        All-dielectric planar solar cells with multilayer ARC and non-periodic DBR nanolayers based on transmission line equivalent circuit

        In this paper, an analytical approach based on transmission line equivalent circuit (TLEC) is used to model planar solar cells. The proposed method has been applied to the all-dielectric planar solar cells with multilayer anti-reflection coating (ARC) and non-periodic distributed Bragg reflector (DBR) nanolayers. This analytical approach is very efficient and easy to be implemented. Consequently, it is straightforward to employ several optimization procedures to determine the optimum thickness for different layers. In this paper, optimization process according to the Fuzzy adaptive modified particle swarm optimization (FAMPSO) algorithm is utilized to maximize the absorption enhancement of these types of solar cells. Results demonstrate the possibility of wideband absorption in all-dielectric planar solar cells and comparable to the conventional light trapping structures.

        Therapeutic Outcomes of Pectoralis Major Muscle Turnover Flap in Mediastinitis

        Background: This study aimed to evaluate the therapeutic results and safety of pectoralis major muscle turnover flaps in the treatment of mediastinitis after coronary artery bypass grafting (CABG) procedures. Methods: Data regarding 33 patients with post-CABG deep sternal wound infections (DSWIs) who underwent pectoralis major muscle turnover flap procedures in the Emam Reza and Ghaem Hospitals of Mashhad, Iran were reviewed in this study. For each patient, age, sex, hospital stay duration, remission, recurrence, and associated morbidity and mortality were evaluated. Results: Of the 2,447 CABG procedures that were carried out during the time period encompassed by our study, DSWIs occurred in 61 patients (2.5%). Of these 61 patients, 33 patients (nine females [27.3%] and 24 males [72.7%]) with an average age of 63±4.54 years underwent pectoralis major muscle turnover flap placement. Symptoms of infection mainly occurred within the first 10 days after surgery (mean, 10.24±13.62 days). The most common risk factor for DSWIs was obesity (n=16, 48.4%) followed by diabetes mellitus (n=13, 39.4%). Bilateral and unilateral pectoralis major muscle turnover flaps were performed in 20 patients (60.6%) and 13 patients (39.4%), respectively. Complete remission was achieved in 25 patients (75.7%), with no recurrence in the follow-up period. Four patients (12.1%) needed reoperation. The mean hospitalization time was 11.69±6.516 days. Four patients (12.1%) died during the course of the study: three due to the postoperative complication of respiratory failure and one due to pulmonary thromboembolism. Conclusion: Pectoralis major muscle turnover flaps are an optimal technique in the treatment of post-CABG mediastinitis. In addition to leading to favorable therapeutic results, this flap is associated with minimal morbidity and mortality, as well as a short hospitalization time.

        Performance Analysis of a High-Speed All-Optical Subtractor using a Quantum-Dot Semiconductor Optical Amplifier-Based Mach-Zehnder Interferometer

        This paper presents the simulation and design of an all-optical subtractor using a quantum-dot semiconductor optical amplifier Mach-Zehnder interferometer (QD-SOA MZI) structure consisting of two cascaded switches, the first of which produces the differential bit. Then the second switch produces the borrow bit by using the output of the first switch and the subtrahend data stream. Simulation results were obtained by solving the rate equations of the QD-SOA. The effects of QD-SOA length, peak power and current density have been investigated. The designed gate can operate at speeds of over 250 Gb/s. The simulation results demonstrate a high extinction ratio and a clear and wide-opening eye diagram.

        New candidates for treatment and management of carpal tunnel syndrome based on the Persian Canon of Medicine

        Carpal tunnel syndrome (CTS) is defined as a compressing median mononeuropathy. CTS is one of the major costly debilitating diseases of the hand. Although CTS is a relatively recent concept in current medicine, some evidences show that medieval physicians in Persian medicine (PM) such as Avicenna were familiar with it. The PM textbook written by Avicenna, the Canon of Medicine, defines the anatomy of carpal tunnel and median nerve, as well as mononeuropathy; it also offers suggestions for the prevention and treatment of carpal tunnel syndrome (called as Vaja al-asab and Khadar) in the chapter of nerve diseases. The book describes not only symptoms including pain, paresthesia, hypoesthesia, tingling, and numbness, but also its etiology such as nerve compression (entrapment neuropathy); nonphysical reasons such as disturbed balance among the four body humors; alteration in the nerve's temperament (Mizaj) that prevents the transmission of nerve impulses; and the others such as nutrition, mental condition, sleep, weather condition, body movements, and proper disposal of body waste. Furthermore, the book suggests a lifestyle modification method based on six factors and 10 prescriptions composed with 85 natural products that are not actively used for CTS treatment in modern times. The medicinal suggestions for CTS in the Canon of Medicine will be good candidates for discovering new treatments besides providing historical significance to the various insights considered 1000 years ago.

      • Incidence, Prevalence, and Mortality Rate of Gastrointestinal Cancer in Isfahan, Iran: Application of the MIAMOD Method

        Gastrointestinal cancers remain the most prevalent cancers in many developing countries such as Iran. The aim of this study was to estimate incidence, prevalence and mortality, as well as time trends for gastrointestinal cancers in Isfahan province of Iran for the period 2001 to 2010 and to project these estimates to the year 2020. Estimates were driven by applying the MIAMOD method (a backward calculation approach using mortality and relative survival rates). Mortality data were obtained from the Ministry of Health and the relative survival rate for all gastrointestinal cancers combined was derived from the Eurocare 3 study. Results indicated that there were clear upward trends in age adjusted incidence (males 22.9 to 74.2 and females 14.9 to 44.2), prevalence (males 52.6 to 177.7 and females 38.3 to 111.03), and mortality (males 14.6 to 47.2 and females 9.6 to 28.2) rates per 100,000 for the period of 2001 to 2010 and this upward state would persist for the projected period. For the entire period, the male to female ratio increased slightly for all parameters (incidence rate increased from 1.5 to 1.7, prevalence from 1.4 to 1.6, and mortality from 1.5 to 1.7). In males, totals of 2,179 incident cases, 5,097 prevalent cases and 1,398 mortality cases were predicated to occur during the study period. For females the predicted figures were 1,379, 3,190 and 891, respectively. It was concluded that the upward trend of incidence alongside increase in survival rates would induce a high burden on the health care infrastructure in the province of Isfahan in the future.

        Promotion of excisional wound repair by a menstrual blood-derived stem cell-seeded decellularized human amniotic membrane

        This is the first study demonstrating the efficacy of menstrual blood-derived stem cell (MenSC) transplantation viadecellularized human amniotic membrane (DAM), for the promotion of skin excisional wound repair. The DAM wasseeded with MenSCs at the density of 3 9 104 cells/cm2 and implanted onto a rat’s 1.50 9 1.50 cm2 full-thicknessexcisional wound defect. The results of wound closure and histopathological examinations demonstrated that the MenSCseededDAM could significantly improve the wound healing compared with DAM-treatment. All in all, our data indicatedthat the MenSCs can be a potential source for cell-based therapies to regenerate skin injuries.
