Retrograde intrarenal surgery: Past, present, and future

With the recent technological advancements in endourology, retrograde intrarenal surgery has become a more popular procedure for treatment of urolithiasis. Furthermore, since the introduction of new laser systems and advanced flexible ureteroscopy with miniaturized ureteroscopes, the treatment indications for retrograde intrarenal surgery have expanded to include not only larger renal stones of >2 cm but also upper urinary tract urothelial carcinoma, ureteral stricture, and idiopathic renal hematuria. Clinicians must keep up with these trends and make good use of these technologies in the rapidly changing field of endourology. Simultaneously, we must consider the risk of various complications including thermal injury due to laser use, ureteral injury caused by the ureteral access sheath, and radiation exposure during retrograde intrarenal surgery with fluoroscopic guidance. This review focuses on the past, present, and future of retrograde intrarenal surgery and provides many topics and clinical options for urologists to consider.
The effect of ambient air flows on spray characteristics of a pressure atomizer

This research investigated the effect of the inner-swirl and outer-swirl flows arranged around a pressure atomizer on the spray characteristics. Water and air were used as a test liquid and an ambient gas, respectively. Spray shape was visualized by using laser tomographic method. Droplet-size and velocity were measured using a phase Doppler particle analyzer (PDPA). The air velocity was measured by PDPA using a tracer particle. The results showed that the spray shape was a hollow-cone. In the case with the inner-swirl flow, Sauter mean diameter and spray angle were small as compared with the case without inner-swirl flow. On the other hand, when the air was supplied as the outer-swirl flow, Sauter mean diameter was relatively large, while the spray angle was relatively constant and spray sheet became thinner. Recirculation zone was generated around the center of the hollow-cone spray. In the case with inner-swirl and outer-swirl flow, droplets size was relatively small. The supply of inner-swirl flow made the circulation region shift downstream.

To successively synthesize star-branched polymers, we developed a new iterative methodology which involves only two sets of the reactions in each iterative process: (a) an addition reaction of DPE or DPE-functionalized polymer to a living anionic polymer, and (b) an in-situ reaction of 1-(4-(4-bromobutyl)phenyl)-1-phenylethylene with the generated 1,1-diphenylalkyl anion to introduce one DPE functionality. With this methodology, 3-, 4-, and 5-arm, regular star-branched polystyrenes, as well as 3-arm ABC, 4-arm ABCD, and a new 5-arm ABCDE, asymmetric star-branched polymers, were successively synthesized. The A, B, C, D, and E arm segments were poly(4-trimethylsilylstyrene), poly(4-methoxystyrene), poly(4-methylstyrene), polystyrene, and poly(4-tert-butyldimethylsilyloxystyrene), respectively. All of the resulting star-branched polymers were well-defined in architecture and precisely controlled in chain length, as confirmed by SEC, $^1H$ NMR, VPO, and SLS analyses. Furthermore, we extended the iterative methodology by the use of a new functionalized DPE derivative, 1-(3-chloromethylphenyl)-1-((3-(1-phonyletheny1)phenyl) ethylene, capable of introducing two DPE functionalities via one DPE anion reaction site in the reaction (b). The number of arm segments of the star-branched polymer synthesized by the methodology could be dramatically increased to 2, 6, and up to 14 by repeating the iterative process.
FEASIBILITY OF LANDFILL RECLAMATION IN JAPAN

In this paper, current situation of landfilling in Japan is presented, and future research need for landfill reclamation is discussed. Japan, with very high land utilization, has very little landfill capacity left, and acquiring a new landfill site is a challenging task due to objection from local residents and high land cost. Industrial and municipal waste landfills have left only less than three and ten years capacity, respectively. On the other hand, on march 1998, Ministry of Health and Welfare has published a list of 538 inappropriate municipal landfill sites with no liner, leachate treatment facility, or both, and requiring immediate corrective action. Therefore, early stabilization and reclamation are now crucially important for combating the final disposal problem.
Primary Multiple Cardiac Myxomas in a Patient without the Carney Complex

Cardiac tumors are rare, and multiple myxomas are even rarer. The latter phenomenon is mostly associated with the Carney complex, a dominantly inherited disease characterized by multiple primary cardiac myxomas, endocrinopathy, and spotty pigmentation of the skin. We report the rare case of a patient who did not have the Carney complex but had multiple primary cardiac tumors. A 78-year-old woman with a past history of breast cancer was referred to our hospital for further examination of multiple cardiac tumors. Echocardiography showed 4 tumors in the left atrium and left ventricle. We could not diagnose them preoperatively and decided to resect them surgically because they were mobile and could have caused embolism and obstruction. The postoperative pathological findings of all 4 tumors were myxomas, although the patient did not meet the diagnostic criteria of the Carney complex. Therefore, a rare case of multiple primary cardiac myxomas was diagnosed.
