        Post-Traumatic Cerebral Infarction Following Low-Energy Penetrating Craniocerebral Injury Caused by a Nail

        Post-traumatic cerebral infarction (PTCI) is a secondary insult which causes global cerebral hypoxia or hypoperfusion after traumatic brain injury, and carries a remarkable high mortality rate. PTCI is usually caused by blunt brain injury with gross hematoma and/or brain herniation. Herein, we present the case of a 91-year-old male who had sustained PTCI following a low-energy penetrating craniocerebral injury due to a nail without evidence of hematoma. The patient survived after a decompressive craniectomy, but permanent neurological damage occurred. This is the first case of profound PTCI following a low-energy penetrating craniocerebral nail injury and reminds clinicians of possibility this rare dreadful complication for care of head-injured patients.

      • Study of the Liouduei Area Hakka Industry’ Attractiveness Factors Using the Evaluation Grid Method

        The added cultural value of a product attracts consumers and a quality product is an attractive object that appeals to consumers. Therefore, the Liouduei area Hakka industry, with its rich cultural background, is also an attractive industry. The Hakka industry attracts visitors to the Liouduei area, contributes to local economy, adds to the output value, and further preserves and promotes the unique Hakka culture in the South. However, what are the attractiveness factors of the Liouduei area Hakka industry in the South? In this study, the Evaluation Grid method of the Miryoku Engineering was used to interview a total of 12 promoters, proprietors, and consumers who are highly involved with the Liouduei area Hakka Industry. These interviews identified four original attractiveness factors including: “Local Characteristics,” “Diverse and Varied Products,” “Unique Southern Hakka Culture,” and “Sense of Tradition and Nostalgia,” as well as 15 concrete attractiveness factors, and 14 abstract attractiveness factors. The attractiveness factors extracted from this study can be used by the Liouduei area Hakka Industry businesses for future reference.

        The establishment of the Council for Hakka Affairs under Executive Yuan in 2001 brought a new hope for the development and advancement of the Hakka community in Taiwan. The Hakka consciousness was on the rise in various areas of the country and the nation's cultural and creative industry was also growing. Drawing from the community's rich experience and deep cultural sentiment a unique Hakka industry combining Hakka spirit and ideology with the traditional industry was formed. The added cultural value of a product attracts consumers and a quality product is an attractive object that appeals to consumers. Therefore the Liouduei area Hakka industry with its rich cultural background is also an attractive industry. The Hakka industry attracts visitors to the Liouduei area contributes to local economy adds to the output value and further preserves and promotes the unique Hakka culture in the South.         Predicting the surgical reparability of large-to-massive rotator cuff tears by B-mode ultrasonography: a cross-sectional study

        Purpose: This study aimed to compare the ability of B-mode ultrasonography and magnetic resonance imaging (MRI) to predict the repairability of large-to-massive rotator cuff tears (RCTs). Methods: This cross-sectional study included participants with large-to-massive RCTs who underwent arthroscopic repair. B-mode ultrasonography and MRI were conducted prior to arthroscopic repair. B-mode ultrasonography was used to evaluate the echogenicity of the rotator cuff muscle using the Heckmatt scale. Intra-rater and inter-rater reliabilities were examined for two independent physicians. MRI was used to evaluate the degrees of tendon retraction, fatty infiltration of rotator cuff muscles, and muscle atrophy. Finally, two experienced orthopedic surgeons performed surgery and decided whether the torn stump could be completely repaired intraoperatively. Results: Fifty participants were included, and 32 complete repairs and 18 partial repairs were performed. B-mode ultrasonography showed good intra-rater reliability and inter-rater reliability for assessment of the muscle echogenicity of the supraspinatus and infraspinatus muscles. The correlation coefficients between B-mode ultrasound findings and MRI findings showed medium to large effect sizes (r=0.4-0.8). The Goutallier classification of the infraspinatus muscles was the MRI predictor with the best discriminative power for surgical reparability (area under the curve [AUC], 0.89; 95% confidence interval [CI], 0.81 to 0.98), while the Heckmatt scale for infraspinatus muscles was the most accurate ultrasound predictor (AUC, 0.85; 95% CI, 0.74 to 0.96). No significant differences in AUCs among the MRI and ultrasound predictors were found. Conclusion: B-mode ultrasonography was a reliable examination tool and had a similar ability to predict surgical reparability to that of MRI among patients with large-to-massive RCTs.       • Prognostic Factors in Adult Patients with Solid Cancers and Bone Marrow Metastases

        Background: Solid cancers with bone marrow metastases are rare but lethal. This study aimed to identify clinical factors predictive of survival in adult patients with solid cancers and bone marrow metastases. Methods: A total of 83 patients were enrolled consecutively between January 1, 2000 and December 31, 2012. Bone marrow metastases were confirmed by biopsies. Patient clinical features and laboratory data were analyzed for associations. Results: The median age of the patients was 54 years (range, 23-88 years), and 58% were male. The 3 most common primary tumor locations were the stomach (32 patients, 39%), prostate (16 patients, 19%), and lungs (12 patients, 15%). The median overall survival was 49 days (range, 3-1423 days). Patients with Eastern Cooperative Oncology Group performance status 1, cancers of prostate origin, platelet counts over 50,000/ml, and undergoing antitumor therapies had a significantly better prognosis in the multivariate analysis. The median survival times were 173 and 33 days for patients with 2-3 more favorable parameters (n=24) and those with 0-1 (n=69), respectively (hazard ratio 0.30; 95% CI 0.17-0.52, p<0.001). Conclusions: Solid cancers with bone marrow metastases are dismal and incurable diseases. Understanding prognostic factors to these diseases helps medical personnel to provide appropriate treatments and better inform patients about outcomes. Antitumor therapies may improve outcomes in selected patient cohorts.

        Influence of high-cycle fatigue on the tension stiffening behavior of flexural reinforced lightweight aggregate concrete beams

        The objective of this study was to experimentally investigate the bond-related tension stiffening behavior of flexural reinforced concrete (RC) beams made with lightweight aggregate concrete (LWAC) under various high-cycle fatigue loading conditions. Based on strain measurements of tensile steel in the RC beams, fatigue-induced degradation of tension stiffening effects was evaluated and was, compared to reinforced normal weight concrete (NWC) beams with equal concrete compressive strengths (40 MPa). According to applied load-mean steel strain relationships, the mean steel strain that developed under loading cycles was divided into elastic and plastic strain components. The experimental results showed that, in the high-cycle fatigue regime, the tension stiffening behavior of LWAC beams was different from that of NWC beams; LWAC beams had a lesser reduction in tension stiffening due to a better bond between steel and concrete. This was reflected in the stability of the elastic mean steel strains and in the higher degree of local plasticity that developed at the primary flexural cracks.

        A low-cost and environmentally-friendly chitosan/spent coffee grounds composite with high photothermal properties for interfacial water evaporation

        The present study explores the remarkable photothermal properties and potential applications exhibitedby melanoidin-enriched spent coffee grounds (SCGs). However, the inherent challenge lies in the substantialparticle size of SCGs, leading to rapid sedimentation within the medium and resulting in unevenheat distribution during photothermal analysis. To overcome this obstacle, acid hydrolysis was employedto liquefy the SCGs, allowing their dispersion in water and significantly enhancing their light absorptioncapacity. The photothermal properties of liquefied SCGs (LSCGs) were analyzed under 808-nm nearinfrared(NIR) irradiation and compared with those of SCGs. Remarkably, the photothermal conversionefficiency of the LSCGs was as high as 52%, twice that of SCGs. To further explore their potential, theLSCGs were incorporated into chitosan (CS) to create homogenous and porous CS/LSCGs composites. These low-cost and environmentally friendly CS/LSCGs composites, made entirely from waste materials,demonstrated photothermal properties under NIR irradiation. Notably, they exhibited excellent performancein multiple cycles of water evaporation experiments, achieving an impressive water evaporationefficiency of nearly 90%.
