult. Semi-solid enteral nutrients were developed to prevent feeding-related GER. Semi-solid enteral nutrients have high viscosity and, therefore, are typically administered through a large-diameter percutaneous endoscopic gastrostomy (PEG) tube. Recently, a new formula (Mermed<sup>®</sup>, Mermed Plus<sup>®</sup>) was introduced that uses alginate, which behaves like a gelatin in acidic conditions. This formula improved GER during enteral feedings. Our case report shows that this new formula enables the continuation of enteral nutrition via a nasogastric tube (NGT) in patients with difficulty tolerating enteral nutrition secondary to vomiting. An 86-year-old woman with an atherothrombotic cerebral infarction vomited during tube feeding, resulting in aspiration pneumonia. After 1 week, we introduced a viscosity regulator and restarted enteral feeding using a 100 mL liquid diet, but vomiting persisted. Because of the continued difficulty in tolerating enteral nutrition, the patient was transferred to our hospital. From hospital day 4, Mermed Plus<sup>®</sup> (300 mL/225 kcal, administered for 1 hour, 3 times a day) was started, eventually increasing to 535 mL/400 kcal at hospital day 5. After this, vomiting ceased. Mermed Plus<sup>®</sup> was easily administered via NGT, and its effects were immediate. This treatment appeared to improve the patient's quality of life while reducing the burden on medical staff.

        Inhaled Corticosteroid and Secondary Glaucoma: A Meta-analysis of 18 Studies

        Purpose Guidelines and systematic reviews frequently warn of inhaled corticosteroid (ICS)-induced glaucoma. However, most of the published studies deny it. Methods We performed a systematic review of randomized, cohort, nested-case control, cross-sectional studies by using Meta-analyses of Observational Studies in Epidemiology statement. Four major databases, PubMed, EMBASE, Cochrane Search Manager, and the Web of Science Core Collection as well as meta-analysis were used. Studies comparing incidence, prevalence and intraocular pressure (IOP) between patients who were treated with and without ICSs were included. A random-model meta-analysis was performed using the inverse variance method. Results Out of 623 studies screened, 18 with 31,665 subjects were finally included. No significant difference between the 2 groups was observed for crude glaucoma incidence (odds ratio [OR], 0.95; 95% confidence interval [CI], 0.86–1.04; P = 0.26; I 2 = 0%; P for heterogeneity = 0.57) as a primary endpoint, adjusted glaucoma incidence (OR, 0.90; 95% CI, 0.65–1.24; P = 0.64), crude prevalence (OR, 1.82; 95% CI, 0.23–14.19; P = 0.57), adjusted prevalence (OR, 1.22; 95% CI, 0.50–2.96; P = 0.66), IOP change during ICS treatment (mean difference [MD] +0.01 mmHg; 95% CI, −0.19–0.20; P = 0.95), and single measurement IOP (MD +0.37 mmHg; 95% CI, −0.24–0.97; P = 0.23). Time-to-event analysis for glaucoma development as one of the secondary endpoints (adjusted hazard ratio, 0.52; 95% CI, 0.28–0.96) suggested a reverse association between ICS and glaucoma. Conclusions The ophthalmological side effects of ICSs, such as glaucoma and intraocular hypertension, should not be exaggerated.

        Infl iximab Therapy Impacts the Peripheral Immune System of Immunomodulator and Corticosteroid Naïve Patients with Crohn’s Disease

        Background/Aims: Infliximab (IFX), an antibody to tumor necrosis factor, (TNF)-α has effi cacy in treating Crohn’s disease (CD). However, knowledge of the potential effects of IFX on patients’ immune profi les is lacking. The purpose of this study was to reveal the immunological effects of IFX. Methods: Twenty-two patients with a CD activity index (CDAI)of 194.2±92.9 and an average duration of disease of 3.26months and 21 healthy controls were included. Patients were to have their fi rst IFX remission induction therapy with 3 infusions (5 mg/kg) at weeks 0, 2, and 6. Oral 5-aminosalicylic acid was the only ongoing medication in the patient population. Blood samples at baseline, 12 hours after the first infusion and at week 14 were labeled with anti-CD4/CD25 antibodies for immunohistochemical measurement of regulatory T-cells (Treg). Serum cytokines and chemokines were measured by suspension array and ELISA. Results:CDAI signifi cantly decreased prior to the second IFX infusion (p<0.001). Clinical remission rates were 77.3% and 91% by the second and third infusions, respectively. At baseline, interleukin (IL)-6 (p<0.03), IL-8 (p<0.03), IL-10 (p=0.050), IL-13(p<0.01), transforming growth factor-β1 (p<0.01), and ‘regulated on activation, normal T cell expressed and secreted’(RANTES) (p<0.01) were elevated in patients. After the initial IFX infusion, TNF-α (p<0.04), IL-6 (p<0.03), interferon (IFN)-γ(p<0.04), IFN-γ-inducible protein-10 (p<0.01), monocyte chemoattractant protein-1 (p<0.01), macrophage infl ammatory protein-1β (p<0.01), and RANTES (p<0.01) were decreased. IFX infusion was associated with an increase in Treg (p<0.01)and a decrease in the Th1 (IFN-γ)/Th2 (IL-4) ratio (p<0.03). Conclusions: IFX use was associated with restoration of the Th1/Th2 balance after a single infusion and seemed to promote induction of naïve Th0 lymphocytes to Treg. This knowledge should have clinical relevance.
