        Outcome of Salvage Lumbar Fusion after Lumbar Arthroplasty

        Study Design: Retrospective review. Purpose: This study aims to define the role of lumbar fusion for persistent back pains after the lumbar disc replacement. Overview of Literature: Little is written about lumbar fusion after optimally placed lumbar arthroplasty in patients with persistent lower back pains. Methods: Retrospective review of cases of lumbar artificial disc requiring subsequent fusion because of persistent back pains despite optimally placed artificial discs. Outcomes were evaluated using Oswestry Disability Index (ODI) and visual analogue scale (VAS). Clinical improvements indicated 25% improvement in ODI and VAS values. Results: Five patients met the study criteria. The mean baseline ODI for the five patients was 52. The mean baseline VAS scores for back and leg pains were 76 and 26, respectively. All the five patients had optimally placed prosthesis. The indication for surgery was the constant low back pains found in all the patients. Revision surgery involved disc explantation and fusion in two of the patients and posterolateral fusion without removing the prosthesis in three. None of the patients achieved adequate pain control after the revision surgery despite the solid bony fusion documented by postoperative computed tomography. The mean ODI value after the fusion was 55. The mean values for back and leg pains VAS were 72 and 30, respectively. Conclusions: Lack of good pain relief after successful lumbar artifical disc replacements may indicate different etiology for the back pains. The spine-treating surgeons should have a high threshold level to perform salvage fusion at that level.

        A HEV evaluation program, funded by ADEME, was carried out by a group of Laboratories of different specialties in order to evaluate and compare consumption, emission and component technologies of the three first HEVs put on the market (Toyota Prius, Nissan Tino and Honda Insight). This paper presents the results obtained until now. These results show good consumption and emission performance of the tested vehicles compared to conventional ones. The energy management seems to be globally the same for the three vehicles excepting for cold starts where the Insight allows a very earlier stop of the engine compared to the Tino and especially to the Prius. A mapping of the engine consumption of the Prius and the Insight was performed in order to furnish data for the simulation models. The Permanent Magnet motors of the Prius and Tino have different number of pair poles and then different emf at a given speed. The low emf values of the Prius allow operation at high speed with less field weakening control than for the Tino. The inverters of the Prius and the Tino, controlled by a PWM at respectively 5 kHz and 7 kHz. switching frequency, are made of IGBTs with high commutation performances.

        Utility of FDG-PET/CT for the Detection and Characterization of Sternal Wound Infection Following Sternotomy

        Purpose FDG-PET/CT has the potential to play an important role in the diagnosis of sternal wound infections (SWI). The purpose of this study was to analyze the diagnostic accuracy of FDG-PET/CT for SWI in patients following sternotomy. Methods We performed a single-center, retrospective analysis of patients who had undergone median sternotomy and FDG-PET/ CT imaging. The gold standard consisted of positive bacterial culture and/or the presence of purulent material at surgery. Qualitative patterns of sternal FDG uptake, SUVmax, and associated CT findings were determined, and an imaging scoring system was developed. The diagnostic performances were studied in both the recent (≤ 6 months between sternotomy and imaging) and remote surgery phase (> 6 months). Results A total of 40 subjects were identified with 11 confirmed SWI cases. Consensus interpretation was associated with a sensitivity of 91%and specificity of 97%. Combination of uptake patterns yielded an AUC of 0.96 while use of SUVmax yielded an AUC of 0.82. Conclusions Results suggest that FDG-PET/CT may be useful for the diagnosis of SWI with optimal diagnostic accuracy achieved by identifying specific patterns of uptake. SUVmax can be helpful in assessing subjects with remote surgery, but its use is limited in the context of recent surgery. Further studies are required to confirm these results.
