      • Development of a Screening Swallowing Algorithm for Using Ultrasonic Doppler Sensor and Microphone

        Objective: The present study is to develop an algorithm for screening only swallowing movements out of various laryngopharyngeal movements using ultrasonic Doppler sensor and microphone. Background: Only swallowing movements need to be discriminated out of various laryngopharyngeal movements (e.g., vocalization, cough) measured by a swallowing measurement system using ultrasonic Doppler sensor for specialization in swallowing research. Method: laryngopharyngeal movement and audio signals during swallowing, cough, vocalization, respiratory, and neck movement were simultaneously measured three times to five healthy adults by ultrasonic Doppler sensor and microphone. A swallowing screening algorithm calculating movement-to-audio ratio applied with swallowing apnea was developed for screening only swallowing movements. Results: Vocalization, cough, and respiratory movements were perfectly discriminated with swallowing movements by the swallowing screening algorithm; however, neck rotation movements 73% due to occurring no laryngopharyngeal movements. Conclusion: The swallowing screening algorithm developed in the study could discriminate swallowing movements from vocalization, cough, and respiratory movements. Application: The swallowing screening algorithm can be applied to the swallowing measurement device and contribute to accurate evaluation of swallowing movements for patients with dysphagia.

        가성무시(pseudoneglect)란, 신경학적 손상이 없는 정상인들이 무시증후군(visuospatial neglect)을 탐지하는데에 가장 많이 사용하는 선 이등분(line bisection)검사에서 좌측으로 치우쳐서 중심점을 표시하는 현상으로써, 우반구 손상을 입은 환자들이 공간의 좌측을 무시하고 중심점 우측으로 선을 이등분하는 것과는 반대된다. 본 연구는 시각적 주의에 영향을 미치는 가성무시현상에 대해 선 이등분과 선 사등분 전략을 적용한 도로에서의 모의운전(driving simulation)과 지필검사를 이용하여 확인하였다. 지필검사에서 선 이등분점과 좌측 사등분점이 유의미하게 좌측으로 치우친 반면, 우측 사등분 점은 유의미한 좌측 치우침을 보이지 않았다. 모의운전에서는 차량의 우측 치우침이 도로 조건과 관계없이 나타났으나 각 도로에 대한 상대적 치우침을 분석한 결과, 선 이등분 조건의 도로 1이 가장 많은 좌측 치우침을, 우측 선 사등분 조건의 도로 2가 가장 적은 좌측 치우침을 나타내어 지필검사와 모의운전의 결과가 유사하며 모두 좌측 치우침 현상이 있음을 확인했다. 또한 우측 치우침에 대한 원인으로 중앙선 회피효과를 분석한 결과, 중앙선 우측에서 주행한 도로 2와 4에서 회피경향이 나타났다. 본 연구에서는 가성무시가 운전 중에 유의미하게 나타남으로써 일상생활에서도 이와 같은 현상이 존재함을 나타냈다. This study's aim was to confirm that pseudoneglect affects visual attention through car laterality, using a driving simulator with either bisection or quadrisection strategies being applied to road usage. On the pencil and paper tests, the left quadrisection and bisection marks deviated significantly to the left. While driving, the car was significantly lateralized to the right of the lane regardless of conditions. However, in terms of relative laterality, the biggest left laterality occurred on roads 1(bisection), while the smallest left laterality occurred on roads 2 (right quadrisection). Thus, the effect of pseudoneglect was demonstrated in both the pencil and paper tests and the driving simulation task. Also, roads 2 and 4, which were driven on the right side of the lane, showed a tendency for drivers to avoid the centerline, as this was the expected cause of right laterality. This study demonstrated that the pseudoneglect phenomenon can occur in a routine driving task.

      • Development of a Musical Brain Fitness Program for Social, Physical, Emotional, and Cognitive Capabilities of Seniors

        A steady increase in the number of people with dementia along with aging requires a counterplan to help the aged effectively manage their cognitive capabilities. Although many brain fitness systems have been developed, seniors have had difficulty in utilizing them due to a lack of usability and customization. The present study developed a medical entertainment (meditainment) program for brain fitness of seniors. The brain fitness meditainment program, called Smart Harmony, was developed by a five-step process: (1) needs survey, (2) benchmarking, (3) idea development, (4) prototyping, and (5) usability testing. First, desirable values and user needs for brain fitness were investigated. Second, existing brain fitness programs were benchmarked. Third, novel design concepts were generated and selected. Fourth, the selected concepts were prototyped. Lastly, iterative usability tests were conducted for refinement. Smart Harmony is a musical game which can enhance the social, physical, emotional, and cognitive (SPEC) activities of users. The features of Smart Harmony includes (1) played by up to seven users, (2) eight musical instruments such as piano and violin and numerous favorite songs for selection, and (3) scores by brain age of the team and individuals for motivation. Smart Harmony is expected to be utilized in welfare centers to improve seniors’ SPEC capabilities. A clinical evaluation is needed to examine the validity of Smart Harmony for brain fitness.
