A study of clinical effect of iontophoresis mask patch using reverse electrodialysis technique

Background: Previously, the reverse electrodialysis (RED) system was found to improve transdermal drug delivery compared to passive diffusion. Objectives: To evaluate antiaging effect and safety profile of iontophoresis mask patch using RED technique. Methods: This study was a prospective, spilt-face, controlled trial comparing iontophoresis mask patch using RED technique with passive diffusion mask patch. Clinical studies were performed on 20 Korean women for 5 weeks. We applied iontophoresis mask patch using RED on the right side of the face and passive diffusion mask patch on the left side of the face and treated four times at weekly intervals. Both mask patches contain same materials (SYN-AKE, oligo hyaluronic acid, leuphasyl, argireline etc.). Clinical effect was assessed by measuring erythema index, melasma index, stratum corneum hydration, and skin roughness at baseline, week1 (after 1 treatment), week 4 (after 4 treatment), and week 5 (1 week after 4 treatment). Results: After 4 treatment, improvements in the erythema index, stratum corneum hydration and skin roughness were significantly greater in the test side than in the control side. However, there was no statistically significant improvement in the melsma index on both side. Conclusion: This study suggests that iontophoresis mask patch using RED has a greater effect on antiaging than passive mask patch.
Clinical analysis of 69 cases of skin tumors on the fingers in Korea

Background: The patient with an undifferentiated mass of the finger is frequently encountered in the clinic. But there is little discussion of tumors arising in the fingers in the dermatology literature. Objectives: To investigate the clinical characteristics of digit tumors. Methods: We retrospectively reviewed clinical photographs and medical records of 69 patients who visited Kangbuk Samsung Hospital for digit tumors between January 2013 and July 2017. The diagnosis are confirmed by skin biopsy. Results: The mean patient age was 48.57 years old, and the male-to-female ratio was 1:1.09. The most common digit tumor was pyogenic granuloma, which accounted for 34.78 % of the total number followed by mucous cysts (21.74%), masson’s hemangioma (7.25%). Furthermore, the most common digit tumor on the volar surface was identified to pyogenic granuloma (57.14%), and the most common digit tumor on the dorsal surface was revealed to mucous cyst (44.12%). Conclusion: Our study shows that various digit tumors can occur. Further studies with more large group are needed to better understand the incidence or site predilection of digit tumors.
Squamous cell carcinoma presenting as cellulitis

In general, skin squamous cell carcinoma (SCC) presents as papules or plaques with erythematous or pigmented appearance that may ulcerate the skin. There are few reports from literature that SCC presents as skin cellulitis. By knowing the unusual presentations of SCC and diagnosing SCC in the early stage, advanced metastasis of the disease can be prevented and good prognosis with modest mean survival rate can be achieved. A 73-year-old female presented with diffuse erythematous patches with heating sense on left thigh. The patient received wide excision of well-differentiated squamous cell carcinoma of left shin 4 years ago. The patient was treated as an outpatient for cellulitis with antibiotics, which produced almost complete improvement after 6 months. However, multiple tiny erythematous papules with diffuse erythematous patches on left thigh were persistent after 6 months of antibiotics treatment. Punch biopsy of the lesion confirmed well-differentiated, microinvasive squamous cell carcinoma. The histologic examination of the lesion showed scattered atypical squamoid and basaloid tumor nests in the upper and deep dermal layer. Herein, we report a case with squamous cell carcinoma presenting as cellulitis.

Guttate psoriasis is a distinctive form of psoriasis that typically affects children, adolescents, and young adults. The lesions are 0.5-1.5 cm in diameter located on the upper trunk and proximal extremities. Streptococcal throat infection frequently precedes the onset of guttate psoriasis. However, vaccination is also reported to cause guttate psoriasis in some cases. The precise mechanism leading to guttate psoriasis after vaccination is unknown. There is a hypothesis that vaccination may be a triggering factor of guttate psoriasis by stimulating the immune system with an unexplained mechanism. A 68-year-old female presented with multiple scattered brown to erythematous scaly patches and plaques on the trunk and extremities, which occurred 5 days after influenza vaccination. The patient was otherwise healthy and had revealed no other disease within the preceding month. Histological findings of the lesion revealed mild spongiosis, exocytosis, superficial perivascular lymphohistiocytic infiltration, dilated blood vessels in the papillary dermis and focal subcorneal pustules. Finally, the patient was diagnosed with guttate psoriasis. The lesions were improved with narrow-band UVB and alitretinoin (Alitoc<sup>®</sup>, 30 mg/day). We report a case with guttate psoriasis following influenza vaccination.

Background: Congenital melanocytic nevus may be defined as a lesion present at birth and containing nevus cells. Maturation in the melanocytic nevus means a conversion of pigment-producing large, clear type A cells to nerve-like type C cells through lymphocytoid type B cells. Objectives: This study was performed to investigate the clinical and pathologic characteristics of congenital melanocytic nevus and associated factors with nevus cell types in Korean population. Methods: This study was performed with patients histologically confirmed as congenital melanocytic nevus between 2007 and 2015. The proportion and region of interest (ROI) of type A, B and C cells were calculated using ‘image J’ program. Statistical differences were assessed using the Kruskal-Wallis test and Mann-Whitney test. Results: Mean size of nevus was 2.7±2.0 cm, and the most common location of the lesion was head and neck (n=20, 32.3%). Depth of the nevus cell and the nevus cell infiltration to the eccrine gland positively correlate with the proportion of the type C cell to the type A+B cell (p<0.05), and angioadnexocentric nevus was reported at 4 cases (4.84%). Conclusion: As the depth of the nevus cell and the nevus cell infiltration to the eccrine gland increase, the proportion of the type C cell to the type A+B cell markedly increases. Clinicopathologic features of angioadnexocentric nevus identified at this study are identical to previous studies.

Effects of egg york containing IgY specific for Helicobacter pylori on the bacterial growth and intragastric infection were investigated in comparison with a proton-pump inhibitor pantoprazole. For in vitro antibacterial activity test, H. pylori (1*108 CFU/mL) was incubated with a serially diluted IgY for 3 days. As a result, IgY fully inhibited the bacterial growth at 16 mg/mL, which was determined to a minimal inhibitory concentration. In vivo elimination study, male C57BL/6 mice were infected with the bacteria by intragastric inoculation (1*108 CFU/mouse) 3 times at 2-day intervals, and 2 weeks later, orally treated twice a day with 50, 100, 200 or 500 mg/kg IgY for 18 days. After the final administration, biopsy sample of the gastric mucosa was assayed for the bacterial identification via urease, oxidase, catalase, nitrate reduction and H2S tests in addition to microscopic examination for mucosal inflammation. In CLO kit test, 75, 50, 12.5 and 12.5% of the animals revealed positive reaction following treatment with 50, 100, 200 and 500 mg/kg IgY, respectively, resulting in a superior efficacy at 200 mg/kg than 30 mg/kg pantoprazole that displayed 75% elimination. The CLO test results were confirmed by bacterial identification. Microscopic examination revealed that H. pylori infection caused severe gastric mucosal inflammation, which were not observed in the CLO-negative mice following treatment with IgY or pantoprazole. Taken together, IgY inhibited the growth of H. pylori, and improved gastritis and villi injuries by eliminating the bacteria from the stomach. The results indicate that IgY could be a good candidate overcoming tolerance of antibiotics for the treatment of H. pylori-mediated gastric ulcers.
A case of lipidized dermatofibroma

Lipidized dermatofibroma is a very rare variant of dermatofibroma (DF). Lipidized DF occupies 2.1% among the variants of DF. It preferentially affects the lower limb, so it has been called “ankle-type” fibrous histiocytoma. Clinically, it presents as a solitary large exophytic yellowish to brownish nodule. Histologically, it shows acanthosis, irregular elongation of the rete ridge and fibroblast-like spindle cells in storiform pattern which are characteristics of the DF. Moreover it also contains abundant sclerotic collagen bundles surrounding foamy macrophages. Also, there is a hypothesis of relationship between serum lipid profile abnormality and lipidized DF. A 59-year-old female presented with asymptomatic solitary red to brownish 1.5-cm sized nodule on the left shin and solitary erythematous 1-cm sized nodule on the right calf, which were detected a year ago. Histological findings of the lesion revealed foamy macrophages surrounded by abundant sclerotic collagen bundles, distinctive stromal hyalinization, and spindle cells arranged in a storiform pattern. No cytologic atypia was observed. Abnormality of serum lipid profile was accompanied. Consequently, this case was diagnosed with lipidized DF.

Background: Laser treatment has emerged as a novel treatment modality for onychodystrophy. Indeed, only a few small series have assessed the efficacy of laser treatment in onychodystrophy. Recently, the picosecond-domain neodymium-doped yttrium aluminum garnet (Nd:YAG) laser has been introduced for the improvement of various skin disorders Objectives: The aim of this study was to verify the efficacy and safety of a 1064 nm picosecond Nd:YAG (PSNY) laser in the management of idiopathic onychodystrophy. Methods: Retrospective chart reviews and photographic analyses were performed. Nail lesions were irradiated with a low fluence 1064 nm PSNY. Clinical improvement was assessed by image analysis software Image J and two blinded dermatologists using 5-point global assessment scale. Results: 5 patients (3 males and 2 females, mean age 41 ± 27 years) were analyzed. A total of 12 nails, consisting of 11 finger nails and 1 toe nail, were treated with a 1064 nm PSNY. The mean treatment session was 7.4 ± 4.1. After receiving a 1064nm PSNY laser therapy, 5 patients (100 %) showed more than 50 % improvement. There were no serious adverse events associated with the treatment during follow-up period. Conclusion: In this report, we present that idiopathic onychodystrophy successfully treated with a 1064nm PSNY laser. This device offers a new and potentially therapeutic modality for the patients with idiopathic onychodystrophy.

Minocycline has increasingly been associated with different adverse reactions including drug-induced lupus (DIL). DIL is a less well-defined syndrome. Patients with DIL usually show the clinical features of polyarthralgia, polyarthritis often accompanied by liver abnormalities and positive antinuclear antibody test results. Representative dermatological manifestations are subcutaneous nodules, rash, livedo reticularis, oral ulceration, alopecia. A 20-year-old female presented with multiple erythematous, slightly elevated tender subcutaneous nodules on back, thigh and forearm. The patient had a past history of taking minocycline (Minocin<sup>®</sup>, 50 mg/day) steadily for 10 months with acne. Two weeks after taking minocycline (100 mg/day) again due to recurrence of acne, new skin lesions as well as arthralgia were newly developed. Laboratory test for anti-histone antibodies and anti-SSA were positive. Histological findings of the skin lesion revealed mild septal panniculitis with mild lymphoid lobular infiltration. Finally, the patient was diagnosed with DIL. After stopping minocycline and taking hydroxychloroquine sulfate (Oxiklorin<sup>®</sup> 200 mg/day) and methylprednisolone (Somelon<sup>®</sup> 12 mg/day), arthralgia completely disappeared within 2 weeks. Herein, we report this patient as a rare and educational case of DIL triggered by long-term exposure to minocycline.
