      • Clinical Comparison between Paclitaxel Liposome (Lipusu<sup>®</sup>) and Paclitaxel for Treatment of Patients with Metastatic Gastric Cancer

        Aim: To compare the efficacy and safety of paclitaxel liposome (Lipusu$^{(R)}$) with paclitaxel in combination with tegafur and oxaliplatin in treating patients with advanced gastric cancer. Materials and Methods: Patients with advanced gastric cancer receiving chemotherapy were retrospectively collected, and divided into two groups. Patients in group A received paclitaxel liposomes at a dose of 135 $mg/m^2$ on day 1 of each cycle, and patients in group B were given paclitaxel at the same dose with the same timing. All patients received tegafur at a dose of 500 $mg/m^2$ on days 1-5, and oxaliplatin at a dose of 80-100 $mg/m^2$ on day 1 for 2 cycles (each cycle was 21 d in total). Results: Fifty-eight patients could be evaluated for efficacy. The overall response rate was 47% in group A (14/30), and 46% in group B (13/28). Disease control rate was 73% in group A (22/30), and 71% in group B (20/28) (P>0.05). No significant differences were detected in hematologic and neurologic toxicities between the two groups (P>0.05). However, nausea, vomiting and hypersensitive reactions were significantly lower in group A than in group B (P<0.05). Conclusion: Paclitaxel liposomes are as effective as paclitaxel when combined with tegafur and oxaliplation in treating patients with advanced gastric cancer, but adverse reactions with paclitaxel liposomes are less common.

      • Bevacizumab Regulates Cancer Cell Migration by Activation of STAT3

        There are numerous clinical cases indicating that long-term use of bevacizumab may increase the invasiveness of tumors. However, to date, little is known about underlying molecular mechanisms. Therefore, the purpose of our study was to investigate effects of bevacizumab in four cancer cells lines (WSU-HN6, CAL27, Tca83, and HeLa). It was found to promote migration and invasion in the WSU-HN6 and Tca83 cases, while exerting inhibitory effects in CAL27 and HeLa cells. The signal transducer and activator of transcription (STAT) 3 inhibitors niclosamide and S3I-201 inhibited the STAT3 signal pathway, which is activated by bevacizumab. These inhibitors also substantially blocked bevacizumab-induced migration of WSU-HN6 and Tca83 cells. Bevacizumab upregulated interleukin (IL)-6 and phosphorylated (p)-STAT3 expression time-dependently. Therefore, we propose that bevacizumab has differential effects on the migration of different cancer cell lines and promotes migration via the IL-6/STAT3 signaling pathway.

        Remarkable impact of steam temperature on ginsenosides transformation from fresh ginseng to red ginseng

        Background: Temperature is an essential condition in red ginseng processing. The pharmacological activities of red ginseng under different steam temperatures are significantly different. Methods: In this study, an ultrahigh-performance liquid chromatography quadrupole time-of-flight tandem mass spectrometry was developed to distinguish the red ginseng products that were steamed at high and low temperatures. Multivariate statistical analyses such as principal component analysis and supervised orthogonal partial least squared discrimination analysis were used to determine the influential components of the different samples. Results: The results showed that different steamed red ginseng samples can be identified, and the characteristic components were 20-gluco-ginsenoside Rf, ginsenoside Re, ginsenoside Rg1, and malonyl-ginsenoside Rb1 in red ginseng steamed at low temperature. Meanwhile, the characteristic components in red ginseng steamed at high temperature were 20R-ginsenoside Rs3 and ginsenoside Rs4. Polar ginsenosides were abundant in red ginseng steamed at low temperature, whereas higher levels of less polar ginsenosides were detected in red ginseng steamed at high temperature. Conclusion: This study makes the first time that differences between red ginseng steamed under different temperatures and their ginsenosides transformation have been observed systematically at the chemistry level. The results suggested that the identified chemical markers can be used to illustrate the transformation of ginsenosides in red ginseng processing.

        Occupational Health Protection for Health Workers in China With Lessons Learned From the UK: Qualitative Interview and Policy Analysis

        Background: Healthcare settings have been recognized among the most hazardous places to work. Based on the five categories of occupational hazards that were identified by the ILO and WHO, this study aimed to analyze policy framework relevant to occupational health protection of health workers (HWs) in public health service in China, then discussed how to share the experience of the National Health Service (NHS) England for improvement. Methods: Based on policy learning theories, policy analysis and qualitative interview were used in this study. Results: In the Chinese public health service, at least five laws related to the regulation of occupational health protection for HWs; however, enforcement of relevant laws was separated and multi-centered; the national monitoring system, which targeted to occupational hazards and health outcome for HWs in China, had yet to be developed; the top three priorities were workplace violence, bloodborne pathogens, and musculoskeletal disorders; national strategies included Security Hospital, and Healthy China 2030. In NHS England, three laws were fundamental; several monitoring systems had been set up, including NHS Staff Survey, Commissioning for Quality and Innovation incentive scheme; mental health, musculoskeletal problem, and nutrition disorder and overweight were raised great concern; Health and Safety, and NHS Healthy Workforce Program were critical nationwide strategies. Conclusion: There were several similarities as well as differences between the Chinese public health system and NHS England, which laid foundation of learning by China. Recommendations of improving occupational health policies in China were provided, based on the lessons learned from the NHS England.

        Fluid-Structure Interaction Study on Diffuser Pump With a Two-Way Coupling Method

        In order to study the effect of the fluid-structure interaction (FSI) on the simulation results, the external characteristics and internal flow features of a diffuser pump were analyzed with a two-way flow solid coupling method. And the static and dynamic structure analysis of the blade was also caculated with the FEA method. The steady flow field is based on Reynolds Averaged N-S equations with standard $k-{\varepsilon}$ turbulent model, the unsteady flow field is based on the large eddy simulation, and the structure response is based on elastic transient structural dynamic equation. The results showed that the effect of FSI on the head prediction based on CFD really exists. At the same radius, the van mises stress on the nodes closed shroud and hub was larger than other nodes. A large deformation region existed near inlet side at the middle of blades. The strength of impeller satisfied the strength requirement with static stress analysis based on the fourth strength theory. The dynamic stress varied periodically with the impeller rotating. It was also found that the fundamental frequency of the dynamic stress is the rotating frequency and its harmonic frequency. The frequency of maximum stress amplitude at node 1626 was 7 times of the rotating frequency. The frequency of maximum stress amplitude at node 2328 was 14 times of the rotating frequency. No matter strength failure or fatigue failure, the root of blades near shroud is the key region to analyse.

        Iron-loaded Natural Clay as Heterogeneous Catalyst for Fenton-like Discoloration of Dyeing Wastewater

        The clay-based Fe-bearing catalyst was successfully prepared through ion-exchange reaction and applied as heterogeneous catalyst for discoloration of acid fuchsine (AF) in an aqueous solution by Fenton-like action.Experimental results demonstrated that the AF discoloration ratios increased by increasing Fe-loaded clay dosage and initial H2O2 concentration, and by decreasing the pH, respectively. The lower the initial AF concentration, the shorter the reaction time needed to achieve complete discoloration of AF. Comparative studies indicated that AF discoloration ratios were much higher in presence of Fe-loaded clay and H2O2 than those in presence of H2O2, raw natural clay or Fe-loaded clay only and raw natural clay and H2O2 jointly. After AF discoloration, there existed no new phases in the clay samples detected by XRD and no change in the clay crystal morphology observed by SEM. A mechanism proposed suggested adsorption and Fenton-like reaction were responsible for discoloration of AF.

        Thyroid-Associated Orbitopathy: Evaluating Microstructural Changes of Extraocular Muscles and Optic Nerves Using Readout-Segmented Echo-Planar Imaging-Based Diffusion Tensor Imaging

        Objective: We aimed to investigate the ability of readout-segmented echo-planar imaging (rs-EPI)-based diffusion tensor imaging (DTI) in assessing the microstructural change of extraocular muscles (EOMs) and optic nerves in patients with thyroidassociated orbitopathy (TAO) as well as in evaluating disease activity. Materials and Methods: We enrolled 35 TAO patients and 22 healthy controls (HCs) who underwent pre-treatment rs-EPIbased DTI. Mean, axial, and radial diffusivity (MD, AD, and RD) and fractional anisotropy (FA) of the medial and lateral EOMs and optic nerve for each orbit were calculated and compared between TAO and HC groups and between active and inactive TAO groups. Factors such as age, sex, disease duration, mediation, and smoking history between groups were also compared. Logistic regression analysis was used to evaluate the predictive value of significant variables for disease activity. Results: Disease duration was significantly shorter in active TAOs than in inactive ones (p < 0.001). TAO patients showed significantly lower FA and higher MD, AD, and RD than HCs for both medial and lateral EOMs (p < 0.001), but not the AD value of lateral EOMs (p = 0.619). Active patients had significantly higher FA, MD, and AD than inactive patients for medial EOMs (p < 0.005), whereas only FA differed significantly in the lateral EOMs (p = 0.018). The MD, AD, and RD of optic nerves were significantly lower in TAO patients than HCs (p < 0.05), except for FA (p = 0.129). Multivariate analysis showed that the MD of medial EOMs and disease duration were significant predictors for disease activity. The combination of these two parameters showed optimal diagnostic efficiency for disease activity (area under the curve, 0.855; sensitivity, 68.4%; specificity, 96.9%). Conclusion: rs-EPI-based DTI is promising in assessing microstructural changes of EOMs and optic nerves and can help to indicate the disease activity of TAO, especially through the MD of medial EOMs.

        Enhanced Removal of Phenol from Aquatic Solution in a Schorl-catalyzed Fenton-like System by Acid-modified Schorl

        Schorl modified by H₂SO4 has been successfully developed to enhance schorl-catalyzed Fenton-like reaction for removal of phenol in an aqueous solution. The phenol removal percentage can be increased from 4% to 100% by the system of modified schorl and H₂O₂. Batch experiments indicate that the percent increases in removal of phenol by increasing the dosage of catalyst, temperature and initial concentration of H₂O₂. The results of XRD, FT-IR and SEM suggest that no new phases are formed after removal of phenol by modified schorl. ICP-AES results reveal that more dissolution of iron results in higher catalytic oxidant activity in the system of modified schorl and H₂O₂. Besides minor adsorption, mineralcatalyzed Fenton-like reaction governs the process.
