        Weight Gain in Pregnancy, Maternal Age and Gestational Age in Relation to Fetal Macrosomia

        To investigate the possible risk factors related to macrosomia. Pregnant women and their newborns (n = 1041) were recruited from a cohort study in Maternal and Child Care Center of Hefei from January 2011 to July 2012. Questionnaires were applied to collect the demographic data besides the medical records. Detailed health records of the entire pregnancy were obtained using retrospective study. Meanwhile the data of neonatal outcomes was prospectively tracked. Associations between exposure risk factors and macrosomia were analyzed using Pearson’s chi squared test. Logistic regression models were used to assess the independent association between these potential predictors and macrosomia. The incidence of macrosomia of this cohort was 11.24% of which male: female = 2.55:1. Male incidence (8.07%) of macrosomia was higher than female (3.17%), p < 0.001. Body mass index (BMI) before pregnancy (pre-BMI), maternal height, parity were not independently associated with macrosomia; multiple logistic regression analysis indicated that macrosomia was mainly independently associated with weight gain in pregnancy (OR=1.14,95% CI [1.10-1.19]), maternal age (OR = 1.09,95% CI [1.03-1.15]) and gestational age (OR = 1.62,95% CI [1.31-1.99]), respectively. Our findings indicate that weight gain in pregnancy, maternal age and gestational age should be considered as independent risk factors for macrosomia.

        The Effect of Evening Primrose Oil for the Prevention of Xerotic Cheilitis in Acne Patients Being Treated with Isotretinoin: A Pilot Study

        Background: The most common adverse effects of oral isotretinoin are cheilitis, skin dryness, dry eyes, and conjunctivitis, whereas evening primrose oil (EPO) is known to improve skin moisture and transepidermal water loss (TEWL) in healthy adults and atopic patients. Objective: To evaluate the clinical efficacy and safety of EPO in preventing xerotic cheilitis in acne patients being treated with oral isotretinoin. Methods: Forty Korean volunteers of Fitzpatrick skin types III and IV, having moderate acne, were enrolled and randomized to receive either isotretinoin with or without EPO for 8 weeks. The efficacy of treatment was evaluated on the basis of global acne grading system scores, number of inflammatory and noninflammatory lesions, TEWL, corneometry, physician’s global assessment, and patient satisfaction. Results: The results after 8 weeks of treatment showed that the TEWL of the lip increased significantly during isotretinoin treatment, whereas the TEWL of the hand dorsum showed no significant change. The increase of the TEWL of the lip was more definite in the control group than in the experimental group. The number of acne lesions decreased significantly in both groups, and there were no differences between them. Conclusion: Our study suggests that the addition of EPO improved xerotic cheilitis in acne patients being treated with oral isotretinoin. However, besides TEWL and corneometry assessments, additional studies are required for a complete understanding of the role of EPO in xerotic cheilitis in acne patients being treated with oral isotretinoin.         Muscle-derived Stem Cells Differentiate into Functional Smooth Muscle Cells for Ureter Tissue Engineering: An Experimental Study

        We assessed the ability of muscle-derived stem cells (MDSC) to differentiate into smooth muscle cells (SMC) and their potential to promote the regeneration of smooth muscle with a vessel extracellular matrix (VECM)for tissue engineering of the ureter. MDSC were isolated,proliferated, and identified by flow cytometry. SMC phenotype differentiation was induced with a smooth muscle induction medium. Gene expression was evaluated by real-time quantitative polymerase chain reaction (PCR)and Western blot studies. The VECM was obtained by a decellularization process, and cytotoxic effects were evaluated by exposing the induced cells to a VECM extract. The induced cells were seeded onto VECM in vitro for 1 week, and then the compound grafts were used for ureter reconstitution in vivo. The grafts were obtained for histological studies at 2, 4, 8, and 16 weeks post-operation. Intravenous urography was used to evaluate renal function and ureteral patency. Flow cytometry demonstrated that the MDSC expressed Sca-1 and desmin, but did not express CD45. After induction, SMC phenotype gene expression was confirmed in the induced cells by real-time quantitative PCR and Western blot studies. VECM exhibited a nontoxic effect on the induced cells in vitro. At 16 weeks postoperation,a histological evaluation showed that multilayered urothelium and organized muscle fiber bundles had formed in the grafts. Intravenous urography demonstrated no evidence of ureteral stricture or hydroureteronephrosis. These results demonstrate that MDSC can be induced into SMC and that this was useful for promoting regeneration of smooth muscles for ureter tissue engineering.         Bullous pemphigoid (BP) is a chronic autoimmune subepidermal blistering disease that usually occurs in the elderly. Two types of clinical variants exist. The generalized form typically involves the lower abdomen, groin, thighs and inner arms. The localized forms of BP include (1) mucous membrane pemphigoid or cicatrical pemphigoid, (2) localized scarring pemphigoid or Brunsting-Perry pemphigoid, and (3) localized nonscarring pemphigoid. We report here on a case of localized nonscarring pemphigoid on the face of a 71-years-old man. (Korean J Dermatol 2008; 46(11): 1526∼1529)

        The skin lesion of herpes zoster is classically limited to single dermatomal area, and most cases of multidermatomal herpes zoster have contiguous skin lesions. Noncontigous multidermatomal herpes zoster is very rare in both immunocompetent and immunocompromised persons. The phenomenon of zoster occurring in two noncontiguous dermatomes has been referred to as zoster duplex unilateralis or bilateralis. We report a case of herpes zoster multiplex in five disparate dermatomes in a 71-year-old woman with non-Hodgkin`s lymphoma. (Korean J Dermatol 2008;46(7):973~976)

      • The role of collagen VI α6 chain (COL6A6) gene in atopic dermatitis

        Background: Collagen VI α6 chain (COL6A6) is one of the chains that makes up the triple helix of collagen VI. In our previous study, we found that family-specific COL6A6 gene polymorphisms are associated with early onset of atopic dermatitis (AD) in Koreans. Objectives: This study aimed to identify how COL6A6 changes in AD and its role. Methods: Histologic examination was performed in patients with AD, psoriasis, papular urticaria, and pityriasis rosea and controls. To determine the role of COL6A6, COL6A6 small interfering RNA (siRNA) was transfected into human keratinocytes and fibroblasts. RNA sequencing and real-time PCR were performed for mRNA expression analysis. Results: Total COL6A6 mRNA expression was higher in AD skin than in control skin. COL6A6 protein levels increased in the dermis of AD skin compared to controls, whereas a reduction in COL6A6 mRNA expression was observed in the basal layer of the epidermis. IL-4 and IL-13 induced COL6A6 protein expression in the dermis, while they suppressed COL6A6 mRNA expression in the epidermis. In the COL6A6 siRNA-transfected keratinocyte model, mRNA expressions of FLG, LORICRIN, and CASP14 decreased compared to controls. In contrast, in the COL6A6 siRNA-transfected fibroblast model, the mRNA expression of MMP1 increased. Conclusion: Our study suggests that COL6A6 induced by IL-4 and IL-13 may play a role in fibrotic remodeling and inflammatory processes, which are major features of AD.

        The Short Stature in Atopic Dermatitis Patients: Are Atopic Children Really Small for Their Age?

        Background: Short stature is sometimes seen in children with atopic dermatitis (AD); however, the topic has never been studied systematically. Objective: The aim of this study was to show whether AD itself affects stature in children and to evaluate the influence of other relevant factors such as genetic background, diet restrictions, and sleep disturbance on the stature of children with AD. Methods: The study population included Korean children 7 to 8 years of age who live in one district of Seoul, Korea. We used a questionnaire as an investigating tool to survey genetic backgrounds, environmental factors, and comorbidities. Student`s t-test and linear regression were employed for statistical analysis. Results: In univariate analysis, the average stature in the AD group was short compared with the normal control group. Parental stature, dietary habit, and sleep patterns were also relevant factors with respect to stature. However, in multivariate analysis, AD itself had no influence on stature. Significant correlations were found for such factors as parental height, sleep disturbance, presence of asthma, and dietary restrictions, in decreasing magnitude. Conclusion: These results suggest that AD itself may not be the causative factor for short stature in children with AD. Therefore, consideration of other relevant factors related to short stature in patients with AD will be important for the proper management of the disease. (Ann Dermatol 25(1) 23∼27, 2013)

        A Tender Fingertip Papule in an Elderly Woman

        A 65-year-old Korean woman presented with a hyperkeratotic, firm, tender, and yellowish papule measuring 3 mm in diameter, which first appeared on the tip of her right thumb 2 months prior to presentation (Fig. 1A). She related a history of trauma at this site from an unknown sharp object at approximately the same time the lesion appeared. The lesion was clinically diagnosed as a foreign body reaction, excised with a skin punch, and examined histologically. Hematoxylin-eosin staining revealed that the wall of the cyst was lined with squamous cells, and the cavity was composed of keratinized lamellar material (Fig. 1B). We confirmed the diagnosis of an epidermal cyst on the distal tip of her thumb. Postoperative follow-up was unremarkable, without any complications or recurrence. Epidermal cysts have been termed infundibular cysts, suggesting that they arise from the infundibular portion of the hair follicle. Therefore, these cysts most commonly occur on hair-bearing areas of the body such as the scalp, face, neck, trunk, and scrotum. Additionally, they may rarely occur following traumatic implantation of epidermal elements into the dermis particularly on the palms and soles<sup>1-3</sup>. Burial of tissue, a secondary proliferative response to blunt trauma, and inflammation have been described as possible pathomechanisms for the development of traumatic epidermal cysts<sup>4</sup>. Human papillomavirus (HPV) infections may play a role in causing epidermal cysts. Egawa et al. have reported a case wherein an HPV-associated epidermal cyst was observed to have developed following epidermoid metaplasia of the eccrine duct epithelium<sup>5</sup>. Because our patient had a positive history of trauma with a sharp object, her cyst was clinically diagnosed as a foreign body reaction. Although histopathological examination revealed that it was an epidermal cyst, the precise mechanism of development of the cyst remains unclear. The time interval between the trauma and appearance of the lesion was too short for the growth of an epidermal cyst. Thus, we hypothesize that a traumatic or HPV-associated epidermal cyst already existed in this patient and subsequently increased in size following trauma, which acted as a trigger. Occurrence of epidermal cysts on fingers is extremely rare, and they can easily be misdiagnosed/confused with viral warts or calluses. Other differential diagnoses that should be considered for such lesions on the fingertip are ganglion cysts, mucous cysts, acquired digital fibrokeratomas, trichilemmal cysts, and glomus tumors<sup>6,7</sup>. This case highlights an unusual clinical manifestation of an epidermal cyst at an unexpected location. To the best of our knowledge, this is the first case of a hyperkeratotic, firm papule on the fingertip of a patient, which was diagnosed as a traumatic epidermal cyst. It is important that dermatologists should perform a careful physical examination without assuming a clinical diagnosis to correctly diagnose and appropriately treat papular lesions of the fingers.
