        Strongyloides stercoralis and other intestinal parasites in patients receiving immunosuppressive drugs in northern Iran: a closer look at risk factors

        OBJECTIVES: The objective of this study was to evaluate the prevalence of Strongyloides stercoralis and other intestinal parasites in patients receiving immunosuppressive drugs in northern Iran and to investigate related risk factors. METHODS: This cross-sectional study was conducted among 494 patients receiving immunosuppressive drugs, including cancer patients undergoing chemotherapy (n=188) and those treated with prolonged corticosteroid administration (n=306). All fresh fecal samples were examined using the direct wet-mount, formalin ethyl acetate concentration, and agar plate culture techniques. RESULTS: In total, 16.8% of patients were positive for at least 1 intestinal parasite; the helminthic and protozoan infection rates were 5.1% and 12.3%, respectively. The infection rate was significantly higher in corticosteroid-treated individuals (19.6%) than cancer patients (12.2%) (p<0.05). The prevalence rate of S. stercoralis among patients receiving chemotherapy and those treated with corticosteroids were 4.3% and 5.2%, respectively. The prevalence rate of S. stercoralis infection was significantly higher in older patients (p<0.05). CONCLUSIONS: Strongyloidiasis is one of the most common parasites among patients receiving immunosuppressive drugs in northern Iran. Early diagnosis and proper treatment of these patients are necessary to minimize the complications of severe strongyloidiasis.

        Mucoadhesive Chitosan Electrospun Nanofibers Containing Tetracycline and Triamcinolone as a Drug Delivery System

        The mucoadhesive Chitosan (CS) nanofibers as a drug delivery system were developed. Chitosan was modified via the immobilization of thiol groups from L-cysteine as a mucoadhesive reagent. The mucoadhesive properties of the chitosan nanofibers were evaluated by tensiometer set and via tensile studies. Drug and mucoadhesive agent loading lead to decrease diameters and increased porous of nanofibers. The release of Tetracycline (Tet) and Triamcinolone (Tri) were increased with increasing immersion time and it became constant at long immersion times. Mucoadhesion studies were done at pH 2-7 and in pH 6 maximum mucoadhesive properties observed. Release studies demonstrated a sustained release of both drug continued up to 48 hours. Microbial studies were performed on the nanofibers. The drug delivery system represented a novel tool for improve the therapeutic efficacy of various drugs that are poorly absorbed from the gastrointestinal tract. Also it is an efficient system for treatment of oral ulceration.

        ZPD-Based Dynamic Assessment and Collaborative L2 Vocabulary Learning

        Despite growing interest in the implementation of dynamic assessment in second or foreign language learning research, few studies have attempted to incorporate the notion into real second language classrooms. This study explored the effect of cumulative Group-Dynamic Assessment on depth of vocabulary knowledge in an EFL context. To this end, 50 EFL learners were selected from a junior high school in Iran after administering the Oxford Quick Placement Test. Then, they were randomly assigned to two equal groups, namely, experimental Group-Dynamic Assessment and control Non- Dynamic Assessment. The Group-Dynamic Assessment group was instructed through interactionist cumulative Group-Dynamic Assessment procedures while the control group was taught without providing any cumulative ZPD-sensitive feedback. Before and after the instruction, a vocabulary knowledge scale (Paribakh & Wesche, 1993, 1996) was administered to measure both groups’ depth of second language vocabulary knowledge. The split-plot ANOVA results revealed that the implementation of interactionist cumulative Group-Dynamic Assessment helped the learners outperform the Non-Dynamic Assessment group on both immediate and delayed posttests. Additionally, complementary qualitative analysis showed that cumulative Group-Dynamic Assessment had both diagnostic and developmental potentials to contribute not only to individual participants but also to the whole class to increase their depth of vocabulary knowledge. The findings pointed to the use of ZPD-based collaborative frameworks in teaching second language vocabulary in meaningful contexts and thereby helping learners develop deeper word knowledge.
